There is a good correlation between IPF and RP mainly in thrombocytopenia by peripheral destruction. Determination of IPF is an easy technique in their implementation, standardized and reproducible, so it could be a useful screening technique in patients with thrombocytopenia.

The MLL gene, located at 11q23 band, is frequently disrupted by different chromosomal rearrangements that occur in a variety of hematological malignancies. MLL rearrangements are associated with distinct clinical features and a poor prognosis. The aim of this study was to analyze the incidence and the prognostic significance of MLL rearrangements in a consecutive series of adult AML patients and to determine the immunophenotypic features of these cases. The identification of abnormal immunophenotypes could be used for the detection of minimal residual disease (MRD). Ninety-three adult patients with de novo acute myeloid leukemia (AML) were analyzed by Southern blot in order to detect MLL rearrangements (MLL+). RT-PCR and genomic long-range PCR were performed to further characterize MLL partial tandem duplication (PTD) in those patients in whom conventional karyotype did not show 11q23 chromosomal translocations. All the patients were homogeneously immunophenotyped at diagnosis. MLL rearrangements were detected in 13 (14%) patients. Four patients (5%) showed 11q23 translocations by karyotypic conventional analysis. Nine patients (10%) revealed PTD of MLL and one patient showed a MLL cleavage pattern. The MLL+ patients usually expressed myeloid and monocytic antigens CD33 (12/13 cases), CD13 (9/13), CD117 (9/13), CD64 (11/13) and in some cases CD14 (4/11). HLA-DR was also positive in (12/13). Eight out of 13 cases expressed the stem cell marker CD34. Only one patient revealed lymphoid marker reactivity (CD7) and CD56 was expressed in 5/13 cases. All the MLL+ patients showed at least one aberrant phenotype at diagnosis, which allowed us to set out a simple panel for the MRD studies. Twenty-seven samples from eight patients in morphologic complete remission (CR) were analyzed using the aberrant immunologic combinations detected at diagnosis. Phenotypically abnormal cells were detected in all the patients who subsequently relapsed, whereas only one patient with MRD+ remained in CR. Owing to the high level of residual leukemic cells, the MLL+ patients showed a short CR duration and a poor survival. In conclusion, immunophenotyping may be a suitable approach to investigating MRD status in AML patients with PTD of the MLL gene.

RP measurement by flow cytometry, directly from whole-blood, is a useful screening test to differentiate between thrombocytopenia with high or low thrombopoietic activity. A RP percentage in excess of 11%, has a high sensitivity and good specificity for a diagnosis of thrombocytopenia with increased thrombopoietic activity.

Objective: to find out and interpret the experiences and meanings attributed to the active and healthy aging of a group of elderly people over 64 living in community in the city of Montevideo, Uruguay.Method: we chose the qualitative method with dialectical hermeneutic approach. Data collection occurred through in-depth interviews with 11 elderly people aged 64 or more in the city of Montevideo, Uruguay.Results: data analysis allowed us to construct the categories personal development, physical state changing, losses, social role changes, lack of recognition, recreation activities. Conclusion:we identified relevant aspects on the need to feel useful for society and family; they recognize that their functional capacity has decreased; they feel physical limitations. Feelings of economic, physical and labor losses showed up in the study. They point out that their mental health is their strength that keeps them alive and active. DESCRIPTORS:Aging. Elderly person. La recolección de datos se realizó a través de entrevistas en profundidad a 11 personas mayores de 64 años de la ciudad de Montevideo, Uruguay.Resultados: el análisis de los datos permitió construir las categorías de: desarrollo personal, alteración del estado físico, pérdidas, cambios del rol social, falta de reconocimiento, actividades de recreación.Conclusión: se identificaron aspectos relevantes sobre las necesidades de sentirse útiles para la sociedad y familia, reconocen que su capacidad funcional ha disminuido, sienten limitaciones físicas. Summary:We investigated whether daily oral washings with recombinant human granulocyte-macrophage colonystimulating factor (rhGM-CSF) solution improved grade III-IV oropharyngeal mucositis (OM) in patients with hematological malignancies undergoing stem cell transplantation. Forty-one consecutive patients (21 males and 20 females, median age (range) 44 (16-69) years) were prospectively randomized to perform daily mouth-washes with either a 400 g rhGM-CSF (Molgramostin, Schering-Plough) solution (group A, n ‫؍‬ 18) or with a saline solution (group B, n ‫؍‬ 23). Primary end-points were the intensity of OM, night rest quality and characteristics of food intake. Secondary end-points were need for and duration of parenteral nutrition, oral and intravenous analgesic requirements, incidence of viral or fungal oral infections and development of neutropenic fever. No differences were found between the placebo and rhGM-CSF-treated groups regarding overall duration of OM, maximum grade, reduction in at least one grade of OM (nine patients (56%) in group A vs 13 patients (68%) in group B), reduction of spontaneous or swallowing-induced pain, improvement in oral food intake, use of parenteral nutrition or use of systemic analgesics. In conclusion, mouth-washings with a 400 g of rhGM-CSF solution do not improve severe OM in hematological patients undergoing stem cell transplantation.

La investigación presenta tres universos culturales que sufren Hipertensión Arterial, Diabetes y Artrosis, describiendo las distintas dimensiones y la interpretación de los significados. Se utilizó el método etnográfico, utilizando la entrevista etnográfica y la observación participante y, en la interpretación, el interaccionismo simbólico y otros autores. Los aspectos éticos de la investigación con seres humanos fueron respetados. Surgen convergencias en las dimensiones en que son percibidas las tres enfermedades: la pérdida del bienestar y soporte social, miedo a la invalidez y muerte, percibiendo la familia y aceptando la cronicidad y, valorizando la medicina alternativa. El cuidado de enfermería es percibido por esta clientela con ambigüedad.
