La respuesta sexual se encuentra mediada por factores anatomo-hormono-neuro-vasculoendoteliales. Ha sido explicada a través de varios modelos como el tetrafásico (Masters y Johnson, 1966), el trifásico (Kaplan, 1979), y el pentafásico (Bancroft, 1983), entre otros. El orgasmo se ubica como una de estas fases en estos modelos. Éste se define como un conjunto de sensaciones corporales sumamente placenteras y de excitación intensa, que libera tensiones y que produce satisfacción. Existen pocas diferencias entre el orgasmo del varón y de la hembra; desde un punto de vista funcional, el orgasmo del varón está estrechamente relacionado con la eyaculación, mientras que en la hembra no existe una conexión tan obvia y directa. En este estudio se recopilaron las semejanzas y diferencias presentes en el orgasmo del varón y de la hembra, referidas a los fundamentos anatómicos, fisiológicos, neuroendocrinos, neurológicos, psicológicos, socioculturales y espirituales.
La hernia de Spiegel es un defecto infrecuente de la pared abdominal producido por una alteración en la unión del borde lateral del músculo recto del abdomen y el borde medial del músculo transverso del abdomen en la línea semilunar. Representan apenas del 0,12 al 2% de todas las hernias. La presente investigación se enfocó en describir las manifestaciones clínicas, el diagnóstico, la incidencia, el procedimiento quirúrgico y su evolución en dos casos reportados. Caso 1: Femenina de 33 años, multípara, obesa, quien consulta por dolor y masa palpable en el cuadrante superior izquierdo del abdomen. Al examen físico, se evidenció dolor a la palpación en mesogastrio e hipocondrio izquierdo, exhibiendo protrusión del contenido abdominal, en la línea de Spiegel. Intervención quirúrgica: hernioplastia protésica de Spiegel con colocación de malla supraaponeurótica. Caso 2: Masculino de 89 años, con diagnóstico de enfermedad pulmonar obstructiva crónica, acudió a consulta presentando dolor abdominal en fosa iliaca y en la región inguino–escrotal izquierda. Al examen físico se evidenció saco herniario en la región inguino–escrotal por lo que se decide su resolución quirúrgica. Como hallazgo transoperatorio se evidenció defecto aponeurótico del recto anterior del abdomen izquierdo en su tercio inferior. Intervención quirúrgica: hernioplastia protésica de Spiegel con colocación de malla supraaponeurótica. Conclusión: La hernia de Spiegel es una entidad poco frecuente por lo que se debe concientizar al cirujano para sospechar este tipo de hernia en aquellos pacientes con presencia de aumento de volumen en la línea de Spiegel.
Peptic ulcer perforation is a frequent emergency, which must be resolved on time. It is closely related to Helicobacter pylori infection, smoking and the consumption of NSAIDs. The aim was to determine the surgical management and the description of the intraoperative findings in patients admitted with the diagnosis of perforated peptic ulcer in the General Surgery Service of the University Hospital of Maracaibo. The clinical history of 42 patients diagnosed with Perforated Peptic Ulcer, admitted in the General Surgery service of the University Hospital of Maracaibo in a period from January 2016 to June 2019, was reviewed. The average age of onset was 44.88 ± 6.04 years. 88.09% of the patients belonged to the male sex. On the other hand, the most frequent location of the ulcer corresponded to those of grade III, according to the classification of peptic ulcers described by Johnson. The techniques implemented for the closure correspond to, simple closure in 5 (11.90%), ulcorraphy + Graham patch omentopexy in 29 (69.04%) and Heineke-Mikulicz pyloroplasty in 8 (19.04%) cases. Mortality was located at 16.66% associated with sepsis. It was concluded that the management of the perforated peptic ulcer was carried out mainly by ulcorraphy + Graham patch omentopexy, then by pyloroplasty, and lastly by simple ulcer closure.
