Metodologija rada:Prospektivni observacioni trial Evropskog Resuscitacionog saveta. Program organizovan od strane Resuscitacionog saveta Srbije a na osnovu ovlašćenja Evropskog resuscitacionog saveta. Saradnja sa Sekcijom urgentne medicine. U studiju su uključeni svi pacijenti -i odrasli i deca, koji su zatečeni mrtvi u vanbolničkim uslovima, a kod kojih je intervenisala hitna medicinska pomoć. Podaci su prikupljani tokom jednogodiš-njeg perioda (2015 godina). Popunjavanje ankete -lekara nakon izvršene intervencije na terenu. RezultatiŠokabilan inicijalni ritam je imalo 22 pacijenta (15.5/100.000). Dvadeset osoba muškog pola (14.1/100.000) i 4 žene (2.8/100.000). Tokom primene postupaka reanimacije u dva slučaja se ritam iz nešokabilnog prevodi u šokabilan. Prosečna starost pacijenata sa šokabilnim ritmom je 65.6 godina. Proseč-no vreme od prijema poziva u dispečerskom centru do primene DC šoka je 9.8 min. Dispečer je tri puta uspeo telefonski da vodi KPR uz navođenje laika o postupcima osnovne životne podrš-ke do dolaska lekarske ekipe. Ekipa je dva puta zatekla svedoka kako radi reanimaciju uz primenu "samo kompresije", dok je u jednom slučaju laik odustao od primene KPR i taj je pacijent imao ne šokabilan inicijalni ritam. Od ukupnog broja pacijenata sa šokabilnim ritmom, ROSC je postignut kod 11 (7.8/100.000) pacijenata i oni su sa opipljivim pulsom predati Odeljenju za prijem i zbrinjavanje urgentnih stanja Opšte bolnice Subotica. Iz bolnice je otpušteno 5 (3.5/100.000) pacijenata dok se za 4 (2.8/100.000) pacijenta zna da su živi i nakon 30 dana po otpustu.
Bolesti srca su osnovni uzrok u 2/3 vanbolničkih srčanih zastoja. Iznenadni srčani zastoj nastaje kod osoba bez prethodnog potvrđenog kardiološkog oboljenja, a dešava se u situacijama u kojima je moguće prisustvo očevidaca i tada je neophodna njihova saradnja. Nije retkost da se u medijima pojavljuju vesti, kao i na društvenim mrežama snimci o mladim sportistima ili javnim ličnostima koji su iznenada izgubili svest na sportskom terenu ili na poslu. I pored malo istraživanja koji se bave ovom problematikom, u literaturi se mogu naći podaci koji ukazuju da je znatno bolji ishod nakon iznenadnog srčanog zastoja ukoliko se zastoj dogodi na poslu, nego na bilo kojem drugom mestu. Uključivanjem u studiju EURECA One 2014. Evropskog resuscitacionog saveta u organizaciji Resuscitacionog saveta Srbije, prvi put smo i za našu sredinu dobili relevantne podatke iz te oblasti. METODProspektivna studija prikupljanja podataka po jedinstvenom upitniku u vremenskom periodu od 01. do 31. oktobra 2014. godine na teritoriji opštine Subotica. U studiju su uključeni svi pacijenti kod kojih je zabeležen srčani zastoj u vanbolničkim uslovima i intervenisala ekipa hitne medicinske pomoći. REZULTATISrčani zastoj je potvrđen kod 9 pacijenata od strane hitne medicinske pomoći u toku meseca oktobra 2014. godine -incidenca 6.35/100.000. Od 9 potvrđenih srčanih zastoja 8 je primljeno kao prvi red hitnosti, a 1 kao drugi red hitnosti. Mere kardiopumonalne resuscitacije započete su kod 8 (89% od svih pacijenata kod kojih je Hitna medicinska pomoć osvedočila srčani zastoj). Incidenca 5.65 na 100.000 stanovnika. Pretpostavljeni kardiološki uzrok srčanog zastoja predhodio je kod svih 9 pacijenata . Incidenca kardioloških uzoka je 6.35/100.000 pacijenata. Kada je svedok bio prisutan, mere resuscitacije su započete kod 6 (75%), ili 4.23/100000 pacijenata, međutim ni jedna resuscitacija nije započeta od strane laika već isključivo od strane Službe hitne medicinske pomoći nakon pristizanja na intervenciju. Ritam kod kojih je proporučena isporuka DC šoka, ventrikularna fibrilacija ili ventrikularna tahikardija bez pulsa kao prvi ritam zabeleženi su kod 3 (37.5%) ili kod 2.12/100000 pacijenata. Povratak spontane cirkulacije (ROSC) zabeležen je kod 2 (25%) odnosno 1.41/100000. Otpust iz bolnice utvrđen je za jednog pacijenta što predstavlja incidencu od 0.71/100000 stanovnika. Preživljavanje nakon mesec dana zabeleženo je kod jednog pacijenta ili 0.71/100000 stanovnika. ZAKLJUČAKPrikupljanje podataka samo u jednom mesecu je suviše kratak vremenski period te je nemoguće donositi zaključke oko ishoda iznenadnog srčanog zastoja u našoj Opštini, zbog malog posmatranog uzorka. Potrebno je nastaviti sa prikupljanjem podataka, te se nakon dugoročne opservacije mogu očekivati rezultati koji će doprineti poboljšanju kvaliteta rada naše ustanove u zbrinjavanju pacijenata sa srčanim zastojem.
