Objetivo: analisar a utilização do cateter central de inserção periférica em neonatos. Métodos: estudo retrospectivo, documental. Para coleta de dados, utilizou-se de instrumento para registro e acompanhamento de cateter central de inserção periférica. Análise realizada por meio de estatísticas descritivas para obtenção de frequências absolutas, relativas, valores mínimos e máximos, desvios-padrão e médias e teste de qui-quadrado para k-proporções para variáveis qualitativas. Resultados: foram analisados 383 registros. O cateter foi indicado, principalmente, para administração de antibioticoterapia (46,5%). O vaso mais acessado foi a veia cefálica (23,5%). A complicação mais incidente foi de progressão (6,8%), com prevalência de remoção por término de tratamento/indicação (25,3%). Conclusão: cateter utilizado, principalmente, para administração de antibioticoterapia; veia cefálica mais acessada, com uso de Fentanil para atenuar dor e desconforto; progressão, complicação mais incidente; e maior índice de remoção por término de tratamento/indicação.
Nº 48 Octubre 2017Página 465 ORIGINALES Búsqueda activa contribuye a la identificación de eventos adversos e incidentes en unidad de cuidados intensivos RESUMEN:Objetivos: Analizar los eventos adversos e incidentes en la unidad de cuidados intensivos de un hospital universitario y comprobar cuál es la parte de contribuición de la búsqueda activa en la identificación de las ocurrencias. Métodos: Estudio retrospectivo, documental cuantitativo. Resultados: Se registraron 253 eventos/incidentes, de los cuales 239 (94%) fueron registrados por búsqueda activa. Lesión por presión fue el incidente con más frecuencia (n=88; 34,78%) y afectó a más pacientes (n=54; 48,9%). Se observó que 136 (53,8%) eventos/incidentes fueron leves. Conclusión: La búsqueda activa ha demostrado ser una estrategia de gestión que corrobora sustancialmente a la identificación de situaciones de riesgo en la unidad de cuidados intensivos.Palabras clave: Seguridad del paciente; Calidad de la atención de salud; Unidad de cuidados intensivos; Enfermería. RESUMOO conhecimento técnico-científico e os registros de enfermagem estão atrelados à segurança do paciente e refletem a qualidade da assistência prestada. O estudo objetivou analisar os registros de enfermagem em instrumento para registro e acompanhamento de cateter venoso central de inserção periférica em unidade de terapia intensiva neonatal, unidade de terapia intensiva pediátrica e unidade de cuidados intermediários de um hospital de ensino da região oeste do Estado do Paraná, Brasil;e caracterizar a equipe de enfermagem das unidades estudadas. Estudo de levantamento documental, de análise descritiva. Fizeram parte da amostra 666 prontuários, no período de julho de 2009 a julho de 2014. Identificam-se nos instrumentos, registros de enfermagem inadequados, incompletos ou inexistentes.Para caracterização da equipe de enfermagem, 15 enfermeiros e 37 técnicos de enfermagem responderam ao questionário. Dos enfermeiros, 93,3% possuem habilidade técnica e legal para inserção do cateter e 93,6% dos técnicos de enfermagem foram capacitados para os cuidados de manutenção do mesmo. Consideram-se necessárias capacitação e educação permanente dos profissionais de enfermagem quanto aos registros de enfermagem, que, quando inadequados, incompletos ou inexistentes, podem inviabilizar o planejamento, o desenvolvimento e a avaliação da assistência, comprometer o processo de cuidar e, consequentemente, a segurança do paciente. INTRODUÇÃOO cateter central de inserção periférica (PICC) foi incorporadoàs unidades de terapia intensiva neonatal, na década de 1970, para administração de nutrição parenteral. Atualmente é amplamente utilizado, não somente em recém-nascidos, mas também em adultos e crianças (1)(2) . Em razão de sua elevada taxa de sucesso na inserção e baixo risco de complicações, passou a ser utilizado como método de primeira escolha de acesso vascular prolongado, contribuindo principalmente para o tratamento endovenoso de neonatos que requerem infusão de múltiplas soluções intravenosas concomitantes (3)(4)(5) . Enfermeiros que atuam em neonatologia e pediatria necessitam de capacitação para inserção de PICC e constante atualização para subsidiar a prática, particularmente quanto à indicação, cuidados de manutenção e remoção do cateter; a qual deve ser norteada por protocolos institucionaisno desenvolvimento da Sistematização da Assistência da Enfermagem (SAE) (2,5) . Para o cuidado de neonatose crianças em uso de PICC são requeridos da equipe de enfermagem conhecimentos e habilidades técnicas específicas para manuseio seguro do dispositivo intravenoso. BACKGROUND Digital health interventions (DHI), such as conversational agents and chatbots, play a crucial role for health support, promotion and rehabilitation. As any other health interventions, DHIs require monitoring and evaluation (M&E). However, the current evaluation process lacks a standardized approach and necessitates the use of multiple strategies to ensure comprehensiveness. In 2016, the World Health Organization (WHO) proposed a seven-step practical guide for M&E DHIs, to conducting research and assessment, that became the main and most comprehensive reference in this context. Despite its importance, the guide is complex and lacks decision support tools, making its application a challenging task. OBJECTIVE The aim of this study was to create the MEDHI-Tool to support the use of the WHO M&E guideline for DHIs. METHODS This is applied methodological research, carried out in 2 phases: development and evaluation of the MEDHI-Tool; and application of this tool to M&E, a chatbot for guidance, monitoring and stroke tracking, recently made available to the public and which is being used by people from an association of patients with this disease. The development was carried out in Excel and its content was based on the consolidation of the information present among the chapters of the guide, which were classified into categories (description, objective and recommendations) and annotated in tables, to serve as a basis for creating the filters. The product was evaluated by experts for organization, relevance, understanding and applicability in M&E. RESULTS The MEDHI-Tool has four tabs composed and follow by the sequence of steps of the WHO guide: 1 and 4 (first tab); 2; 3; 5 to 7 (fourth tab). The tool provides a standard of guidelines to be applied to all maturity cycles of any DHI profile. The evaluation of the tool yielded a favorable response regarding each analyzed criterion. In the case study, the MEDHI-Tool proved to be a valuable resource in guiding decisions on appropriate metrics, objectives, study types, data collection, and indicators. CONCLUSIONS It was proposed the development of a tool to guide the M&E of DHI such as chatbots. The MEDHI- Tool has proven to be useful in providing evidence for planning processes, ensuring effective analysis of data generated and improving the delivery of products provided by DHI.
