Sosiaali- ja terveydenhuollon digitalisaatiolle on asetettu paljon odotuksia niin palveluiden laadun, saavutettavuuden kuin tehokkuuden osalta. Asiakas- ja potilastietojärjestelmiltä odotetaan paljon tavoitteiden saavuttamiseksi. Asiakastietojärjestelmät ovat laajasti käytössä sosiaalialalla, mutta niiden käyttöä työvälineenä ja niiden avulla saavutettuja hyötyjä on tutkittu ammattilaisten näkökulmasta vain vähän. Keväällä 2019 kartoitettiin ensimmäistä kertaa valtakunnallisesti sosiaalialan ammattilaisten käyttäjäkokemuksia asiakastietojärjestelmistä. Tässä artikkelissa kuvataan kyselytutkimuksen toteutus ja tulokset eniten käytettyjen tuotemerkkien osalta. Tutkimus toimii pilottina vuonna 2020 toteutettavalle sosiaalialan ammattilaisten käyttäjäkokemuskyselylle, joka on osa valtakunnallista Sosiaali- ja terveydenhuollon tietojärjestelmien seurannan ja arvioinnin hanketta (STePS 3.0). Tutkimusaineisto kerättiin keväällä 2019 sähköisellä kyselyllä, joka pohjautui lääkäreille ja hoitajille aiemmin toteutettuihin käyttäjäkokemuskyselyihin. Sosiaalialan kyselyn teemoina olivat tekninen toimivuus, käytettävyys, tuki asiakastapauksen hahmottamiselle ja oman työn hallinnalle, yhteistyö ja tiedonkulku sekä järjestelmien hyödyt ja haitat. Aineisto koostui 1145 eri tehtävässä ja toimintaympäristössä toimivan sosiaalialan ammattilaisen vastauksista. Tätä artikkelia varten aineistosta on analysoitu vastaajien eniten käyttämien asiakastietojärjestelmien tuotemerkkien osalta joukko edustavia väittämiä kyselyn eri teema-alueista. Kaikkien asiakastietojärjestelmien kouluarvosanojen keskiarvo oli 6,7. Asiakastietojärjestelmiä pidettiin teknisesti vakaina ja toimivina, mutta arviot tiedonkulussa ja monialaisen yhteistyön tuessa, asiakkuuksien hallinnassa ja asiakkaasta saatavan kokonaiskuvan hahmottamisessa tietojärjestelmissä olivat varsin kriittisiä. Käyttäjien kokemuksia tutkimalla on mahdollista saada arvokasta tietoa asiakastietojärjestelmien kehittämisen ja suunnittelun tueksi. Jatkotutkimuksen aiheena on yksityiskohtaisempia ja fokusoidumpia analyysejä laajasta kyselytutkimusaineistosta muun muassa eri vastaajajoukkojen osalta.
Tietojärjestelmien kehittämistyön lähtökohtana tulee olla vahva ymmärrys käyttäjien tarpeista, tehtävistä sekä toimintaympäristöstä. Lisäksi suunnitteluratkaisuja tulee arvioida käyttäjien kanssa kehittämisprosessin aikana ja käyttöönoton jälkeen. Vaikka kansallisesti kohdennetaan runsaasti resursseja tietojärjestelmäpalveluiden kehittämiseen, sosiaalialalta tutkimusta aiheesta löytyy niukasti. Sosiaalialan ammattilaisten kokemuksia asiakastietojärjestelmien kehittämistyöstä kartoitettiin osana keväällä 2019 toteutettua valtakunnallisesta käyttäjäkokemuskyselyä. Kysymyksissä käsiteltiin kokemuksia yhteistyöstä tietojärjestelmätoimittajan kanssa, osallistumisesta kehittämistyöhön ja osallistumistavoista tietojärjestelmien kehitykseen. Tuloksia tarkasteltiin asiakastietojärjestelmän tuotemerkeittäin. Puolet vastaajista ei ollut osallistunut lainkaan kehittämistyöhön, mutta vain 19% ei ollut halukkaita osallistumaan ollenkaan. Suosituimmat osallistumistavat olivat ongelmien raportointi järjestelmien kehittämisen vastuuhenkilölle ja oman työn esittely kehittäjille; sen sijaan tietojärjestelmätoimittajien yleisimmin käyttämät tavat eli palautteen kirjaaminen verkkosivustolle tai sähköpostiin eivät olleet käyttäjien mieleen. Suurin osa tiesi, miten ongelmatilanteista ja kehitysehdotuksista raportoidaan. Sosiaalialan ammattilaiset ovat kiinnostuneita osallistumaan kehittämistyöhön, mutta he eivät ole tyytyväisiä mahdollisuuksiin ja tapoihin osallistua käyttämiensä järjestelmien kehittämiseen. Järjestelmätoimittajien ja käyttäjien väliltä puuttuvat sopivat toimintatavat. Tulokset ovat samansuuntaisia kuin vastaavissa lääkäreille ja sairaanhoitajille tehdyissä tutkimuksissa. On myös mahdollista, että organisaatiot eivät hyödynnä innokasta kehittäjäpotentiaalia. Kehittäjien tulee jalkautua kentälle ja organisaatioiden palkata tietojärjestelmäkehityksestä vastaavia ammattilaisia tulkeiksi järjestelmätoimittajan ja käyttäjien välille. Sosiaalialan ammattilaisten näkökulma on huomioitava vahvemmin järjestelmäkehityksessä, jotta tietojärjestelmät saadaan onnistuneesti tukemaan sote-yhteistyötä tulevaisuudessa.
Summary Objective: Human and Organizational Factors (HOF) studies in health technology involve human beings and thus require Institutional Review Board (IRB) approval. Yet HOF studies have specific constraints and methods that may not fit standard regulations and IRB practices. Gaining IRB approval may pose difficulties for HOF researchers. This paper aims to provide a first overview of HOF study challenges to get IRB review by exploring differences and best practices across different countries. Methods: HOF researchers were contacted by email to provide a testimony about their experience with IRB review and approval. Testimonies were thematically analyzed and synthesized to identify and discuss shared themes. Results: Researchers from seven European countries, Argentina, Canada, Australia, and the United States answered the call. Four themes emerged that indicate shared challenges in legislation, IRB inefficiencies and inconsistencies, general regulation and costs, and lack of HOF study knowledge by IRB members. We propose a model for IRB review of HOF studies based on best practices. Conclusion: International criteria are needed that define low and high-risk HOF studies, to allow identification of studies that can undergo an expedited (or exempted) process from those that need full IRB review. Enhancing IRB processes in such a way would be beneficial to the conduct of HOF studies. Greater knowledge and promotion of HOF methods and evidence-based HOF study designs may support the evolving discipline. Based on these insights, training and guidance to IRB members may be developed to support them in ensuring that appropriate ethical issues for HOF studies are considered.
In this paper, we describe the requirements elicitation for a digital war room -a group work facility to support people interacting with digital (and analog) materials. The target user group is product design teams and construction engineers/architects. Main requirements for an interactive group work space include fluent sharing of documents within the facility and ability to comment on them, support for both analog and digital sketching and writing by hand, minimum of three displays allowing parallel tasks and comparison of different alternatives and the importance of the physical properties of the facility.
