RESUMOOBJETIVO: Realizar a validação de conteúdo dos Critérios de Beers 2012 e STOPP 2006 para a obtenção de critérios nacionais de classificação de medicamentos potencialmente inapropriados (MPI) para idosos. MÉTODOS: A técnica Delphi modificada em duas etapas foi utilizada para o estabelecimento do Consenso Brasileiro de Medicamentos Potencialmente Inapropriados para Idosos, com base nos critérios de Beers 2012 e STOPP 2006. O painel de especialistas foi composto por dez indivíduos. Utilizou-se um questionário eletrônico em que cada especialista emitiu uma nota, por meio de uma escala de Likert de cinco pontos. Na primeira etapa Delphi, os participantes foram solicitados a avaliar a inadequação potencial de uma lista preliminar de medicamentos e propor sugestões. Posteriormente, calculou-se a média das notas e o respectivo IC de 95% para cada critério. Foram encaminhados para a segunda rodada os critérios com limite máximo de IC95% < 4,0. Todos os medicamentos contidos em critérios com um limite mínimo de IC95% ≥ 4,0 na segunda rodada foram classificados como potencialmente inapropriados. RESULTADOS: Os critérios que não obtiveram consenso foram: uso de aspirina para a prevenção primária de eventos cardiovasculares, prescrição de escala móvel de insulina, uso de alfa-bloqueadores, estrógenos orais ou transdérmicos para idosos com incontinência urinária e antidepressivos tricíclicos para aqueles com retenção urinária. Ao final, foram totalizados 118 critérios, sendo 43 independentes de condição clínica e 75 dependentes de condição clínica/doença dos idosos. CONCLUSÃO: Foi realizada a validação de conteúdo de critérios de medicamentos que devem ser evitados em idosos. Esses dados podem otimizar a prescrição de medicamentos nessa população. PALAVRAS-CHAVE: lista de medicamentos potencialmente inapropriados; idoso; Brasil. An electronic questionnaire was used, in which each expert scored the criteria through a 5-points Likert scale. In the first round, the participants were asked to assess the potential inadequacy of a preliminary list of medicines and propose suggestions. Then, for each criterion, the scores mean was calculated and its respective 95% CI. The criteria with a 95% CI < 4,0 upper limit were submitted to the second round. All medicines in the criteria with a 95% CI ≥ 4,0 lower limit, in the second round, were classified as potentially inappropriate. RESULTS: The criteria that did not reach consensus were: aspirin use for primary prevention of cardiovascular events, sliding scale insulin prescription and the use of alpha-blockers, oral or transdermal estrogens and tricyclic antidepressants for the elderly with urinary tract incontinence. At the end, there were 118 criteria, in which 43 were independent from clinical conditions and 75 were dependent from clinical conditions/disease of the elderly. CONCLUSION: It was validated a criteria of medicines that should be avoided in the elderly. These data can optimize the medicines prescription in this population.
The data collected in this study indicated a high prevalence of the use of PIM. The factors that contributed the most to this prevalence included medical prescriptions, polypharmacy, medications supplied by the Brazilian National Health System, and black skin colour (specifically, being of African descent).
The 2012 Beers criteria identified more PIMs than the STOPP criteria. This difference highlights the need to develop national criteria.
IntroductionAs non-communicable disease (NCD) burden rises worldwide, community-based programmes are a promising strategy to bridge gaps in NCD care. The HealthRise programme sought to improve hypertension and diabetes management for underserved communities in nine sites across Brazil, India, South Africa and the USA between 2016 and 2018. This study presents findings from the programme’s endline evaluation.MethodsThe evaluation utilises a mixed-methods quasi-experimental design. Process indicators assess programme implementation; quantitative data examine patients’ biometric measures and qualitative data characterise programme successes and challenges. Programme impact was assessed using the percentage of patients meeting blood pressure and A1c treatment targets and tracking changes in these measures over time.ResultsAlmost 60 000 screenings, most of them in India, resulted in 1464 new hypertension and 295 new diabetes cases across sites. In Brazil, patients exhibited statistically significant reductions in blood pressure and A1c. In Shimla, India, and in South Africa, country with the shortest implementation period, there were no differences between patients served by facilities in HealthRise areas relative to comparison areas. Among participating patients with diabetes in Hennepin and Ramsey counties and hypertension patients in Hennepin County, the percentage of HealthRise patients meeting treatment targets at endline was significantly higher relative to comparison group patients. Qualitative analysis identified linking different providers, services, communities and information systems as positive HealthRise attributes. Gaps in health system capacities and sociodemographic factors, including poverty, low levels of health education and limited access to nutritious food, are remaining challenges.ConclusionsFindings from Brazil and the USA indicate that the HealthRise model has the potential to improve patient outcomes. Short implementation periods and strong emphasis on screening may have contributed to the lack of detectable differences in other sites. Community-based care cannot deliver its full potential if sociodemographic and health system barriers are not addressed in tandem.
