Objectives: to verify the association between knowledge and adherence to foot self-care practices performed by patients with diabetes mellitus type 2. Methods: cross-sectional, descriptive study carried out with 197 patients in basic health units located in the Northeast region of Brazil. For data collection, we used a semi-structured questionnaire that addressed issues inherent to knowledge and Diabetes Self-Care Activities. Results: we observed that patients with moderate knowledge about self-care practices were more likely to perform foot self-examination, dry the interdigital spaces, moisturize their feet with creams and oils, observe the presence of mycosis and ingrown toenail when compared to patients with insufficient knowledge. Conclusions: the patients' level of knowledge was closely related to the self-care activities carried out, which reinforces the importance of nurses working on training those on essential health care.
Objective: to characterize the differences in the clinical and epidemiological profile of cases of death that had tuberculosis as an immediate or associated cause, and to analyze the spatial distribution of the cases of death from tuberculosis within the territories of Ribeirão Preto, Brazil. Method: an ecological study, in which the population consisted of 114 cases of death from tuberculosis. Bivariate analysis was carried out, as well as point density analysis, defined with the Kernel estimate. Results: of the cases of death from tuberculosis, 50 were the immediate cause and 64 an associated cause. Age (p=.008) and sector responsible for the death certificate (p=.003) were the variables that presented statistically significant associations with the cause of death. The spatial distribution, in both events, did not occur randomly, forming clusters in areas of the municipality. Conclusion: the difference in the profiles of the cases of death from tuberculosis, as a basic cause and as an associated cause, was governed by the age and the sector responsible for the completion of the death certificate. The non-randomness of the spatial distribution of the cases suggests areas that are vulnerable to these events. Knowing these areas can contribute to the choice of disease control strategies.
OBJECTIVE: To characterize deaths from pulmonary tuberculosis, according to sociodemographic and operational variables, in the city of São Luís, Brazil, and to describe their spatial distribution. METHODS: This was an exploratory ecological study based on secondary data from death certificates, obtained from the Brazilian Mortality Database, related to deaths from pulmonary tuberculosis. We included all deaths attributed to pulmonary tuberculosis that occurred in the urban area of São Luís between 2008 and 2012. We performed univariate and bivariate analyses of the sociodemographic and operational variables of the deaths investigated, as well as evaluating the spatial distribution of the events by kernel density estimation. RESULTS: During the study period, there were 193 deaths from pulmonary tuberculosis in São Luís. The median age of the affected individuals was 52 years. Of the 193 individuals who died, 142 (73.60%) were male, 133 (68.91%) were Mulatto, 102 (53.13%) were single, and 64 (33.16%) had completed middle school. There was a significant positive association between not having received medical care prior to death and an autopsy having been performed (p = 0.001). A thematic map by density of points showed that the spatial distribution of those deaths was heterogeneous and that the density was as high as 8.12 deaths/km2. CONCLUSIONS: The sociodemographic and operational characteristics of the deaths from pulmonary tuberculosis evaluated in this study, as well as the identification of priority areas for control and surveillance of the disease, could promote public health policies aimed at reducing health inequities, allowing the optimization of resources, as well as informing decisions regarding the selection of strategies and specific interventions targeting the most vulnerable populations.
BackgroundIn Brazil, people still fall ill and die from tuberculosis (TB), and this can be explained by the significant impasse in the equity of distribution of therapeutic resources to the population as a whole. The aim was to identify geographical areas which have shown progress in terms of equity (of income, schooling and urban occupancy) and test its effect on mortality from TB in a municipality of southeast Brazil.MethodsIt is an ecological study considering TB as the basic cause for deaths registered between 2006 and 2013 on the Mortality Information System and other variables obtained through the Demographic Census of the Brazilian Institute of Geography and Statistics (2010). The geographical area for analysis comprised the areas of coverage of the health services. Social indicators have been constructed through the Principal Component Analysis (PCA). The cases were geocoded and the annual mortality rate from TB was calculated with smoothing using the local empirical Bayesian method. Multiple linear regression was then performed. There was confirmation of the existence of spatial dependence of residue through the application of the Global Moran I test, and application of the Models with Global Spatial Effects, to identify the best standard of spatial regression.ResultsThe mortality rates ranged from 0.00 to 2.8 deaths per 100,000 people, per year. In the PCA, three indicators were constructed, and designated as indicators of income, social inequality, and social equity. In multiple linear regression, the indicator of social equity was statistically significant (P < 0.0001) but had a negative association, an adjusted R2 of 28.36% and with spatial dependence (Moran I = 0.21, P = 0.003455). The best model to deal with existing spatial dependence was the Spatial Lag Model.ConclusionsThe better social conditions have shown progress in reducing mortality from TB, thereby reinforcing the achievement of Sustainable Development Goals. Resumo Este estudo objetivou descrever a prevalência da hanseníase e verificar os fatores associados às formas clínicas multibacilares em cenário da Estratégia Saúde da Família, prioritário para o controle e a vigilância da doença no nordeste brasileiro. Trata-se de estudo transversal, que utilizou dados do Sistema de Informação de Agravos de Notificação relativos aos casos de hanseníase notificados em Imperatriz, Maranhão, entre 2008 e 2017. Determinaram-se as prevalências a cada ano e para o período. Para associar as variáveis e as formas clínicas multibacilares, utilizaram-se modelos de regressão de Poisson, com nível de significância de 5%. Dos 2.476 casos de hanseníase analisados, a maioria referiu-se às formas clínicas multibacilares. A prevalência variou entre 15,6 e 7,8/10 mil habitantes, encontrando-se níveis alto e muito alto de endemicidade. As variáveis sexo masculino, faixas etárias entre 30 e 59 anos e ≥60 anos, escolaridade <8 anos, grau 2 de incapacidade física, episódio reacional tipos 1 e 2 e zona de residência urbana apresentaram associações significativas (p≤0,05) com as formas clínicas multibacilares. Tais achados podem servir de base para elaboração e implementação de medidas de controle e vigilância da hanseníase, direcionando as ações para os grupos mais vulneráveis e tornando-se mais efetivas.
