In locations with a high rate of tuberculosis (TB) and HIV infection, there are a number of strategies to prevent negative outcomes such as opportunistic infections, hospitalizations and death, and this article investigates risk factors for the occurrence of hospitalization and death in cases of TB/HIV co-infection in the south of Brazil. The data are taken from a population-based retrospective cohort study on cases of TB/HIV co-infection from 2009 to 2013 in Porto Alegre, Brazil. Sociodemographic, epidemiological and clinical variables were analyzed. Relative risk (RR) estimates for hospitalization and death were determined by regression models. There were 2,419 co-infection cases, of which 1,527 (63.1%) corresponded to hospitalizations, and 662 (27.4%) to death. The occurrence of hospitalization was associated with ≤ 7 years of schooling (RR = 3.47, 95%CI: 1.97–6.29), 8–11 years of schooling (RR = 2.56, 95%CI: 1.44–4.69), place of origin—district health authorities Northwest/Humaitá/Navegantes/Ilhas (RR = 2.01, 95%CI: 1.44–2.82), type of entry into the surveillance system as in cases of reentry after withdrawal (RR = 1,35, 95%CI: 1.07–1.70), closure in surveillance as in withdrawal of treatment (RR = 1.47, 95%CI: 1.18–1.83) and multidrug-resistant tuberculosis (RR = 3.94, 95%CI: 1.97–8.81). The occurrence of death was associated with age (RR = 1.07, 95%CI: 1,01–1,14), ≤ 7 years of schooling (RR = 3.94, 95%CI: 2.26–7.09), 8–11 years of schooling (RR = 2.84, 95%CI: 1.61–5.16), place of origin—district health authorities Baltazar (RR = 2.05, 95%CI: 1.48–2.86), type of entry in the surveillance system as cases of re-entry after withdrawal (RR = 1.53, 95%CI: 1.22–1.91), relapse (RR = 1.33, 95%CI: 1.03–1.73). The occurrence of hospitalizations and deaths is high among co-infected patients. Our estimation approach is important in order to identify, from the surveillance data, the risk factors for hospitalization and death in co-infected patients, so that they may receive more attention from the Brazilian national healthcare system.
RESUMO: Objetivo: determinar a prevalência de depressão em idosos residentes em uma Instituição de Longa Permanência para Idosos (ILPI). Metodologia: trata-se de um estudo descritivo, transversal, de abordagem quantitativa. A população foi constituída de 48 sujeitos. Para a avaliação do estado mental foi aplicado o Mini Mental State Examination e para a depressão a Escala de Depressão Geriátrica (EDG-15). Foram seguidos os princípios éticos da Resolução 196/96 do Conselho Nacional da Saúde. Resultados: os resultados demonstraram que 25% dos idosos não apresentaram sinais de depressão; 43,75% apresentaram sinais de depressão leve/moderada e 31,25% apresentaram sinais de depressão severa. A prevalência de depressão foi maior em mulheres (55,5%) do que em homens (44,5%). Conclusão: os altos níveis de depressão, identificados na amostra, reportam para a necessidade de planejamento de apoio psicológico, capaz de proporcionar melhor qualidade de vida aos residentes na ILPI estudada, como também alertam para semelhante prevalência nas ILPIs em geral. Objetivo: identificar os fatores associados à cesariana eletiva em mulheres atendidas em um hospital referência do oeste catarinense. Método: estudo transversal realizado com mulheres submetidas à cesariana. A coleta de dados ocorreu entre agosto e setembro de 2016, por meio de questionário padronizado e informações complementadas pelo prontuário e cartão pré-natal. Dados foram submetidos à regressão logística multivariada. Resultados: foram incluídas no estudo 206 mulheres. O resultado final do modelo multivariado mostrou associação de forma independente com a cesariana eletiva ter sido paga via particular ou por plano de saúde (Odds Ratio = 39,9 p0,01) e a cor da pele autorreferida branca (Odds Ratio = 2,94 p=0,035). Conclusão: é necessário repensar a formação dos profissionais da área da saúde habilitados a atenderem gestantes, capacitando-os para o cuidado pré-natal e para o parto vaginal, visando, sobretudo, o respeito às indicações absolutas de cesarianas, especialmente nas gestantes atendidas no setor privado.
