(250 words)Objective: To evaluate the efficacy of psychological treatments for depression in pre-adolescent children, a disorder affecting 1-2% of children in this age-range.
Design:Systematic review of studies of psychological interventions to treat depressive disorder in preadolescent children (aged up to 12 years old). The primary outcome was level of depressive symptoms.Data Sources: Studies were found using Medline, PsycINFO, EMBASE and Web of Knowledge databases and selected on several criteria.Eligibility criteria for selecting studies: Only randomised controlled trials were included. Where individual studies covered a broader age range (usually including adolescents up to age 18 years), authors of those studies were contacted and requested to provide individual patient level data for those aged 12 years and younger.Results: 2822 abstracts were reviewed, and from these 124 full text articles were reviewed, yielding 7 studies for which we were able to access appropriate data for this review. 5 of these studies evaluated cognitive behaviour therapy (CBT). Combined results from these studies suggest that there is a lack of evidence that CBT is better than no treatment (Standard Mean Difference -0.342 (95% Confidence Interval -0.961, 0.278), although the number of participants included in the trials was relatively small. The evidence for efficacy of family therapy and psychodynamic therapy is even more limited.
Conclusion:The very limited number of participants in randomised controlled trials means that there is inconclusive evidence for the psychological treatment of depression in children aged 12 years and under. Given the prevalence and significant impact of this disorder, there is an urgent need to establish the effectiveness or otherwise of psychological intervention.Final accepted version -Published version is in European of Child and Adolescent Psychiatry, 2016. Introducción: En marzo de 2020 se decretó un confinamiento en España muy estricto, de al menos 6 semanas de duración, y para toda la población. Esto provocó grandes cambios en la vida de las personas en general, de niños y adolescentes y sus familias en particular. Objetivos: Estudiar la experiencia subjetiva del confinamiento por la pandemia por SARS-CoV-2 en España de los padres de niños y adolescentes, en población general y población clínica. Material y métodos: Se realizó una encuesta ad hoc y se aplicó un cuestionario de variables sociodemográficas y clínicas, así como el Cuestionario de Capacidades y Dificultades SDQ. La encuesta se compartió a través de redes sociales. Se envió también a padres de pacientes de salud mental en seguimiento en el centro de salud mental de zona. Resultados: Un total de 243 progenitores participaron en el estudio. Un 17,7% trabajaban como sanitarios o en residencias y el 79% de la muestra vivía en la Comunidad de Madrid. En general, la percepción de los padres sobre la experiencia de los hijos/as obtuvo puntuaciones positivas. Un total de 241 progenitores completaron el cuestionario SDQ. En las comparaciones por profesión, lugar de residencia o antecedentes de salud mental, estos 3 fueron factores de riesgo para obtener mayores puntuaciones en el SDQ. Conclusiones: Profesión de riesgo en los padres, lugar de residencia o seguimiento en salud mental pueden afectar más negativamente a los hijos, según perciben los padres. Será necesario realizar estudios a medio largo plazo para valorar el impacto real de la pandemia en las familias y sus integrantes.
A pesar del conocimiento que tenemos en la actualidad de la amplia variedad de sintomatología y fenotipos de pacientes que engloban los Trastornos del Espectro Autista, algunos de estos fenotipos siguen sin estar claramente descritos en las clasificaciones psicopatológicas. Se propone un caso clínico de una paciente adolescente que consulta por sintomatología depresiva, descubriéndose tras la evaluación y durante la evolución un Trastorno del Espectro Autista subyacente que había pasado desapercibido.
