Contractures of the hand are rather uncommon conversion phenomena. As yet, very little is known about treatment. After a brief overview of the literature on the various treatment strategies, we describe the treatment of four patients. There are indications that a complaint-centered approach, which attempts to influence the symptom directly, produces positive results. Hypnotherapeutic techniques were used in the four patients described here. Treatment resulted in improvement in three of the four.

Teasdale probeerde een meditatietechniek te illustreren die behulpzaam zou zijn bij terugvalpreventie van depressie.Het boek Mindfulness-based cognitive therapy for depression gaat over terugvalpreventie bij depressie. Het is geen handboek, maar een ontdekkingstocht van de schrijvers naar methoden om recidieven te voorkomen. Dat heeft als nadeel dat het boek geen systematisch overzicht geeft van manieren om depressie te voorkomen. Er staat bijvoorbeeld niets geschreven over de mogelijkheden van medicatie of interpersoonlijke psychotherapie. Het heeft ook een voordeel: de schrijvers weten de lezers mee te slepen in hun eigen ontwikkeling, hun dilemma's en overwegingen. Dat maakt dat zeker het eerste deel van het boek leest als een spannend jongensboek.De eerste vier hoofdstukken gaan over de ontwikkeling die de auteurs meemaakten. Sinds de jaren tachtig van de vorige eeuw werd steeds duidelijker dat depressie een chronisch recidiverende ziekte is. Daarmee verschoof de vraagstelling van 'hoe genees je een depressie', naar 'hoe voorkom je de volgende depressieve episode?' Uit onderzoek kwam naar voren dat patie¨nten die behandeld waren met cognitieve therapie, langer van depressie gevrijwaard werden dan patie¨nten die anders behandeld werden. Iets in de cognitieve therapie werkte preventief, maar wat?De gangbare cognitieve theorie over depressie gaat uit van de kwetsbaarheidhypothese. In hun jeugd verwerven patie¨nten cognities die hen kwetsbaar maken op het moment dat levensomstandigheden deze cognities activeren. De consequentie van deze hypothese is dat patie¨n-ten ook in hun niet-depressieve episoden tekenen van kwetsbaarheid zouden moeten vertonen. Er is uitvoerig onderzocht of patie¨nten in hun niet-depressieve episoden hierin verschillen van 'normalen'. Dat blijkt niet zo te zijn. Ex-depressieve patie¨nten verschillen niet van anderen. Zij lijken niet kwetsbaarder.De situatie verandert dramatisch op het moment dat deze patie¨nten in een dip raken. De theorie die je de 'diptheorie' kan noemen, stelt dat negatieve gedachtepatronen geactiveerd worden en beschikbaar komen in een mild-depressieve stemming. In een experimentele conditie bleek het luisteren naar sombere muziek voldoende te zijn om een dip te veroorzaken. Op dat moment komt een lawine van negatieve gedachten en emoties los. Exdepressieven blijken hun negatieve emoties slechter te kunnen relativeren dan 'normalen'. Samenvatting Dit artikel gaat over de theorie en praktijk van de behandeling van patie¨nten met een obsessieve stoornis. Volgens de cognitieve theorie zijn het niet de intrusieve gedachten die het ongemak veroorzaken, maar de interpretatie van de intrusies. De interpretatie leidt tot angst en zet een proces in werking dat de intrusies versterkt. Er zijn verschillende therapiee¨n voor obsessies, die kort besproken worden. We gaan uitgebreid in op de cognitieve therapie bij obsessies, alsmede misinterpretaties die een rol spelen, zoals een overmatig gevoel voor verantwoordelijkheid, de overschatting van de betekenis van gedachten, perfectionisme, de overschatting van de kans op en ernst van negatieve gevolgen. Er zijn veel voorbeelden van en suggesties over hoe men de behandeling kan uitvoeren. Ten slotte komen valkuilen aan bod.
