Aim: The aim of the study was to assess the severity of pain experienced by a newborn during a heel puncture for screening using the Newborn Pain Scale (NIPS), measure the heart rate and compare the effectiveness of non-pharmacological methods of pain control. Design: Randomized clinical trial. No experimental factors. The test was performed during routine screening. Surroundings: Provincial Specialist Hospital in Olsztyn. Patients/Participants: Pain was assessed in 90 full-term newborns. The newborns were rooming in with their mothers in the hospital. Interventions: Newborns were divided into three groups. Three different methods of pain relief were used: breastfeeding, 20% glucose administered orally and non-nutritional sucking. Main Outcome Measures: The primary pain outcome was measured using the NIPS and the secondary pain outcome measures (heart rate, oxygen saturation) were measured using a pulse oximeter. Results: During capillary blood sampling from the heel, most newborns, n = 56 (62.2%), experienced no pain or mild discomfort, severe pain occurred in n = 23 (25.6%) and moderate pain occurred in n = 11 (12.2%). No significant statistical differences were found between the degree of pain intensity and the intervention used to minimize the pain p = 0.24. Statistically significant relationships were demonstrated between heart rate variability and the degree of pain intensity (p = 0. 01). There were no statistically significant differences between the newborn’s pain intensity and the mother’s opinion on the effectiveness of breastfeeding in minimizing pain. Conclusions: This study did not answer the question of which pain management method used during the heel prick was statistically more effective in reducing pain. However, the results indicate that each of the non-pharmacological interventions (breastfeeding, oral glucose dosing and non-nutritive sucking) applied during heel puncture resulted in effective pain management in most of the newborns enrolled in the study. The relationship between heart rate variability and the severity of pain was confirmed. Mothers of newborns in the breastfeeding group were satisfied with the pain relief methods used in the child and the opportunity to console their newborn during painful procedures in a technologically invasive environment.
STRESZCZENIECelem pracy jest ukazanie, jak ważną rolę w aspekcie psychicznym i moralno-etycznym odgrywa personel medyczny we wsparciu kobiety i jej rodziny po przebytym poronieniu. Poronienie jest najczęstszym problemem prokreacji. Problem ten należy rozpatrywać nie tylko w aspekcie medycznym, ale także społecznym, rodzinnym i psychologicznym. Można mó-wić o zjawisku żałoby spowodowanej śmiercią bliskiej osoby. W konsekwencji nieprzebytej żałoby lub jej przedłużania może dojść do niewłaściwych zachowań interpersonalnych lub zaburzeń psychicznych (depresja, zespoły lękowe, zespół stresu pourazowego). Podstawowym elementem wsparcia psychicznego kobiety po przebytym poronieniu jest akceptacja jej uczuć, zezwolenie na wyrażanie negatywnych emocji, zapewnienie możliwości kontaktu z osobami najbliższymi. Zarówno personel medyczny, jak i rodzina, wspierając kobietę, często zapominają o ojcu dziecka. Mężczyzna również czuje się osamotniony w swej tragedii. Zamyka swój smutek w sobie. Nieokazywanie uczuć pogłębia depresję. Jeśli kobieta przeży-wająca dramat poronienia ma już dzieci, personel powinien w swej opiece pamiętać i o nich. Dziecku należy wytłumaczyć, że poronienie nie jest niczyją winą. Personel medyczny odgrywa ważną rolę w procesie wsparcia psychicznego kobiety i jej rodziny po stracie dziecka. Profesjonalizm, doświadczenie, wiedza i umiejętność komunikacji interpersonalnej są niezbędne w dążeniu do realizacji takich celów, jak: pomoc w pogodzeniu się ze stratą, zezwolenie na doświadczanie cierpienia i żalu, pomoc w zaakceptowaniu rzeczywistości i życia bez dziecka, mobilizacja do inwestowania energii i uczuć w nowe relacje i związki. Pomocne personelowi medycznemu mogą okazać się kursy i szkolenia z postępowania w przypadku niepowodzenia położniczego. Powinny one uczyć umiejętnej współpracy z pacjentem przeżywającym stratę dziecka.SŁOWA KLUCZOWE: poronienie, żałoba, ciąża, wsparcie psychiczne. Miscarriage is the most common problem of procreation. This problem should be considered not only in the medical aspect but also the social, family and psychological ones. You can talk about the phenomenon of mourning due to the death of a loved one. A consequence of unexperienced mourning, or its renewal may be inappropriate interpersonal behavior or mental disorders (depression, anxiety syndromes, and post-traumatic stress disorder). The basic element of psychological support of women with a history of miscarriage is the acceptance of her feelings, permission to express negative emotions, providing the possibility of contact with the closest people. Both medical staff and family supporting women often forget about the child's father. The man also feels alone in their tragedy. He closes his sadness in himself. If a woman experiencing a miscarriage drama has children, staff should remember about them. The child should be explained...
STRESZCZENIECelem pracy jest przedstawienie głównych zasad etycznych i odniesienie ich do dylematów związanych z podjęciem lub zaniechaniem resuscytacji w neonatologii. Szybki postęp w neonatologii zwiększył odsetek przeżywalności noworodków. Przed 1970 rokiem odstąpienie od leczenia skrajnych wcześniaków było "normą". Przełomowy dla postaw wobec noworodków był rok 1981 i śmierć dziecka "Baby Doe". Zapoczątkowa-no wtedy epokę nakazu -wszystko, co możliwe dla utrzymania dziecka przy życiu, musi być zrobione. Dopuszcza się wstrzymanie leczenia, jeśli nie ma szansy na przeżycie noworodka. Znamy cztery główne zasady etyczne. Prawo pacjenta do wyrażania zgody na proponowane leczenie dotyczy osób świadomych, zdolnych do podejmowania decyzji. Noworodek jest pacjentem niezdolnym do wyrażenia swej zgody na proponowane leczenie czy resuscytację. Głos w jego imieniu przejmują rodzice oraz zespół leczący. Zasada nieczynienia krzywdy oznacza działanie w najlepszym interesie pacjenta. W myśl tej zasady resuscytacja nie powinna być podejmowana w sytuacjach nierokujących powodzenia. Należy zrezygnować z uporczywej terapii. Zasada sprawiedliwości -resuscytacja powinna być dostępna dla każdego człowieka, jeśli tylko jest w stanie przynieść mu korzyść. Leczenie pacjenta powinno zawsze odbywać się z poszanowaniem jego osoby. Oprócz prawa do bycia szczęśliwym i zdrowym człowiek ma przede wszystkim prawo do życia. Według nauczania Kościoła katolickiego przyznajemy osobowe prawa człowiekowi już od chwili zapłodnienia. Te same prawa głosi zasada świętości życia ludzkiego. Życie jest darem Boga, należy je chronić, gdyż jest cenne samo w sobie.
