Objetivos: avaliar o conhecimento do fisioterapeuta em relação à interpretação gráfica e à monitorização ventilatória. Métodos: estudo quantitativo realizado entre março e abril de 2018, em três hospitais de Fortaleza/CE, com 41 fisioterapeutas intensivistas sobre seu conhecimento no que diz respeito à interpretação gráfica e monitorização ventilatória, bem como sua abordagem terapêutica frente aos achados clínicos. Os dados foram colhidos por meio de um questionário com 10 questões objetivas e analisados por meio do software Statistical Package for Social Sciences (SPSS) 17.0. Resultados: a maioria dos fisioterapeutas tinham entre 6 e 10 anos de atuação na UTI (43,9%). Quando questionados sobre os conhecimentos básicos de pressão e parâmetros de normalidade do Índice de Oxigenação, 80,5% e 70,6% fisioterapeutas ofereceram respoatas corretasrespectivamente. Acerca do conhecimento de monitorização ventilatória, o conceito de drive pressure foi o que apresentou melhor índice de assertiva com 73,2% acertos. Já sobre a análise gráfica, a interpretação da apneia (87,8%) e a abordagem na auto-PEEP (58,5%) foram as questões com maior número de acertos. Conclusão: houve assertividades relevantes no tocante ao conhecimento dos conceitos básicos, da monitorização dos parâmetros ventilatórios e da interpretação gráfica por parte dos profissionais entrevistados, porém sugere-se que haja mais estudos sobre a temática por meio de uma estratégia de educação continuada a fim de gerar um maior suporte teórico-prático para os fisioterapeutas intensivistas.
INTRODUÇÃO: Pacientes em Unidade de Terapia Intensiva (UTI) requerem longos períodos de internação, estando submetidos ao imobilismo, que resulta em perda significativa de massa muscular. A mobilização precoce é uma terapêutica realizada no ambiente de UTI e tem como objetivo diminuir o comprometimento funcional decorrente do período de internação. OBJETIVO: analisar a prática de mobilização precoce realizada pelo fisioterapeuta intensivista, identificar as principais intervenções utilizadas por esses profissionais e descrever as barreiras encontradas que inviabilizam a prática da mobilização precoce, em pacientes internados em unidades de terapia intensiva. METODOLOGIA: Estudo de campo, quantitativo e transversal, realizado entre fevereiro e maio de 2020, com fisioterapeutas intensivistas de três hospitais na cidade de Fortaleza, a saber dois deles são da rede pública (um municipal e outro estadual) e o outro de rede privada. Foram inclusos no estudo fisioterapeutas intensivistas atuantes nos hospitais mencionados e que possuam vínculo com a instituição. Foram excluídos os fisioterapeutas na função de residentes, estagiários e preceptores presentes. Para coleta de dados foi utilizado o formulário eletrônico on-line viabilizada por meio do aplicativo Google Forms. Os dados foram analisados e tabulados através do Software Statistical Package for the Social Sciences (SPSS) versão 20.0. A estatística descritiva, com frequências absolutas e relativas foi utilizada para caracterizar o perfil da amostra estudada. Os testes estatísticos aplicados foram o Qui-Quadrado para determinar diferenças nas respostas dos critérios de segurança e barreiras para implementação da mobilização precoce e o tempo de experiência em unidade de terapia intensiva e o Kruskal-Wallis para comparar as intervenções de mobilização precoce entre grupos de Fisioterapeutas dos três hospitais. RESULTADOS: Participaram da pesquisa 68 fisioterapeutas, a maioria (36,8%) com tempo de atuação na terapia intensiva de 6 a 10 anos. O gerenciamento da mobilização precoce é realizado em sua maioria apenas pelo fisioterapeuta. Acerca da utilização de escalas funcionais utilizadas em UTI, a Medical Research Council (MRC) foi a mais citada pelos profissionais com (67,7%). A estratégia de mobilização mais utilizada foi a sedestação (91,2%). O desconforto respiratório foi a situação clínica mais citada para a interrupção da mobilização precoce (83,8%). CONCLUSÃO: As intervenções mais frequentes foram a sedestação, uso do cicloergômetro e transferências leito poltrona. As barreiras relacionadas ao paciente foram a instabilidade hemodinâmica, uso de drogas sedativas e analgésicas.
