La maladie de Verneuil ou l'hidrosadénite suppurative est une affection naissant dans les glandes sudoripares apocrines. Le diagnostic est souvent fait au stade de chronicité. La recherche d'autres localisations doit être systé-matique. Le traitement est essentiellement chirurgical et astreignant, consistant en des excisions larges et parfois répétées. La récidive est fréquente. Notre étude rétrospective concerne 40 cas de maladie de Verneuil, tous de sexe masculin avec un âge moyen de 32 ans. La clinique était dominée par la présence de plusieurs fistules au niveau de la région périanale. Le traitement était chirurgical par des exérèses larges associées à des soins locaux quotidiens. Les résultats étaient satisfaisants, et le délai de cicatrisation variait entre 45 et 60 jours. Le recul était de dix ans, et on avait noté sept récidives opérées par la même technique avec de bons résultats. The diagnosis is often made after some chronic episodes. Search for other locations in the body must be systematic. The treatment is essentially surgical and constrained, consisting of large excisions, and sometimes repeated. Recurrence is common. Our study is based on 40 cases of Verneuil's disease, all male, whose average age is 32 years. Clinical examination was dominated by the presence of several fistulas in the perianal region. Treatment was surgical, associated with large excisions, requiring daily local care. Results were satisfactory and the time for scar tissue development varied between 45 and 60 days. Thefollow-up was of one to ten years and we noted seven cases of recurrence, operated using the same technique and with good results.
Il s'agit d'une étude rétrospective concernant 36 cas colligés sur une période de 20 ans (1989)(1990)(1991)(1992)(1993)(1994)(1995)(1996)(1997)(1998)(1999)(2000)(2001)(2002)(2003)(2004)(2005)(2006)(2007)(2008). Cette localisation hydatique occupe la quatrième place après le foie, le poumon et le muscle. Sa fréquence est de 5 %. L'âge moyen de nos malades est de 36 ans, avec des extrêmes allant de 24 à 60 ans. L'examen clinique se résume à une splénomégalie dans 35 % des cas ou une douleur de l'hypochondre gauche dans 92 % des cas. L'imagerie médicale a permis de poser le diagnostic dans tous les cas et de préciser le siège exact du kyste. L'échographie abdominale était toujours concluante, ainsi que le scanner chaque fois qu'il était réalisé. Le traitement de cette affection est chirurgical. Nous avons réalisé dix splénectomies totales, 15 splénectomies partielles, une résection du dôme saillant dans sept cas et quatre périkystec-tomies partielles. Les suites étaient simples avec une mortalité nulle et une morbidité faite de trois infections pulmonaires bien contrôlées par une antibiothérapie adaptée.Mots clés Rate · Splénectomie · Échinococcose · Kyste hydatique · Diagnostic et traitement Abstract Our study was conducted between 1989 and 2008 in 36 cases of hydatid spleen cysts. Spleen is in the fourth rank among the organs affected by hydatid cysts after liver, lungs, and muscle and has the incidence rate of 5%. The average age is 36 years with extremes of 24 and 60. The clinical signs are most often splenomegalia in 35% and pain in the left hypochondrium in 92% of the cases. Ultrasound scan and computed tomography scan confirmed the diagnosis. The surgical treatment consisted of 10 total splenectomy and 15 partial splenectomy. RDS was performed in seven cases and partial pericystectomy on four cases. The results were good with no mortality and morbidity consisting of tree lung infection with good improvement by antibiotic.
