Speculation concerning the role of psychiatric disorders in patients undergoing bariatric surgery is abundant in the literature. Psychiatric factors predicting the success or failure of bariatric surgery have not been adequately described. However, an increasing number of cases of eating disorders in the postoperative period are being reported. A concise literature review and 5 cases are presented with the purpose of studying the specific pathological eating habits of these patients. The criteria for Anorexia or Bulimia Nervosa were not fulfilled in most of the reports studied and were not met by the 5 patients described. The criteria for a new eating disorder (Post-Surgical Eating Avoidance Disorder, PSEAD) are proposed.
A alimentação e a dieta têm um importante papel no desenvolvimento e manutenção dos transtornos alimentares. Portanto, devem ser levados em consideração nos programas de tratamento dessas condições clínicas. Pacientes com transtornos alimentares apresentam importantes restrições dietéticas, padrões alimentares inadequados e hábitos errôneos devido a uma série de falsos mitos e crenças e a uma sensação de incompetência para lidar com o alimento. Tais alterações podem levar a mudanças em seu estado nutricional, que necessita de cuidados dietéticos específicos, como reabilitação nutricional e orientação sobre dieta adequada. Além disto, o aconselhamento nutricional é necessário para esclarecer e desmistificar crenças inadequadas e para estabelecer uma adequada relação com o alimento.
ResumoObjetivo: O artigo faz uma revisão do tema Anorexia Nervosa na Adolescência. É consenso na literatura que este transtorno apresenta alta morbidade e mortalidade, e sua incidência vem crescendo nas últimas décadas, o que torna o tema de grande importância para a investigação científica.Método: Os autores realizaram uma revisão não sistematizada da literatura e também utilizaram suas experiências clínica no trabalho com pacientes adolescentes que sofrem de anorexia nervosa.Resultados: Os fatores de risco mais conhecidos são a idade e o sexo, sendo na adolescência o pico de incidência e a prevalência nove vezes maior no sexo feminino. Acreditou-se inicialmente que a anorexia manifestava-se apenas nas classes socioeconômicas mais altas, expostas às exigências do padrão de beleza ocidental. Estudos mais recentes identificam incidência semelhante a do mundo ocidental também em classes mais baixas, áreas rurais e comunidades orientais. O quadro clínico caracteriza-se por recusa em manter o peso no limite inferior (ou acima dele) para a idade e altura, medo intenso de ganhar peso ou tornar-se gordo/a, mesmo estando abaixo do peso para a altura e a idade, distorção da imagem corporal e amenorréia. As complicações mais comuns são semelhantes às de um quadro de desnutrição crônica, e a principal comorbidade é a depressão.Conclusões: O tratamento deve ser multidisciplinar, levando em conta a pluralidade dos fatores etiológicos. According to the literature, this disorder presents high morbidity and mortality, and its incidence has been rising over the past decades, highlighting the importance of investigating this subject.Methods: The authors performed a non-systematic review of the related literature and also reported their clinical experiences.Results: Age and gender are important risk factors. The peak of incidence occurs in adolescence and the prevalence is 9 times higher in females. It was initially believed that anorexia nervosa manifested only in higher socio-economic levels, exposed to the demands of western beauty standards. More recent studies have identified similar incidence of this disorder also in lower socio-economic levels, rural areas and Asian communities. The clinical features are: refusal to maintain weight at or above a minimally normal weight for age and height, intense fear of gaining weight or becoming fat, even though under weight, disturbance in the way of experiencing one's body weight or shape, and amenorrhea.