Objetivo: compreender a vivência do enfrentamento e repercussões da COVID-19, na percepção de mulheres em tratamento oncológico. Método: estudo qualitativo, do tipo ação-participante, fundamentado no Itinerário de Pesquisa de Paulo Freire, que possui três fases: Investigação Temática; Codificação e Descodificação; Desvelamento Crítico. Foi realizado Círculo de Cultura virtual, com a participação de 12 mulheres em tratamento do câncer de mama, de diferentes localidades do Brasil. Resultados: no Círculo de Cultura virtual discutiram dois temas: desafios no enfrentamento do câncer e da COVID-19; aprendizados gerados nessa vivência, considerando um renascimento das próprias cinzas. Considerações finais: o momento pandêmico tem instigado reflexões sobre o viver. Assim, as mulheres em tratamento oncológico e também em restrição social puderam expressar seus sentimentos, descobrindo e redescobrindo fragilidades e fortalezas para ressignificar e crescer como seres, em uma sociedade, que pode e deve articular estratégias para promoção da saúde.
Objetivo: analisar o conteúdo de campanhas publicitárias sobre tuberculose produzidas no Brasil pelo Ministério da Saúde. Método: apoiadas nos referenciais da Promoção da Saúde e Educação em Saúde, foram realizadas análises das campanhas veiculadas nos anos de 2007, 2008, 2010 e 2011. Resultados: compuseram-se duas categorias temáticas: “Tosse pode ser tuberculose! O tratamento é gratuito e não pode ser interrompido!” e “O doente e a doença: uma composição imagética nas campanhas publicitárias”. Os textos apelam para a conscientização individual e para o autocuidado, baseados na associação entre saúde, ausência de sintomas e prevenção pela adoção de uma postura vigilante. As imagens utilizadas são constituídas de elementos pouco representativos do universo das pessoas acometidas pela doença. Conclusões: essas reflexões contribuem com o campo da saúde e da Enfermagem, em especial, na medida em que produzem uma consciência ética comprometida com a redução de vulnerabilidades e com a proteção da vida.
Objective: to describe the experience of applying of The Silences Framework to underpin health research investigating Tuberculosis/ HIV/AIDS coinfection.Method: the Silences Framework originally developed following a study exploring the decisions and silences surrounding black Caribbean men living in England, discussing the themes 'sexual health' and 'ethnicity'. Following this study, a conceptual theory for research on sensitive issues and health care of marginalized populations was developed called 'Screaming Silences', which forms the foundation of The Silences Framework. 'Screaming Silences' define research areas and experiences that are poorly studied, little understood or silenced.Results: the Silences Framework supports researchers in revealing "silences" in the subjects they study -as such, results may reflect how beliefs, values, and experiences of some groups influence their health. This framework provides the application of four complementary stages: working the silences, hearing silences, voicing silences and working with the silences. The analysis occurs cyclically and can be repeated as long as the silences inherent in a study are not revealed. Conclusion: this article presentsThe Silences Framework and the application of the notion of "sounds of silence", mapping an antiessentialist theoretical framework for its use in sensitive research in health and nursing areas, being a reference for other researchers in studies involving marginalized populations. The Silences Framework define áreas de pesquisa e experiências que são pouco estudadas, compreendidas ou silenciadas.Resultados: os "silêncios" podem refletir aspectos não compartilhados sobre como crenças, valores e experiências de alguns grupos influenciam sua saúde. Esse referencial prevê a aplicação de quatro estágios complementares: trabalhando os silêncios, ouvindo os silêncios, dando voz aos silêncios e trabalhando com os silêncios. A análise ocorre de modo cíclico e pode ser repetida enquanto os silêncios não forem desvelados.Conclusão: este artigo apresenta a noção de "sons do silêncio", mapeando um quadro teórico antiessencialista para sua utilização em pesquisas sensíveis na area da saúde e da enfermagem, podendo ser referência para outros investigadores em estudos envolvendo populações marginalizadas. The objective this study is to describe the actions and intervention projects in mental health in the health services of the cities that are part of Rio Grande do Sul's 15th Regional Health Coordinating Body (15a Coordenadoria Regional de Saúde). This is an exploratory, descriptive and qualitative research. A questionnaire was applied to health professionals and managers who were part of regional mental health forums. From the content analysis as proposed by Minayo, the following categories emerged- one which describes mental health care in the health services of the municipalities, including aspects related to mental disorders, to the organization of the mental health team, to the main developed actions and to the difficulties found; the other category is about intervention projects in mental health that, according to the subjects, will be implanted in the cities. Relevant points the regional network of mental health care were identified which will be helpful in planning and improving care.
