The results show a significant dependence of the skin surface topography on the age of the volunteer and the body site measured. Particularly the waviness, the Fourier analysis and the furrow profiles reflect such dependencies. The system used also fulfils the requirements for the testing of therapies, as was demonstrated by the reduction of mimic wrinkles after treatment with botulinum Toxin A.

In this study, we provide evidence that botulinum toxin A is an effective and safe treatment for frontal hyperhidrosis.

Die Hyperhidrose wird als Schwitzen definiert, das das physiologische Maß überschrei-tet.Ihr zugrunde liegt eine funktionelle Störung der ekkrinen Schweißdrüsen.Je nach Lokalisation des Schwitzens werden generalisierte von fokalen Formen der Hyperhidrose unterschieden. Gute epidemiologische Daten zur Häufigkeit der Erkrankung und mög-lichen Provokationsfaktoren liegen nicht vor. Die Therapie richtet sich nach der Pathogenese.Bei der generalisierten Hyperhidrose sollte zunächst eine sekundäre Hyperhidrose ausgeschlossen bzw. identifiziert und adäquat behandelt werden. Da die fokale Hyperhidrose nahezu ausschließlich idiopathischer Genese ist,kann hier sofort eine symptomatische Therapie erfolgen.Grundsätzlich sollte nach einem Stufenschema vorgegangen werden. Reicht eine topische Therapie z. B. mit Aluminumchloridhexahydrat nicht aus,kann diese ergänzend mit physikalischer Therapie (Iontophorese bei palmarer und plantarer Hyperhidrose) und/oder systemischer Therapie (oralen Anticholinergika) kombiniert werden. Bei unbefriedigendem therapeutischem Ergebnis steht in einem nächsten Schritt die Behandlung mit Botulinumtoxin A bzw.ggf.auch eine operative Behandlung (axilläre Saugkürretage bzw.Sympathektomie) zur Verfügung.Jedoch sollte diese aufgrund der relativ häufigen langfristigen Nebenwirkungen der Sympathektomie wie kompensatorisches Schwitzen nur den schwersten Fällen vorbehalten bleiben. Hyperhidrosis can be divided into generalized and focal.Good epidemiologic data on the frequency of the illness and possible provocation factors is not available.Treatment is based on the pathogenesis.Generalized hyperhidrosis is often secondary so underlying infections,malignancies or hormonal imbalances should be excluded.In contrast,focal hyperhidrosis is almost always idiopathic,so therapy can be started immediately,following a step-wise approach.Topical therapy with 10-30% aluminum chloride-hexahydrate is usually the first approach, followed by iontophoresis (for the palms and soles) and/or oral anticholinergics. As second line therapy botulinum toxin A or surgical procedures as axillary curettage or sympathectomy are recommended.Because of long-term side-effects such as compensatory and gustatory sweating, sympathectomy should be only considered in very severe cases.

Methanthelinium bromide has been considered for many years as a therapeutic option for the treatment of focal hyperhidrosis. However, its efficacy and safety have not previously been investigated in controlled randomised studies. We demonstrated both a decline in axillary sweat production and good tolerance. Therefore, the treatment of axillary hyperhidrosis with methanthelinium bromide in a regimen of 50 mg twice daily can be recommended, especially in those cases which are refractory to topical treatment with for example aluminium chloride hexahydrate solution. In contrast, we found no evidence for a clinically relevant diminution of palmar sweating.
