Introduction: Obesity is a disease of high prevalence in Brazil and in the world, and bariatric surgery, with its different techniques, is an alternative treatment. Objective: To compare techniques: adjustable gastric band (AGB), sleeve gastrectomy), Roux-en-Y gastric bypass (RYGB) and biliopancreatic diversion (BPD) analyzing leaks, bleeding, death, weight loss, resolution of type 2 diabetes, systemic arterial hypertension, dyslipidemia and obstructive sleep apnea. Methods: Were selected studies in the PubMed database from 2003 to 2014 using the descriptors: obesity surgery; bariatric surgery; biliopancreatic diversion; sleeve gastrectomy; Roux-en-Y gastric bypass and adjustable gastric banding. Two hundred and forty-four articles were found with the search strategy of which there were selected 116 studies through the inclusion criteria. Results: Excess weight loss (EWL) after five years in AGB was 48.35%; 52.7% in SG; 71.04% in RYGB and 77.90% in BPD. The postoperative mortality was 0.05% in the AGB; 0.16% on SG; 0.60% in RYGB and 2.52% in BPD. The occurrence of leak was 0.68% for GBA; 1.93% for SG; 2.18% for RYGB and 5.23% for BPD. The incidence of bleeding was 0.44% in AGB; 1.29% in SG; 0.81% in RYGB and 2.09% in BPD. The rate of DM2 resolved was 46.80% in AGB, 79.38% in SG, 79.86% in RYGB and 90.78% in BPD. The rate of dyslipidemia, apnea and hypertension resolved showed no statistical differences between the techniques. Conclusion: The AGB has the lowest morbidity and mortality and it is the worst in EWL and resolution of type 2 diabetes. The SG has low morbidity and mortality, good resolution of comorbidities and EWL lower than in RYGB and BPD. The RYGB has higher morbidity and mortality than AGB, good resolution of comorbidities and EWL similar to BPD. The BPD is the worst in mortality and bleeding and better in EWL and resolution of comorbidities.

-Background -The laparoscopic adjustable gastric band is one of the most used techniques in Europe and Australia in the treatment of morbid obesity. It is safe procedure with very low mortality, but is not free of complications or failure, requiring reoperation in some cases. . Results -Mean operative time was three hours. There were no complications during the operation. In the immediate postoperative period, all patients were fed back into the first day. Over the long term, 18 months later, showed a 68% loss of excess and resolution of type II diabetes and dyslipidemia and hypertension improved in all cases. Conclusion -The application of modified Scopinaro operation by laparoscopy in the rescue of adjustable gastric banding is the method that promotes weight loss and persistent satisfactory, through a small food restriction, accompanied by a high degree of satisfaction with tolerable side effects. ABCDDV/778RESUMO -Racional -A banda gástrica ajustável por videolaparoscopia é uma das técnicas cirúrgicas mais utilizadas na Europa e Austrália no tratamento da obesidade mórbida. É procedimento seguro, com muito baixa mortalidade, porém não é isento de complicações ou insucessos, com necessidade de re-operação em alguns casos. Objetivo -Avaliar os resultados da aplicação da técnica de Scopinaro modificada, na falha da aplicação da banda gástrica ajustável. Método -Utilizou-se a técnica de Scopinaro por videolaparoscopia sem ressecção do corpo e antro gástrico (técnica de Domene). Com este procedimento, foram operados 14 pacientes, sendo cinco do sexo feminino, com idade entre 24 e 68 anos, IMC (índice de massa corpórea) variando entre 35,3 a 55,2 kg/m 2 . Resultados -O tempo operatório médio foi de três horas. Não houve complicações durante a operação. No pós-operatório imediato, todos os pacientes foram realimentados no primeiro dia. A evolução em longo prazo, 18 meses após, mostrou perda de 68 % do excesso e resolução da diabetes tipo II e dislipidemia e melhora da hipertensão em todos os casos. Conclusão -A aplicação da operação de Scopinaro modificada por videolaparoscopia no resgate da banda gástrica ajustável é método que promove perda de peso satisfatória e persistente, através de pequena restrição alimentar, acompanhada de alto grau de satisfação com efeitos indesejáveis toleráveis.

