Objective:This retrospective multicenter study, centrally conducted and supported by the Society of Endocrinology and Metabolism of Turkey, aimed to evaluate the impact of free RET proto-oncogene testing in medullary thyroid carcinoma (MTC) patients. Surgical timing, adequacy of the treatment, and frequency of prophylactic thyroidectomy (PTx) in mutation carriers were also assessed.Methods:Genetic testing for MTC and pheochromocytoma was conducted between July 2008 and January 2012 in 512 patients. Application forms and RET mutation analyses of these patients whose blood samples were sent from various centers around Turkey were assessed retrospectively. An evaluation form was sent to the physicians of the eligible 319 patients who had confirmed sporadic MTC, familial MTC (FMTC), multiple endocrine neoplasia type 2 (MEN2), or who were mutation carriers. Physicians were asked to give information about the surgical history, latest calcitonin levels, morbidity, mortality, genetic screening, and PTx among family members. Twenty-five centers responded by filling in the forms of 192 patients.Results:Among the 319 patients, RET mutation was detected in 71 (22.3%). Cys634Arg mutation was the most prevalent mutation (43.7%), followed by Val804Met in 18 patients (25.4%), and Cys634Tyr in 6 patients (8.5%). Among 192 MTC patients, the diagnosis was sporadic MTC in 146 (76.4%), FMTC in 14 (7.3%), MEN2A in 15 patients (7.9%), and MEN2B in one patient. The number of mutation carriers among 154 apparently sporadic MTC patients was 8 (5.2%). Ten patients were submitted to PTx out of twenty-four mutation carriers at a mean age of 35±19 years.Conclusion:Turkish people have a similar RET proto-oncogene mutation distribution when compared to other Mediterranean countries. Despite free RET gene testing, the number of the PTx in Turkey is limited and relatively late in the life span of the carriers. This is mainly due to patient and family incompliance and incomplete family counselling.
Hipofiz adenomlarının sınıflamasında daha sensitif immünohistokimyasal ve ultrastrüktürel tekniklerin kullanılması ile hormon immünreaktivite gösteren adenomların insidansında artış olduğu belirtilmektedir. Bu çalışmada akromegali nedeniyle opere edilen hastaların immünohistokimyasal inceleme sonuçları ile klinik ve laboratuvar bulguları arasındaki ilişkiyi araştırdık. Gereç ve Yöntem: Hastanemizde akromegali nedeni ile opere edilen 110 hastanın verileri geriye dönük olarak tarandı. Kırk üç hastanın tetkikleri eksik olduğundan çalışma dışı bırakıldı. Tüm hastalar klinik ve laboratuvar olarak operasyon öncesi değerlendirilmiştir. Bulgular: Hastaların 42'si kadın, 25'i erkekti. Otuz hastada sadece büyüme hormonu boyanması varken kalan 37 hastada plurihormonal boyanma saptandı. Tiroid uyarıcı hormon (TSH) boyanması olan 14 hastanın üçüne preoperatif olarak TSH salgılayan hipofiz adenomları tanısı konmuştu. Bu üç olgu dışında klinik ve laboratuvar olarak hormon sekrete eden adenom saptanmadı. Tartışma: Çalışmamız akromegali hastalarında immünohistokimyasal olarak saptanan boyanma paternleri ile klinik ve laboratuvar bulguları arasında ilişki olmadığını gösterdi.
Purpose:The frequency of thyroid surgery for suspected malignancy but with a benign result in pathological examination is increasing in recent years. For this reason, additional preoperative markers are needed for increasing the sensitivity for evaluating the preoperative malignancy risk of thyroid nodules. In this study, we aimed to evaluate the diagnostic value of serum TSH levels for determining the differentiated thyroid cancers (DTC) and to identify a proper cut-off value if relevant association is present. Material and Method: Our study included 380 patients who underwent thyroidectomy due to nodular goiter in our hospital between 01.01.2012 and 01.06.2013 retrospectively. 201 patients who were diagnosed with DTC constituted the study group, and 179 consecutive patients with a benign pathology result were included as controls. Patients who had overt hyperthyroidism or hypothyroidism and was taken medicines that affect TSH level were excluded. Results: There were no significant differences between the two groups in terms of age, sex, and family history of thyroid disease. Preoperative TSH levels were 1.66 mIU/lt and 1.59 mIU/lt in patients with DTC and controls, respectively (p=0.641). There was no correlation between TSH and tumor size, and no relationship between TSH and capsular invasion, vascular invasion, extrathyroidal invasion and lymph node metastasis.Amaç: Günümüzde giderek artan sıklıkta malignite şüphesi nedeni ile yapılan ancak patoloji sonucu benign saptanan tiroid operasyonları yapılmaktadır. Bu nedenle tiroid nodüllerinin preoperatif malignite riskini değerlendirmek için sitoloji ile birlikte duyarlılığını arttıracak ek preoperatif belirteçlere ihtiyaç duyulmaktadır. Bu çalışmada diferansiye tiroid karsinom tanısı alan hastaların preoperatif serum TSH düzeyinin diferansiye tiroid kanseri için tanısal değerini araştırmak ve anlamlı ilişki varsa uygun bir tanısal eşik değer saptamak amaçlanmıştır. Gereç ve Yöntem: Çalışmamıza 01.01.2012-01.06.2013 yılları arasında hastanemizde tiroidektomi yapılıp patoloji sonucu diferansiye tiroid kanseri (DTK) saptanan 201 hasta ve patoloji sonucu benign saptanan 179 birey kontrol grubu olarak dahil edildi. Aşikar hipotiroidi veya hipertiroidi olan hastalar ve tiroid fonksiyonlarını etkileyen ilaç tedavisi alan hastalar çalışma dışı bırakıldı. Bulgular: Çalışmamıza alınan iki grup arasında yaş, cinsiyet ve ailede tiroid kanseri öyküsü açısından fark yoktu. Diferansiye tiroid kanseri olan hastalarda preoperatif TSH düzeyi (TSH: 1,66 mIU/lt) kontrol grubu (TSH: 1,59 mIU/lt) ile benzer saptandı (p=0,641). Ayrıca tiroid kanseri olan grupta TSH düzeyi ile tümör boyutu, kapsül invazyonu, damar invazyonu, ekstratiroidal yayılım ve lenf nodu tutulumu arasında ilişki saptanmadı. Tartışma: Çalışmamızda TSH düzeyi ile DTK arasında ilişki bulunamamıştır. Ayrıca tiroid kanseri olan grupta TSH düzeyi ile kötü prognostik belirteçler arasında ilişki saptanmadı.
