Résumé -Introduction : L'essor de l'implantologie au cours des dernières années a conduit à une augmentation du champ de ses indications, repoussant notamment les contre-indications liées aux terrains pathologiques des patients. Cet article propose une revue de la littérature et une mise au point sur l'implantologie dans un contexte de pathologies de la muqueuse buccale. Méthodologie : À partir d'une recherche bibliographique sur la base de données MEDLINE sans limite de période, les articles traitant de l'implantologie chez des patients atteints de pathologies de la muqueuse buccale ont été étudiés. Résultats : L'implantologie chez ces patients a été peu décrite. Les données de cette revue de la littérature tendent à mettre en évidence un apport indéniable de la mise en place d'implants chez les patients atteints de pathologies de la muqueuse buccale. Discussion : Historiquement considérées comme des contre-indications à la pose d'implants, ces pathologies ne doivent plus aujourd'hui être considérées comme un frein à une réhabilitation prothétique implanto-portée. Conclusion : Toutefois, une maintenance et une hygiène rigoureuses doit être instaurée afin de contrôler les implants et l'éventuelle réactivation de la pathologie.Abstract -Implantology and oral mucosal lesions: a literature review. Introduction: Recent developments in implantology have led to an extension of its indications, especially regarding special care patients. This paper offers a review of the literature on the subject of implantology in cases of oral mucosal lesions. Methods: A review of the literature was performed using MEDLINE, without a date range, searching for articles describing oral implant treatments in patients with oral mucosal alterations. Results: Few cases of oral implant treatments in such patients are described. The results of the literature review showed that oral implant treatments greatly enhance the quality of life of these patients. Discussion: Although these pathologies used to be considered contraindications to dental implant placement, recent data shows the feasibility of such treatments. Conclusion: Nonetheless, both strict oral hygiene and regular maintenance are essential to intercept any potential pathology reactivation.
--Fanconi anemia is an extremely rare genetic disorder that has several modes of presentations, and it can lead to multiple hematological malignancies and malignant solid tumors, especially squamous cell carcinoma. Erythroleukoplakia is a rare oral mucosal disorder, associated with a high risk of malignant transformation. We report a rare case of localized gingival erythroleukoplakia in a 57-year-old female patient with Fanconi anemia that eventually revealed to be gingival in situ squamous cell carcinoma. Early surgical excision was performed but two local recurrences occurred. Early treatment of the recurrences allowed for adequate management of the oral malignancy. The present case aims to underline the need for thorough clinical examination and regular follow-up sessions in the early detection of premalignant and malignant conditions, especially in patients with diseases predisposing to oral cancer.
Résumé -Introduction :Les ulcérations des muqueuses buccales peuvent être dues à une cause locale mais également être la manifestation de certaines pathologies générales ou de leurs thérapeutiques. Des maladies infectieuses, dermatologiques, hématologiques, carentielles, malignes et la prise de certains médicaments sont susceptibles de provoquer des ulcérations buccales, d'évolution aiguë ou chronique. De nombreux médicaments peuvent induire des ulcérations buccales, associées ou non à d'autres atteintes, cutanées et/ou muqueuses. Observation : Une patiente de 85 ans consultait pour une ulcération linguale évoluant depuis 5 semaines. Ce travail présente la démarche diagnostique qui a permis d'aboutir au diagnostic d'ulcération d'origine médicamenteuse liée à la prise de nicorandil. Discussion : Devant une ulcération muqueuse, l'anamnèse et l'examen clinique orientent le diagnostic. Les examens complémentaires permettent de retenir la cause médicamenteuse, après l'exclusion des autres étiologies possibles. La démarche diagnostique peut s'avérer difficile, notamment chez les patients qui souffrent de nombreuses pathologies et sont polymédiqués. Conclusion : L'implication du nicorandil dans la survenue d'ulcérations buccales ou extra-buccales est mal connue par les praticiens, ce qui explique le retard dans le diagnostic et la prise en charge adaptée des patients.Abstract -Diagnostic approach to a single chronic ulcer of the tongue. Résumé -Introduction : L'artérite à cellules géantes (ACG) est une pathologie inflammatoire chronique vasculaire affectant le patient à partir de la 5 ème décennie. Le tableau clinique, très varié et parfois atypique, peut induire des erreurs diagnostiques avec de lourdes conséquences. Observation : Une patiente de 63 ans s'était présentée à la consultation de chirurgie buccale après une errance diagnostique pour des douleurs cervicales postérieures et occipitales, irradiant ensuite au maxillaire droit, interprétées initialement comme d'origine dentaire. Le