BackgroundThe authors present an unusual clinical case of high median nerve compression caused by an iatrogenic pseudoaneurysm of the brachial artery after an angiography with a follow up of 9 months.MethodsA 73-year-old male was seen with progressive numbness, loss of opponency and diminution of strength of finger flexion in the left hand after an angiography with direct puncture of the left humeral artery. Physical examination revealed a hard consistency internal distal arm swelling with the size of a walnut, non-pulsatile and with a Tinel sign on percussion. Upper extremity arterial Doppler ultrasonography and magnetic resonance imaging didn't point to a pseudoaneurysm. Given the persistence and progressive worsening of symptoms, the patient was operated at 4 months after the beginning of symptoms through an anterior approach of the left elbow. The tumefaction corresponded to a brachial artery pseudoaneurysm completely thrombosed causing severe compression of the median nerve. Microsurgical neurolysis of the median nerve was performed and the pseudoaneurysm was removed.ResultsAt 9 months of follow-up the patient had total recovery of flexion of the thumb and forefinger and recovery of opposition with slight hypoesthesia on the volar surface of the radial fingers of the hand.ConclusionsThis case represents a rare form of high median nerve compression after angiography. The increased use of endovascular procedures may cause a higher frequency of these neurological injuries, and if confirmed, the patient should be operated as soon as possible to avoid serious neurological sequelae.
Volkmann ischemic contracture is a cause of limb dysfunction that can lead to varying amounts of limb deformity, stiffness, and paralysis. Several procedures have been used to improve limb function in these patients. An 11-year-old male patient sustained a domestic accident with a glass door, resulting in a severe soft tissue trauma to his proximal third of the right forearm with damage to flexor muscles and tendons, complete section of median and ulnar nerves, and radial and ulnar arteries. He was submitted to immediate repair of the radial artery and median and ulnar nerves. Ulnar artery was not repairable due to extensive soft tissue loss. Despite surgical treatment and successful revascularization, postoperatively he developed a postreperfusion syndrome, and in consequence an ischemic contracture. To improve his condition, simultaneous Burkhalter's procedure and fractional flexor tendon lengthening were used after a previous failed Stiles-Bunnell transfer. After 6 months, a good result was obtained, with restoration of a more anatomical hand rest position, improved arc of motion, and significant improvement of hand function with DASH (Disabilities of the Arm, Shoulder and Hand) score improving from 59 to 43 despite a slight reduction in grip strength. Opposition to the ring or little finger tip with the interphalangeal joint (IPJ) of the thumb extended was observed. The combination of fractional flexor tendon lengthening and Burkhalter's procedure is a reliable and simple procedure in cases of Volkmann's ischemic contracture and combined median and ulnar nerve palsy, without significant complications.
Objetivo: Reunir informações sólidas e atuais a respeito da importância da vigilância em saúde e suas áreas de atuação e interfaces, apresentando, também, discussão no que concerne a seus principais desafios. Métodos: Trata-se de uma revisão integrativa pautada na busca online nas bases de dados: BVS, PubMed e SciELO. Foram feitas pesquisas separadamente em inglês e em português, usando-se os descritores “public health”, “surveillance”, “performance”, “vigilância em saúde” e “atuação”. Foram inclusos artigos publicados entre 2015 e 2020, disponíveis na íntegra e que apresentaram a vigilância como foco central da discussão. Resultados: A amostra foi composta por 17 artigos. Verificou-se que a vigilância é fundamental para os sistemas de saúde, com suas divisões atuando, cada qual em sua área, na identificação, minimização e eliminação de potenciais riscos à saúde, bem como no diagnóstico precoce, caso esses riscos já tenham se convertido em problemas reais. Considerações finais: Com o escopo de alcançar a excelência, deve-se não apenas conhecer a história e atuação e interfaces da vigilância em saúde, mas também enfrentar seus desafios, como aqueles identificados nesse trabalho, se fazendo necessário um constante levantamento bibliográfico acerca das temáticas que permeiam o assunto.
A infância é um período de grande fragilidade psicossocial, devido a ocorrência de amplas mudanças físicas, emocionais e comportamentais; e o cotidiano imposto pela hospitalização pode exacerbar essa vulnerabilidade. O objetivo deste artigo é realizar uma revisão reflexiva bibliográfica, de abordagem qualitativa e caráter descritivo, acerca do sofrimento psíquico de crianças hospitalizadas e das estratégias utilizadas para amenizá-lo. Foram selecionados diversos estudos em periódicos indexados no portal de Periódicos Eletrônicos de Psicologia (PePSIC), com as seguintes palavras-chaves “psicologia hospitalar”, “criança” e “hospitalizada”, em junho de 2019. Após análise, emergiram dois temas considerados relevantes: importância da equipe de saúde no desenvolvimento psicossocial da criança e múltiplos aspectos do acompanhamento psicológico da criança hospitalizada. Por fim, concluiu-se que a hospitalização pode influenciar negativamente as crianças e que certas intervenções podem minimizar o sofrimento psíquico. Espera-se que o resultado dessa pesquisa direcione novas pesquisas e estimule o planejamento de intervenções eficazes nesse sentido.
