A hypoaemic response in reaction to topical and transitory LLLI alveolar mucose compression was observed. Debt index and the ITSRF% are reproducible indices of microvascular perfusion response whose validation under pathological circumstances remains to be evaluated.
1. Information regarding the use of continuous i.v. administration of nitroglycerine as an antihypertensive agent in the management of pre-eclampsia is scarce. In the present study, i.v. nitroglycerine or sublingual nifedipine were administered to 32 women with severe pre-eclampsia who were being managed with controlled plasma volume expansion and MgSO(4) loading and maintenance doses. Maternal blood pressure and heart rate responses, fetal heart rate responses and perinatal fetal-maternal adverse effects were evaluated using classical parametric and non-parametric data analysis and data modelling by mixed models. 2. An important hypotensive response was observed in both groups, although this reponse was greater, faster and exhibited less variability (more precision) in the nitroglycerine-treated group. Heart rate also increased in both the nitroglycerine- and nifedipine-treated groups (4.6 +/- 4.4 vs 8.6 +/- 5.3 b.p.m., respectively), although the increase in the nifedipine-treated group was almost twofold that in the nitroglycerine-treated group. There were no significant changes in fetal heart rate in response to vasodilator therapy. The frequency of perinatal fetal-maternal adverse effects was similar in both groups at 40% and the adverse effects observed included flushing, headache, palpitations and nausea. 3. In conclusion, i.v. infusion of nitroglycerine is an effective, safe and alternative therapy for severe pre-eclampsia.
The data strongly suggest that, at least in the studied sample, gingivitis and periodontitis operate as antagonistic modulators of gingival perfusion. The major practical implication of our findings is that an increase or decrease in this index (GIPI) at a given attached gingiva site could indicate, respectively, the clinical predominance of gingivitis or periodontitis in such a site.
Resumen: El abandono temprano de la lactancia materna representa un problema de salud pública, tanto en las sociedades desarrolladas, como en vías de desarrollo, las construcciones sociales de género y las desigualdades sociales entre hombres y mujeres contribuyen a la complejidad del fenómeno. Este artículo reflexiona sobre las experiencias que padres y madres viven frente a la reconfiguración de cuerpos, roles y cotidianeidades que implica la lactancia. Se trató de un estudio cualitativo, desde un enfoque fenomenológico interpretativista y con perspectiva de género, realizado con parejas cuyos hijos tenían menos de seis meses de edad, que lactaran o hubiesen brindado lactancia y aceptaran de manera voluntaria participar. Se realizó un análisis fenomenológico sobre “unidades de significado” en las cuatro dimensiones que explora la fenomenología: corporalidad, tiempo, espacio y relaciones. En las mujeres se identificó la existencia de conflicto y culpa asociada a la práctica de la lactancia, sus necesidades de sueño y descanso les hacen vivir la lactancia como una función que las sobrecarga de más en su vida cotidiana. Sus parejas muestran limitaciones derivadas de los constructos sociales de masculinidad para involucrarse en la crianza y respaldarlas para la continuidad de la lactancia. Resulta urgente que los profesionales de la salud problematicen los procesos de maternazgo-paternazgo en la crianza y lactancia, desde una perspectiva incluyente de género que reste importancia al binomio materno-filial y haga especial énfasis en la tríada madre-hijo-padre.