tableau clinique, réétudié lors de la consultation, montrait des modifications de l'artère temporale superficielle faisant suspecter une ACG. La patiente fut directement orientée vers le service de rhumatologie pour la confirmation du diagnostic et la prise en charge immédiate. Discussion : Les manifestations de l'ACG sont le reflet de l'atteinte de nombreuses artères issues de l'arc aortique, responsable de tableaux cliniques très variés. Les conséquences graves à court terme de cette affection (cécité irréversible, accident vasculaire cérébral) imposent une prise en charge urgente et efficace par une corticothérapie systémique. Conclusion : La diversité des manifestations de l'ACG peut conduire le patient à consulter en premier lieu le chirurgien-dentiste qui doit savoir inclure cette pathologie dans les diagnostics différentiels des douleurs oro-faciales et adresser rapidement le patient. Abstract -Atypical manifestations of giant cell arteritis: Case report and literature review. Introduction:Giant cell arteritis (GCA) is a vasculitis commonly affecting patients after the fifth decade. Atypical clinical manifestations can induce a wrong diagnosis that can lead to dramatic damages. Observation: A 63-year-old woman had been sent to oral surgery department for maxillary pain, after being seen by a dentist and an endodontist: a dental origin had wrongly been suspected. Besides several symptoms (orofacial pain, asthenia, anorexia), clinical abnormalities of the right temporal artery evoked the diagnosis of GCA. She had been sent quickly to the rheumatology service for confirmation of the diagnosis and special care. Discussion: Inflammation of branches of aortic artery lead to several clinical manifestations, resulting sometimes in atypical expressions. The potentially severe consequences of the disease (irreversible blindness, ischemic stroke) compel an immediate corticosteroid therapy. Conclusion: The large variety of clinical manifestations of the GCA has to be known by the dental surgeon, to be able to identify atypical symptoms and to send his patient as soon as possible to the specialist.
Résumé -Introduction :La personne âgée est atteinte de polypathologies et polymédiquée. Les prescriptions sont faites par plusieurs médecins qui s'ignorent réciproquement, ce qui augmente le risque d'effets adverses. Observation : Le cas d'une patiente de 91 ans vivant dans un établissement d'hébergement pour personnes âgées dépendantes est rapporté. Quatre mois après une perfusion de zolédronate suite à une chute avec tassement vertébral, elle a consulté pour une gêne maxillaire gauche, ce qui a permis de découvrir un foyer d'ostéonécrose spontanée maxillaire. Un deuxième foyer d'ostéonécrose, mandibulaire, a été diagnostiqué trois semaines plus tard. Considérant l'âge de la patiente et l'absence de troubles fonctionnels, seul un débridement chirurgical superficiel a été réalisé. Discussion : L'apparition d'ostéonécroses spontanées bimaxillaires quatre mois après une perfusion unique de zolédronate étant peu probable, la recherche approfondie des antécédents de la patiente a permis de retrouver une prise de trois bisphosphonates oraux pendant sept ans sans aucun examen et suivi bucco-dentaire. L'interrogatoire médical des patients âgés atteints de troubles cognitifs étant difficile, la recherche de tous les médecins prescripteurs est nécessaire. Conclusion : La prise en charge de l'ostéonécrose associée aux bisphosphonates étant complexe, le chirurgien-dentiste joue un rôle important dans la prévention de cette pathologie.Abstract -Osteonecrosis of the jaws: when past bisphosphonates therapy is unknown. Introduction: Older people have multiple morbidities and are polymedicated. The medicines are prescribed by several doctors who are unaware of each other, hence the increased risk of adverse effects. Observation: We report the case of an institutionalized 91-year-old female patient. Following a fall resulting in a vertebral compression fracture, she received an infusion of zoledronic acid. Four months after this treatment, she consulted for left maxillary discomfort. A voluminous bone sequestrum revealed a maxillary osteonecrosis area. Right mandibular osteonecrosis was diagnosed three weeks later. Considering the patient's age and the lack of functional impairment, surgical debridement was the only procedure performed. Discussion: The emergence of spontaneous maxillary osteonecrosis four months after a single infusion of bisphosphonate being implausible, we searched her medical records for the physicians who treated the patient before her institutionalization. We found three oral bisphosphonate medications during seven years. No oral examination was performed during this therapy. The diagnosis of jaw osteonecrosis should lead the oral surgeon to look for past bisphosphonate therapy among other possible etiologies of this pathology. Indeed, this treatment is often forgotten by patients suffering from cognitive impairment. Conclusion: As the management of bisphosphonate-related osteonecrosis of the jaws is difficult, the oral surgeon has an important role in the prevention of this pathology. Mots clés :bisphospho...