Introdução: Bancos de leite humano são responsáveis pela promoção do aleitamento materno, coleta, processamento e controle de qualidade do leite distribuído, apresentando ainda função importante no contexto da saúde pública, no âmbito da promoção à saúde infantil, como estratégia de segurança alimentar e nutricional. Objetivo: Identificar os fatores que contribuem para a doação de leite materno, as vantagens e as dificuldades percebidas pelas doadoras, além de conhecer o seu perfil sociodemográfico. Casuística e Métodos: Estudo descritivo, transversal, quantitativo, realizado com 50 doadoras de um hospital da Região Nordeste do Brasil, credenciado na iniciativa Hospital Amigo da Criança, entre janeiro e março de 2014. Foi realizada uma entrevista individual no domicílio da doadora para preenchimento de formulário estruturado, que continha questões de caracterização pessoal, conhecimento sobre banco de leite humano e doação de leite, e motivos e desvantagens para a doação. Resultados: Os fatores que contribuíram para a doação de leite materno foram o fato de a mulher ser primípara (54,0%), ter recebido orientação sobre aleitamento materno (78,0%) e sobre o banco de leite humano (68,0%), e ter recebido apoio para doação (80,0%). Dentre as vantagens apontadas pelas entrevistadas para doação do leite materno, 90% acreditavam que beneficiaria a saúde da criança receptora, enquanto que o fato de machucar as mamas (28%) foi apontado como principal desvantagem para este ato. Quanto ao perfil das doadoras, a idade variou de menos de 19 (26,0%) anos e mais de 35 (8%), 46,0% eram casadas, 56,0% eram donas de casa, 48,0% tinham Ensino Médio e 42,0% tinham renda familiar de até um salário mínimo. Conclusão: Os inúmeros benefícios para a saúde do bebê, o amor e a solidariedade foram fatores contribuintes para a doação do leite materno. A doação é uma experiência positiva, e o apoio familiar é fator facilitador à prática da doação.
BackgroundThe early identification of the Breathing Symptoms within the scope of Primary Health Care is recommended, and is also one of the strategies of national sanitary authorities for reaching the elimination of tuberculosis. The purpose of this study is to consider which attributes and which territories have shown the most significant progress in Primary Health Care, in terms of coordination of Health Care Networks, and also check if those areas of Primary Health Care that are most critical regarding coordination, there were more or less cases of avoidable hospitalizations for tuberculosis.MethodsThis is an ecological study that uses primary and secondary data. For analysis, coropletic maps were developed through the ArcGIS software, version 10.2. There was also the calculation of gross annual and Bayesian rates for hospitalizations for tuberculosis, for each Primary Health Care territory.ResultsThere were satisfactory results for attributes such as Population (n = 37; 80.4 %), Primary Health Care (n = 43; 93.5 %), Support System (n = 45; 97.8 %); the exceptions were Logistics System (n = 32; 76.0 %) and Governance System, with fewer units in good condition (n = 31; 67.3 %). There is no evidence of any connection between networks’ coordination by Primary Health Care and tuberculosis avoidable admissions.ConclusionThe results show that progress has been made regarding the coordination of the Health Care Networks, and a positive trend has been shown, even though the levels are not excellent. It was found no relationship between the critical areas of Primary Health Care and tuberculosis avoidable hospitalizations, possibly because other variables necessary to comprehend the phenomena.
Objetivou-se identificar o perfil dos óbitos por tuberculose pulmonar em um município do nordeste brasileiro durante o período de 2005-2014. Trata-se de um estudo descritivo, exploratório, documental, com abordagem quantitativa, realizado por meio do Sistema de Informação sobre Mortalidade na qual a amostra foi composta por 37 óbitos cuja causa básica foi por tuberculose pulmonar. O tratamento dos dados se deu por meio de estatística descritiva simples. Houve prevalência de óbitos com idade média de 60 anos, predomínio da forma pulmonar, sem menção de confirmação bacteriológica ou histológica (81,0%), sexo masculino (65,0%), raça/cor parda (67,0%), estado civil solteiro (51,0%), parcela predominante apresentou ensino médio completo (32,0%) e aposentados (32,0%). Observou-se ainda que a maioria dos óbitos ocorreu no hospital (73,0%), com assistência médica antes do óbito (76,0%), sendo a maior parcela atestada por médico assistente (38,0%) e a maioria absoluta dos óbitos (94,0%) não foi submetida à necropsia. A presente investigação revelou o perfil de uma população fatalmente atingida pela tuberculose pulmonar, evidenciando aspectos epidemiológicos importantes a serem considerados em termos de gestão e organização dos serviços de saúde para a equidade no acesso, desenvolvimento social e intensificação das ações de controle da doença.
