Objetivo Analisar se a redução no tempo de espera para cirurgia ortopédica nos pacientes idosos com fratura da extremidade proximal do fêmur impacta na redução da mortalidade intra-hospitalar, da mortalidade em 6 meses de seguimento, e na redução do tempo de internação hospitalar.
Métodos Trabalho de coorte retrospectiva fundamentado na análise de prontuários de 81 pacientes internados com diagnóstico de fratura transtrocanteriana ou subtrocanteriana do fêmur, e submetidos a tratamento cirúrgico em 2015 e 2016 em um hospital de referência no atendimento ao trauma, antes e após a implantação de protocolo para cirurgia em até 48 horas.
Resultados Observou-se redução do tempo médio de internação de 17 para 11 dias após a implantação do protocolo. Com relação à mortalidade intra-hospitalar, cinco pacientes faleceram antes da implantação do protocolo, e cinco, após a implantação do protocolo. Referente à mortalidade extra-hospitalar, avaliada em 6 meses de pós-operatório, observou-se uma redução de 26,7% para 19,4%. Os resultados não apresentaram significância estatística.
Conclusão O presente estudo demonstrou que existe uma tendência à redução do tempo de internação hospitalar e da mortalidade em 6 meses quando a cirurgia para tratamento de fraturas do fêmur proximal no idoso é feita em até 48 horas de internação hospitalar.
SUMMARYForty hips (39 patients) were submitted to metal-on-metal hip replacement (resurfacing) between 2002 and 2005. Evaluation was provided by clinical examination and X-ray tests. The authors performed clinical evaluations before and after surgery. The specific criterion applied was the D'Aubigné and Postel's classification. X-ray images showed radiolucent lines around the acetabular component on the zones described by DeLee and Charnley and around the femoral component on the zones described by Amstutz et al. The mean age was 54.40 years. The minimum follow-up period was 14 months (range:12 to 51 months). The outcomes of 94.44% of the patients in the study were postoperatively rated as satisfactory. There were 2 cases of aseptic loose and no neck-femoral fractures during the follow-up period. The authors concluded that this technique and implant alternative is satisfactory, with good early outcomes in a mean follow-up time of three years.
Sessenta e cinco pacientes com fraturas peritrocanterianas instáveis foram tratados cirurgicamente com fixação intramedular céfalo-diafisária bloqueada (sistema minimamente invasivo). Realizou-se redução fechada em mesa de tração com utilização da radioscopia. Carga parcial assistida foi encorajada a partir do segundo dia pós-operatório. Em todos os pacientes estudados, foi obtida consolidação entre 12 e 18 semanas (média de 14.5 semanas), sendo observados seis casos (9,2%) de consolidação viciosa, quatro (6,1%) de encurtamento do membro (<1cm), três (4,6%) de migração do parafuso de compressão para o acetábulo e um caso (1,5%) de fratura intra-operatória da cortical lateral do fêmur. Cinco pacientes (7.6%) apresentaram trombose venosa profunda (TVP) distal sem repercussões clínicas. Não foi constatada infecção superficial ou profunda até a data da última revisão. Nos três casos de migração proximal do parafuso de compressão, o sistema foi retirado sem intercorrências. Concluímos ser a metodologia utilizada eficaz, segura, de baixa morbidade, técnica rápida, com poucas complicações e que propicia a reabilitação precoce do paciente.
