Cross-sectional and evolutive studies on schistosomiasis mansoni were carried out before and after mass treatment in the endemic areas of Capitão Andrade and Padre Paraiso, state of Minas Gerais, Riachuelo, state of Sergipe, Alhandra, state of Paraiba, and Aliança, Alegre and Coroatá, lowland of the state of Maranhão, Brazil, in the last eighteen years. The studies included clinical and fecal examination by the Kato-Katz quantitative technique, skin test for Schistosoma mansoni infection, evaluation of man-water contact and other epidemiological investigations such as infection rate and dynamic of the snail population. Results showed: (1) Higher prevalence of S. mansoni infection, greater egg load elimination and higher and earlier morbidity of the chronic forms of the disease in the southeast areas of Capitão Andrade and Padre Paraiso; (2) The incidence of hepatosplenic form is higher in some family clusters, in whites and mulattos in all the endemic areas but develop earlier in the southeast; (3) The prevalence and morbidity of schistosomiasis are decreasing both in the mass treated northeast and in the untreated southeast areas; (4) The mass treatment reduces rapidly the prevalence of the infection and the morbidity of the disease but can not control it because of the frequent reinfections due to the intensity of man-water contact.
Os autores fazem um estudo comparativo sobre a morbidade da esquistossomose mansoni em 4 áreas de campo no Brasil, duas na região Sudeste (Capitão Andrade, município de Inhomi e Padre Paraíso, ambas no Estado de Minas Gerais) e duas outras na região nordeste (Riachuelo, no Estado de Sergipe e Alhandra, no Estado da Paraíba). A população total estudada nas 4 áreas foi de 4.870 pessoas, das quais 1.369 em Capitão Andrade (área 1), 850 em Riachuelo (área 2), 1.736 em Padre Paraíso (área 3) e 915 em alhandra (área 4). Na área 1, com uma população de 1.480 habitantes, foi tentado um estudo de toda a pupulação. Nas áreas 2, 3 e 4, devido ao grande número de habitantes, foi estudada uma amostra sistemática por conglomerado de aproximadamente 25% da população (grupamento familiar de uma em cada 4 residências). O estudo constou e uma avaliação das condições econômicas e sanitárias da população, do seu contacto com os focos locais de transmissão da esquistossomose, dos índices e intensidade da infecção pelo S. mansoni e das relações da carga parasitária com as diversas formas clínicas da doença em diferentes grupos etários. Paralelamente, foi feito um estudo dos hospedeiros intermediários de cada área e dos seus índices de infecção com cercárias de S. mansoni. Demonstrou-se uma prevalência de 60,8, 50,5, 63,1 e 46,6% de infecção ativa pelo S. mansoni para as áreas 1, 2, 3 e 4, com a mediana de eliminação de ovos de 207, 77,6, 391 e 211 respectivamente. Verificou-se, ainda, nas áreas da região Sudeste um aumento mais tardio dos parâmetros mencionados. Outras correlações com grupos etários, sexo, cor dos pacientes, bem como os índices de infecção dos planorbídeos com a forma clínica da doença foram feitas.
A comparative study on the evolutive morbidity of schistosomiasis mansoni in treated and non-treated patients in an endemic area was carried-out with evaluation of the treatment after one and six years. In the first year after treatment there was both an improvement in the clinical conditions and a dramatic reduction in the number of S. mansoni eggs eliminated by the treated patients in relation to the controls. However, after six years of observations, although the number of S. mansoni eggs eliminated by the treated patients was still less than in the control group, it was nearly the same number as before treatment. In both treated and non-treated groups there was an increase in the severity of the clinical forms of schistosomiasis, with a certain tendency of greater severity in the control group. Therefore the authors recommend that the specific treatment of the population in endemic area should be done at intervals of two to three years, and that the treatment should be associated to the environmental sanitation, water supply and the education of the population.