Resumo O presente estudo tem como objetivo caracterizar o perfil epidemiológico de vítimas de acidentes escorpiônicos e ofídicos e avaliar a adequação das prescrições de soros antivenenos. Estudo transversal cujas fontes de dados foram as fichas de notificação de acidentes por animais peçonhentos do Sistema de Informação de Agravos de Notificação no município de Vitória da Conquista (BA), Brasil. Foram incluídas as informações de acidentes escorpiônicos ou ofídicos no período entre julho de 2016 e junho de 2017 atendidos no município. Os dados obtidos e as variáveis de interesse foram analisadas de acordo com as perguntas deste estudo. No período observado foram atendidas 293 vítimas de acidentes por animais peçonhentos. Destas, 149 (50,9%) foram homens e 114 (38,9%) possuíam entre 20 a 59 anos. Foram 235 (80,9%) casos de escorpionismo e 58 (19,1%) de ofidismo. Destes, 203 (69,3%) foram classificados como leves e em 200 (68,5%) casos foi prescrita soroterapia para estes pacientes. Quanto à adequação das prescrições, 172 (59,7%) foram julgadas inadequadas e destas, o uso de número de ampolas acima do indicado foi a mais frequente. A prescrição inapropriada de soros antivenenos ocorreu em aproximadamente em 60% dos casos avaliados. Apesar disso, a maioria dos acidentes foi classificada como leve, em homens jovens.
The aim of this study was to use indicators to evaluate physician adherence to prescription guidelines for heart failure treatment in a university hospital. This was a prospective cohort study conducted in a university hospital. The information collected at the time of patient admission, including therapeutic indication, absolute contra indications and intolerance, was utilised for the formulation of a guideline adherence indicator (GAI). This indicator was calculated as follows: (the number of patients who used the medication/the number of eligible patients) x 100. The percentage of eligible patients was calculated using the following formula: (the number of eligible patients/the total number patients) x 100. The GAI was applied to a population of 53 patients. Inhibitors of angiotensin-converting enzyme/angiotensin receptor blocker (ACE-I/ARB) combination therapy were used in the greatest percentage of eligible patients (92.4%) and demonstrated the largest GAI value (73.5%). The percentages of patients who were eligible for beta-blockers, spironolactone and digitalis treatments were 81.1%, 52.8% and 60.4%, respectively. The GAI values for the use of beta-blockers, spironolactone and digitalis were 60.4%, 57.1% and 56.2%, respectively. For the studied patient population, the GAI was consistent with the proportion of patients who were eligible to receive digitalis and spironolactone.Uniterms: Heart failure/treatment. Health care. O objetivo deste estudo foi utilizar indicadores para avaliar a adesão de médicos às diretrizes para tratamento da Insuficiência Cardíaca em um hospital universitário. Estudo de coorte prospectivo, conduzido em um hospital universitário. As variáveis coletadas na admissão do paciente foram: indicação terapêutica, contraindicações absolutas e intolerância, sendo utilizadas para elaboração de um indicador de adesão à diretriz (IAD). Este indicador foi calculado através da relação: (número de pacientes que utilizaram o medicamento/número de pacientes elegíveis) x 100. A % de pacientes elegíveis foi calculada da seguinte forma: (número de pacientes elegíveis/número total de pacientes) x 100. Os IAD foram aplicados para uma população de 53 pacientes. Os Inibidores da Enzima Conversora de Angiotensina/ Bloqueadores dos Receptores de Angiotensina foram as classes farmacológicas que apresentaram maior % de pacientes elegíveis (92,4%) e apresentaram o melhor IAD (73,5%). A % de pacientes elegíveis para beta-bloqueadores, espironolactona e digitálicos foi de 81,1%, 52,8% e 60,4%, respectivamente. O IAD para beta-bloqueadores foi 60,4%, espironolactona 57,1% e digitálicos 56,2%. The usage criteria for amikacin recommended by ASHP were validated by the Delphi technique for utilization in Brazilian hospital settings. The Delphi technique applied to validate a questionnaire instrument for monitoring the correct use of a specific strategic antibiotic indicated for the treatment and prophylaxis of serious antibiotic-resistant Gram-negative bacteria, proved to be a reliable and simple tool for designing guidelines and a consensus document for hospital use of antibiotics.