-Introduction -Vertical gastroplasty is increasingly used in the surgical treatment of morbid obesity, being used alone or as part of the duodenal switch surgery or even in intestinal bipartition (Santoro technique). When used alone has only a restrictive character. Method -Is proposed association of jejunoileal bypass to vertical gastroplasty, in order to give a metabolic component to the procedure and eventually empower it to medium and long term. Eight morbidly obese patients were operated after removal of adjustable gastric band or as a primary procedure associated to vertical banded gastroplasty with jejunoileal bypass laterolateral and anastomosis between the jejunum 80 cm from duodenojejunal angle and the ileum at 120 cm from ileocecal valve, by laparoscopy. Results -The patients presented themselves without complications both in trans or in the immediate postoperative period, and also in the months that followed. The evolution BMI showed a significant reduction ranging from 39.57 kg/m 2 to 28 kg/m 2 . No patient reported diarrhea or malabsorptive disorder in the period. Conclusion -It can be offered a new therapeutic option, with restraining and metabolic aspects, in which there are no consequences as the ones founded in procedures with duodenal diversion or intestinal transit alterations. ABCDDV/802RESUMO -Introdução -A gastroplastia vertical é procedimento técnico cada vez mais utilizado no tratamento cirúrgico da obesidade mórbida, sendo empregado isoladamente ou como tempo cirúrgico do duodenal switch ou da bipartição intestinal (técnica de Santoro). Quando empregada isoladamente tem apenas caráter restritivo. Método -É proposta a associação do desvio jejunoileal à gastroplastia vertical, no sentido de conferir um componente metabólico ao procedimento e eventualmente potencializá-lo a médio e longo prazos. Foram operados oito pacientes com obesidade mórbida após a retirada da banda gástrica ajustável ou como procedimento primário associado à gastroplastia vertical e desvio jejunoileal através de anastomose laterolateral entre o jejuno a 80 cm do ângulo dudeno jejunal e o íleo a 120 cm da válvula ileocecal, por videolaparoscopia. Resultados -Os pacientes evoluíram sem qualquer intercorrência tanto no trans como no pós-operatório imediato, e nos meses que se seguiram. A evolução do IMC médio, mostrou redução expressiva indo de 39,57 kg/m 2 para 28 kg/m 2 . Nenhum paciente referiu diarréia ou qualquer quadro disabsortivo no período estudado. Conclusão -Pode-se oferecer nova opção terapêutica, com ação restritiva e metabólica, na qual não existem os inconvenientes do desvio do trânsito duodenal com todas suas consequências.

-Background -A major complication after laparoscopic fundoplication is migration of the fundic wrap into the chest, with or without rupture of the valve. This can occur as a result of inadequate closure of the diaphragmatic hiatus, by rupture of sutures or carried by laceration of the muscle fibers manipulated. AimTo verify the efficiency of the use of Dacron ® or Prolene ® mesh in the reinforcement of the diaphragmatic hiatus. Methods -Fifteen patients were operated with mean age of 46 years using laparoscopic antireflux surgery, with Prolene ® or Dacron ® mesh reinforcement of the hiatal crura. The patients were operated under general anesthesia by laparoscopy using five trocars. The average time of surgery was 120 minutes. No deaths occurred. The mean postoperative time was 3.5 days. Patients returned to normal activities around the 10th postoperative day. The mean followup was five years. In only two patients, in which were used in a Dacron ® mesh, and in another Prolene ® , there was migration of the mesh to the esophageal lumen causing dysphagia. Conclusion -The mesh use in laparoscopic hiatoplasties is good alternative to close the diaphragmatic defect in large hiatal hernias, correction of reflux esophagitis associated with Barrett's esophagus or hiatal hernia/esophagitis recurrence after surgical correction.RESUMO -Racional -Uma das principais complicações após a fundoplicatura laparoscópica é a migração do fundo gástrico para o tórax, com ou sem ruptura da válvula. Isso pode ocorrer como consequência de fechamento inadequado do hiato diafragmático, por ruptura das suturas realizadas ou por laceração das fibras musculares manipuladas. Objetivo -Estudar a utilização da tela de Prolene ® ou de Dacron ® para reforçar o fechamento do hiato diafragmático. Métodos -Foram operados 15 pacientes (6 homens e 9 mulheres) com idade média de 46 anos (22 a 78) tendo-se realizado operação laparoscópica anti-refluxo, utilizando-se tela Prolene ® ou Dacron ® para reforçar a crura hiatal. Os pacientes foram operados sob anestesia geral, por videolaparoscopia utilizando cinco trocárteres. Resultados -O tempo médio de operação foi de 120 minutos. Não ocorreu nenhum óbito. O tempo pós-operatório médio foi de 3,5 dias. Os pacientes retornaram à suas atividades habituais em torno do 10º dia do pós-operatório. O tempo médio de seguimento foi de cinco anos. Em apenas dois pacientes, nos quais foram utilizadas em um a tela de Dacron ® e em outro Prolene ® , ocorreu migração da tela para o lúmen esofágico ocasionando disfagia. Conclusão -Utilizar tela nas hiatoplastias videolaparoscópicas é boa alternativa para o fechamento dos defeitos diafragmáticos nas grandes hérnias hiatais, na correção da esofagite de refluxo associada ao esôfago de Barrett, ou em caso de recorrência da hérnia hiatal ou esofagite após correção cirúrgica prévia.