Objetivos: conhecer o manejo do fisioterapeuta relacionado à ventilação protetora em pacientes com SDRA por COVID-19, listando as principais estratégias utilizadas e as barreiras encontradas para sua implementação. Métodos: estudo de campo transversal e descritivo, de caráter quantitativo, realizado no período de outubro de 2021 a abril de 2022, com fisioterapeutas de hospitais públicos e privados, na cidade de Fortaleza, que tivessem experiência com pacientes infectados pela COVID-19. Para coleta de dados, utilizou-se um formulário online viabilizado pela plataforma Google Forms. Os dados foram tabulados em planilha Microsoft Excel® 2010 e, posteriormente, transferido para o Statistical Package for the Social Sciences (SPSS®) 20.0, utilizando-se a estatística descritiva. Resultados: participaram 102 fisioterapeutas, 74,5% do gênero feminino. As principais estratégias de ventilação protetora citadas foram: volume corrente de 6 mL/Kg por peso predito, pressão de platô menor que 30 cmH2O e driving pressure menor que 15 cmH2O (93,1%), além da titulação de PEEP (68,6%) e posição prona (64,7%). Manter uma driving pressure menor que 15 cmH2O (57,8%) e discordância profissional (43,1%) foram as principais barreiras encontradas para realização da ventilação protetora. Conclusão: a ventilação protetora configura-se no uso de baixos volumes e pressões, titulação da PEEP e posição prona. A manutenção de driving pressure menor que 15 cmH2O e discordância entre os profissionais na tomada de decisão destacam-se como as principais barreiras.
Objetivo: este estudo objetivou conhecer a vivência de fisioterapeutas com pacientes hospitalizados sob oxigenoterapia, bem como identificar os desafios vivenciados durante a sua intervenção. Métodos: trata-se de um estudo quantitativo e transversal, realizado no período de setembro de 2021 a março de 2022, com fisioterapeutas atuantes em hospitais da cidade de Fortaleza-CE. A coleta de dados foi realizada de forma presencial e virtual pela plataforma Google Forms, por meio de um questionário com perguntas objetivas sobre vivência, intervenção e desafios durante assistência ao paciente sob oxigenoterapia. Os dados coletados foram armazenados no Microsoft Excel® 2010 e analisados pelo SPSS® versão 20.0, utilizando-se de estatística descritiva. Resultados: participaram do estudo 101 fisioterapeutas, 76,2% do gênero feminino, e 45,5% dos participantes eram graduados há 5 anos. Destes, 83,2% dos fisioterapeutas afirmaram que SpO2 < 90% é indicativo de oxigenoterapia. A maioria (79,2%) discordou da utilização da máscara de Venturi para pacientes com Covid-19 e apontou a cânula nasal de alto fluxo (39,6%) e a máscara reservatório não reinalante (40,6%) como as mais utilizadas. Entre os desafios enfrentados durante a assistência, a interação com a equipe interdisciplinar (25,70%), o desmame da oxigenoterapia (21,80%) e a indisponibilidade de recursos (20,80%) foram os mais relatados. Conclusão: grande parte dos profissionais atua indicando o oxigênio suplementar em situações em que há SaO2 < 90%, realizando avaliação à beira leito da cânula nasal de alto fluxo por meio do índice ROX e indicando intervenção fisioterapêutica.
INTRODUÇÃO: A doença causada pelo Novo Coronavírus (SARS-CoV-2), gerou um fluxo crescente de pacientes infectados que necessitaram de atenção especializada em terapia intensiva, impactando sobremaneira o Sistema de Saúde, repercutindo de forma direta na experiência de Fisioterapeutas Intensivistas, profissionais fundamentais nessa assistência. OBJETIVO: Verificar modificações na prática clínica do fisioterapeuta intensivista durante a Pandemia de COVID-19 e identificar desafios enfrentados durante assistência aos pacientes infectados. MATERIAIS E MÉTODOS: Trata-se de um estudo de campo, exploratório, transversal e quantitativo, realizado de fevereiro a abril de 2021, com fisioterapeutas intensivistas de dois hospitais públicos (um municipal e outro estadual) da cidade de Fortaleza. Foi realizado um cálculo do fluxo de fisioterapeutas atuantes nessas unidades, estimando-se uma amostra de 71 profissionais para compor o estudo. Participaram da pesquisa os Fisioterapeutas Intensivistas que realizaram assistência a pacientes com COVID-19 e que possuíam vínculo com os referidos hospitais. Foram excluídos os fisioterapeutas na função de residentes, estagiários e preceptores presentes. Para coleta de dados utilizou-se formulário on-line viabilizado pela plataforma Google Forms, composto por perguntas objetivas relacionadas aos dados do profissional, infecção destes por COVID-19, presença de fatores de risco, modificações na prática clínica durante assistência, desafios enfrentados, situações vivenciadas e segurança durante atendimento ao paciente crítico com COVID-19. Os dados foram tabulados através do Software Statistical Package for the Social Sciences (SPSS), versão 20.0. Utilizou-se estatística descritiva com frequências absolutas e relativas. RESULTADOS: Participaram do estudo 59 fisioterapeutas intensivistas atuantes na linha de frente contra a COVID-19; destes, 40,6% foram infectados pelo vírus SARS-CoV-2. As principais modificações relatadas foram melhora no manejo da ventilação mecânica (86,4%) e melhor integração com equipe multidisciplinar (62,7%). Dentre os desafios enfrentados durante a assistência, o contexto completamente novo (89,9%) e o testemunho de experiências dolorosas de pacientes (76,3%) foram os mais marcantes. CONCLUSÃO: A pandemia proporcionou aos fisioterapeutas intensivistas novos desafios e modificações na prática clínica, requerendo atualização, implantação de novas práticas e maior interação em equipe multidisciplinar
