Introdução: A Portaria de Consolidação PRC GM/MS nº 2/2017 permite que as etapas de solicitação, dispensação e a renovação do tratamento relativas à execução do Componente Especializado de Assistência Farmacêutica (CEAF) possam ser descentralizadas e executadas pelos municípios. Diante disso, em 2021, a Secretária de Estado de Saúde de Minas Gerais (SES-MG) publicou a Resolução SES/MG n° 7.628/2021 que estabelece a Política de Descentralização do Componente Especializado da Assistência Farmacêutica (PDCEAF) e aprova as normas e critérios para a descentralização dessas etapas através da adesão dos municípios mineiros. A PDCEAF visa ampliar e qualificar o acesso aos medicamentos do CEAF pelos usuários, contribuindo para a promoção do uso racional de medicamentos e da integralidade da atenção à saúde, da garantia do cuidado integral farmacêutico graças à dispensação direta ao paciente e da boa qualidade dos atendimentos, a partir da superação da sobrecarga de trabalho vivenciada nas farmácias regionais. Objetivo: O objetivo deste artigo é levantar e discutir a aplicabilidade, os benefícios e as limitações da descentralização de etapas do CEAF. Através de dados coletados, buscar-se-á explicar, explorar e descrever a experiência mineira. Métodos: Um estudo de caso se concentra em uma situação específica, proporcionando uma análise aprofundada e propiciando novos conhecimentos e interpretações sobre um fato. Assim, para esta pesquisa buscar-se-á descrever e explicar o processo de descentralização do CEAF diante das especificidades mineiras. Assim, utilizou-se de pesquisas bibliográficas para compreender sobre o CEAF e a descentralização. Posteriormente, executou-se uma pesquisa documental, no qual se baseou em documentos internos à SES-MG para extrair as informações relevantes ao estudo de caso, como regras e dados gerais. Resultados: Até junho de 2022, 589 municípios aderiram à PDCEAF, representando 69,5% dos municípios mineiros; desses, 168 estão executando, representando 32.253 usuários do CEAF beneficiados. Detectou-se que os principais gargalos para implementação e execução da política foram/são a infraestrutura inadequada das farmácias municipais, recursos humanos insuficientes para a execução a nível regional e municipal e falta de sensibilização dos municípios. Não obstante, verificou-se que a SES-MG tem sido capaz de remodelar e complementar a PDCEAF de modo a superar os obstáculos encontrados, como ao criar um projeto denominado Adere PDCEAF, com esforços direcionados especificamente para conseguir mais adesões à política, e a publicação de resoluções visando estruturar as farmácias municipais. Conclusões: Mesmo com apenas um ano de existência, a PDCEAF tem tido êxito em convencer os municípios em promover a descentralização, e tem sido capaz de se remodelar à medida que a execução ocorre. Dessa forma, ela se apresenta como um exemplo para outros estados interessados em descentralizar o CEAF, devendo ser mais e melhor estudada e viabilizada.
