Samenvatting Dit artikel gaat over de theorie en praktijk van de behandeling van patie¨nten met een obsessieve stoornis. Volgens de cognitieve theorie zijn het niet de intrusieve gedachten die het ongemak veroorzaken, maar de interpretatie van de intrusies. De interpretatie leidt tot angst en zet een proces in werking dat de intrusies versterkt. Er zijn verschillende therapiee¨n voor obsessies, die kort besproken worden. We gaan uitgebreid in op de cognitieve therapie bij obsessies, alsmede misinterpretaties die een rol spelen, zoals een overmatig gevoel voor verantwoordelijkheid, de overschatting van de betekenis van gedachten, perfectionisme, de overschatting van de kans op en ernst van negatieve gevolgen. Er zijn veel voorbeelden van en suggesties over hoe men de behandeling kan uitvoeren. Ten slotte komen valkuilen aan bod.
Het thema evidence-based medicine, of meer algemeen evidence-based practice (EBP), is erg actueel maar ook controversieel (zie de reeks artikelen in PsychoPraxis 2001). Er is vooreerst het wantrouwen van mensen in het werkveld tegenover onderzoekers in researchinstituten en hun als zo valide gepresenteerde resultaten, die nadien worden herroepen. Deze terughoudendheid bij hulpverleners zit diep geworteld. De regel primum non nocere (breng geen schade toe), de eigen ervaring en gevoelens van persoonlijke verantwoordelijkheid ten aanzien van clie¨nten/patie¨nten zijn aan deze houding niet vreemd. EBP staat in veler ogen daarom wellicht even goed voor educationally biased of environmentally biased. Hoe vaak zien wij immers niet het gezegde 'wiens brood men eet, diens woord men spreekt' bewaarheid? En als dit voor hulpverleners geldt, dan allicht ook voor patie¨nten/clie¨nten. Ze verdedigen de persoon die hen naar hun eigen mening heeft geholpen, zelfs wanneer dit manifest in tegenspraak is met de feiten. Juist patie¨nten/clie¨nten ontpoppen zich op dit ogenblik tot stevige gesprekspartners en soms treden ze als echte pressiegroep naar buiten. Geklemd tussen patie¨nten en zorgverzekeraarsGeklemd tussen patie¨nten/clie¨nten enerzijds en zorgverzekeraars anderzijds kiest een therapeut wellicht sneller voor de zekerheid van het bekende. Hier lijkt EBP een houvast te bieden voor de eigen gevoelens van onzekerheid. Met de cijfers in de hand kun je immers je gelijk aantonen: waarom je dat wel of dat net niet doet of zelfs moe´t doen of nalaten. Toon je het niet zelf aan, dan komen later patie¨nten/clie¨nten of zorgverzekeraars je het wel uitleggen. Ook al ligt het niet zo eenvoudig, dan nog kijkt iedere therapeut op tegen een dergelijke confrontatie. Het open conflict met collega's en a fortiori clie¨nten/patie¨nten wordt immers zo veel mogelijk vermeden en buiten de psychotherapeutische werkruimte gehouden.Wat zou men dan nog verwachten van zorgverzekeraars? Zou het hun werkelijk een zorg zijn? Als hun klanten tevreden zijn, dan neemt het algemene ziekterisico immers beduidend af. En verminderen klachten ook niet ondanks aanhoudende last en hinder? De betekenis van economische factoren in de bewijsvoering lijkt een centrale rol te spelen. Daarom zou EBP ook kunnen staan voor economically biased. Maar is het allemaal wel zo eenvoudig?De vakliteratuur wekt soms de indruk dat er op een bepaald moment geen ruimte meer zal zijn voor andere hulpverlening dan EBP. Onszelf voorhouden dat we hulpvragen enkel uit de wereld kunnen helpen met EBP lijkt me echter naı¨ef en pretentieus. Ziektekostenverzekeraars in Belgie¨bijvoorbeeld overwegen of ze voor hun leden ook homeopathische middelen moeten vergoeden. Ze zijn immers goedkoper, en na aanwending ervan -ik zeg niet door aanwending ervan -worden mensen ook beter. Positieve wetenschappers beweren dat het geen effect en dus ook wel geen schadelijk effect heeft, tenzij door uitstel van 'echte' behandeling.
