Objective: The aim of the study was to identify the profile of clinical research related to orthopedic disorders worldwide. Methods: A survey of clinical research in orthopedics was performed in the ClinicalTrials.gov platform and Plataforma Brasil databases. Results: According to data from the ClinicalTrials.gov platform, 1.142 clinical trials related to orthopedic disorders were registered from January 2017 to July 2022. Brazil is in the 14th position in the international ranking, and the Universidade de Sao Paulo (USP) occupies the 6th position among the collaborators who performed the most research in this area. The importance of the foot and ankle area in clinical research related to orthopedic disorders is evident when it is the 7th most studied worldwide and the 2nd most studied in Brazil. Most clinical trials related to orthopedic disorders worldwide target the adult or older adult population and involve interventional studies. On the other hand, most of the clinical trials related to orthopedic disorders in Brazil target the pediatric or geriatric population and involve observational studies. Conclusion: The profile of clinical research related to orthopedic disorders worldwide showed that most clinical trials targeted the adult or older adult population and aimed to treat patients, not prevent diseases. Brazil targeted the pediatric or geriatric population focusing on characterizing populations and diseases. Clinical research related to orthopedic disorders performed worldwide and in Brazil depended on sponsorship and private institutions. Level of Evidence IV; Descriptive Observational Study.
Objective: The aim of the study was to identify: how to fund clinical research in orthopedics and traumatology? Methods: A survey of funding for clinical research was performed. Results: According to data from the PIVOT® platform, the funding is concentrated in the USA, UK, and Medical Societies. The main sponsor was the DOD (USA), followed by the SICOT (Belgium). According to data from the DIMENSIONS® platform, there was a reduction of projects financed. The main country was the USA, followed by Japan. Regarding the amounts, the largest funders were the USA and Belgium. FAPESP (Brazil) is in the 7th position among sponsors. According to data from the InCites® platform, the main countries were the USA, China, and Japan. Conclusion: The reduction of projects in the last two years may be related to the limitations imposed by the SARS-CoV-2 pandemic. Regarding the amounts, the largest funders were the USA and Belgium. This data confirms the importance of funding from the SICOT. The main sponsor was the DOD; this data may be related to injuries that occurred in the war. The fact that the NIDA (USA) is the third largest funder may be related to the increase in the consumption of opioids for pain management. The incidence of falls among the elderly may be associated with Japan being one of the countries most supporting this area. Brazil is in the 8th position, and CAPES, CNPq, and FAPESP are among the top 20 funders. Level of Evidence IV; Descriptive Observational Study.
Introdução: O acidente ofídico ou ofidismo é caracterizado pelo envenenamento decorrente da picada ou mordida de serpentes. Esses acidentes com seres humanos representam um grave problema de saúde pública para as regiões tropicais e subtropicais de países em desenvolvimento. Objetivo: Visto que as moléculas encontradas na composição dos venenos são excelentes ferramentas de pesquisa para descobrir novos medicamentos, principalmente no tratamento farmacológico eficaz contra a dor, este trabalho teve como objetivo discutir a importância das principais toxinas ofídicas avaliadas em pesquisas clínicas para a produção de medicamentos e soros antiofídicos e identificar na literatura potenciais compostos presentes no veneno de serpentes brasileiras para a criação de novos fármacos. Método: O trabalho utilizou o método de revisão integrativa da literatura. Resultados: Com base nos dados obtidos após a busca e seleção dos artigos, a análise dos resultados evidenciou, estatisticamente, que houve um significante aumento de publicações científicas em relação ao estudoe à exploração de toxinas ofídicas de serpentes brasileiras. Por outro lado, ao longo dos anos, poucos ensaios clínicos foram realizados com essas moléculas visando ao teste de seu potencial farmacológico, tal como testes de segurança e eficácia para permitir a implementação de novos fármacos no mercado. Com relação à Cochrane, base de Registro Central de Ensaios Controlados, desde 1990, foram encontrados dez estudos clínicos envolvendo