SUMMARYThe aim of this study was to compare the shortterm clinical efficacy and safety of risperidone with haloperidol and placebo. A meta-analysis of seven published randomized double-blind controlled trials was carried out. Study quality was assessed. The proportion of patients failing to reach at least 20% improvement on the positive and negative syndrome scale (P ANSS) or brief psychiatric rating scale (BPRS), the proportion of patients discontinuing treatment because of adverse effects and the number of patients who needed antiparkinsonian medication were abstracted for use as outcome measures.Treatment failure was present in 50% of risperidone-treated patients compared to 66% in those treated with haloperidol and 83% in those treated with placebo. It would be necessary to treat 11 patients with risperidone to prevent one treatment failure in those patients treated with haloperidol (Odds ratio (0R)=0·74, 95% CI of 0·58-0·94, P=0·02). Pooling of the three multicentre trials which included placebo as a treatment arm, showed that one in three patients treated with risperidone 4-16 mg/day (OR=0·22, 95% CI of 0·13-o·39, P<0·00001) and one in six treated with haloperidol 1o-20 mg/day (OR=0·44, 95% CI of 0·22-o·84, P=0·02) would derive significant benefit. Childhood maltreatment is a key predictor of mental health problems across the life span. Yet, how maltreatment types independently and jointly influence the risk for psychiatric problems remains unclear. The aim of the study was two-fold: first, to replicate recent findings regarding the impact of maltreatment types on youth psychiatric symptoms, based on a Brazilian sample of high-risk adolescents (n = 347; age range = 11-17 yrs), and second, to extend existing findings by examining whether this relationship is mediated by bullying victimization and/or perpetration. Measures included self-report ratings of childhood maltreatment and peer victimization, as well as multi-informant reports of internalizing and externalizing symptoms. Consistent with prior research, we found that: (i) maltreatment types often co-occurred; (ii) there was a linear association between number of maltreatment types experienced and symptom severity (i.e. cumulative effect); and (iii) emotional abuse emerged as the most consistent independent predictor of poor mental health across domains, raters, and gender. Additionally, this study extends previous findings by showing that the influence of maltreatment on psychiatric outcomes is partially mediated by peer victimization, but not by bullying perpetration. In conclusion, these findings expand our understanding of the heterogeneity in individual responses to maltreatment as well as highlighting emotional abuse as an important predictor of poor mental health.
SUMMARY
Haloperidol, the most studied antipsychotic drug, is the only one about which reliable statements on the relationship between blood levels and clinical outcome can be made. A systematic overview was undertaken to determine whether there was an optimum blood concentration range for clinical efficacy. Eighteen published studies which provided individual patient data in tables or graphs were reviewed. Clinical benefits tended to decline when the haloperidol blood concentration was increased above 26 ng/ml. Our data support the existence of a therapeutic window between 4 and 26 ng/ml for haloperidol in the treatment of schizophrenic, schizoaffective and schizophreniform disorders.
<em>Corynebacterium pseudotuberculosis</em> é um cocobacilo gram-positivo, patógeno intracelular facultativo de macrófagos, filogeneticamente relacionado com o Mycobacterium tuberculosis. É amplamente distribuído em algumas espécies de animais, causando a linfadenite caseosa em ovinos e caprinos. A linfadenite caseosa, de ocorrência mundial, é uma doença infecciosa crônica cuja transmissão se dá principalmente através da pele. No presente experimento, foram estudados aspectos do reconhecimento antigênico, pela resposta humoral, em caprinos criados em regime extensivo, naturalmente infectados ou imunizados com uma vacina viva (cepa 1002), atenuada, desenvolvida pela Empresa Baiana de Desenvolvimento Agrícola (EBDA). As amostras de soros dos animais imunizados e de controle, coletadas mensalmente por um período de doze meses, foram analisadas pelo ensaio imunoenzimático ELISA e pelo Western blotting, utilizando-se como antígenos o extrato bacteriano contendo exotoxina e sonicado bacteriano de uma cepa selvagem e da cepa 1002. A análise através do Western blotting revelou que o padrão de antígenos reconhecidos pelos anticorpos séricos de animais imunizados, sem raça definida (srd) ou de duas raças puras, bem como animais srd naturalmente infectados, é semelhante, verificando-se bandas protéicas com pesos moleculares entre 20 e 94 kDa. Esta análise possibilitou observar-se que diferentes proteínas são reconhecidas ao longo da imunização.
<span class="texto">A linfadenite caseosa (LC), comumente chamada de mal-do-caroço, é uma doença que acomete ovinos e caprinos e tem como agente etiológico Corynebacterium pseudotuberculosis. É um patógeno intracelular facultativo de macrófagos e é filogeneticamente relacionado com Mycobacterium tuberculosis. É uma doença de ocorrência mundial, caracterizada por formação de lesões necróticas encapsuladas caseosas, localizada principalmente nos linfonodos externos e pulmões. A transmissão se dá provavelmente através da pele, pelo contato de animais sadios com animais portadores de abscessos que evoluem a supuração. Eventualmente, tem sido encontrada em bovinos, porém é em ovinos e em caprinos que assume importância econômica e sanitária. Machos e fêmeas podem apresentar a doença em qualquer estação, porém encontram-se mais doentes com o desenvolver da idade, principalmente após um a dois anos de vida. A enfermidade causa sérios prejuízos a essas espécies, constituindo uma limitação zoopatológica às suas explorações. Com cerca de 80% do rebanho nacional, a região Nordeste é a maior detentora de caprinos do Brasil, onde esses animais são essenciais ao sustento do pequeno agricultor. Corynebacterium pseudotuberculosis causa consideráveis prejuízos econômicos à região, que vão desde a condenação de pele e carcaças em razão de abscessos, até perdas em eficiência na reprodução ou na produção de carne e leite. O presente trabalho teve como objetivo avaliar em caprinos a resposta imune humoral induzida pela vacina viva liofilizada, elaborada com a cepa 1002, da Empresa Baiana de Desenvolvimento Agrícola (EBDA). Os resultados evidenciaram que o referido preparado liofilizado promove a produção de anticorpos específicos em doses iguais ou maiores do que 109 CFU e que o tratamento ministrado não provocou lesões nos animais vacinados.</span>