as toxinas ofídicas de gêneros de serpentes brasileiras. Destes, apenas seis foram realizados no Brasil, e entre as toxinas ofídicas mais utilizadas nos ensaios clínicos estão as do veneno do gênero Bothrops e Crotalus. No que se refere a base de dados Clinicaltrials.Gov, foram encontrados quatro ensaios clínicos relacionados as toxinas ofídicas de gêneros de serpentes brasileiras, sendo que nenhum foi realizado no Brasil. No que se refere à base de dados SciELO, não foi encontrado nenhum estudo clínico sobre o tema. Já na base de dados PubMed, poucas publicações foram encontradas com os descritores utilizados pois muitas estavam associadas às variações e às manifestações clínicas em seres humanos com base na inoculação venenos de serpentes brasileiras. Entretanto, dos 14encontrados, seis artigos apresentaram possíveis aspectos inovadores para a aplicação das toxinas em indústrias farmacêuticas. Por fim, a busca na Biblioteca Virtual em Saúde exibiu artigos científicos com temáticas inovadoras que contribuem para os avanços biotecnológicos no campo da toxicologia com base na avaliação de diferentes toxinas ofídicas dos principais gêneros de serpentes encontradas no Brasil. Conclusão: Os venenos de serpentes brasileiras são uma rica fonte de moléculas bioativas contendo peptídeos, proteínas e enzimas com importantes atividades farmacológicas. É evidente que esses compostos possuem um grande potencial terapêutico, mas ainda longe de ser adequadamente explorado. Ao se analisarem os resultados encontrados em todas as bases de dados pesquisadas, nota-se maior prevalência de estudos acadêmicos de farmacologia básica focados na caracterização e(ou) exploração de moléculas ofídicas e poucos estudos clínicos Fase I, II ou III para o desenvolvimento de medicamentos ou produtos potencialmente passíveis de registro junto às agências regulatórias. Ainda que venenos de serpentes tenham sido foco de investigação em vários países, no Brasil foram encontradas poucas pesquisas clínicas com as toxinas ofídicas. Apesar disso, nota-se um recente interesse em toxicologia e avanços tecnológicos nesse campo de atuação. Uma possível solução para a falta de investimentos nessa área seria a colaboração internacional multidisciplinar entre centros de pesquisa e indústrias para a produção de antivenenos e novos medicamentos.Palavras-chave: Veneno, Serpentes, Brasil, Pesquisa biomédica, Estudo clínico, Indústria farmacêutica ABSTRACT Introduction The Ophidic Accident or Ophidism is characterized by poisoning due to the bite or bite of snakes. These accidents with humans represent a serious public health problem for the tropical and subtropical regions of developing countries. Objective: Since the molecules found in poisons are excellent research tools to discover new medicines, especially in effective pharmaceutical treatment, this work aimed to discussthe pharmaceutical importance of the main ophidic toxins evaluated in clinical research for drug production and antiophidic serums and to identify, in the literature, potential compounds present in the poison of Brazilian snakes for the creation of new drugs. Method: The work used the integrative literature review method. Results: Based on the data obtained after the search and selection of the articles, analysis of the results showed, statistically, that there was a significant increase in scientific publications in relation to the study and exploitation of ophidic toxins from Brazilian snakes. On the other hand, over the years, few clinical trials have been carried out with these molecules to test their pharmacological potential, such as safety and efficacy tests to allow the implementation of new drugs on the market. With regard to Cochrane, the basis of the Central Registry of Controlled Trials, since 1990, ten clinical studies were found involving snake toxins from Brazilian snakes, of which only six were carried out in Brazil, and among the snake toxins most used in clinical trials are those of the venom of the Genus Bothrops and Crotalus. About the Clinicaltrials.Gov database, four clinical trials related to ophidic toxins from Brazilian snakes were found, none of which were carried out in Brazil. Regarding the Scientific Electronic Library Online (SciELO) database, no clinical study on the subject was found. As for the United States National Library of Medicine (PubMed) database, few publications were found with the descriptors used, since many were associated with variations and clinical manifestations in humans from the inoculation of Brazilian snake venoms. However, of the 14 found, six articles presented possible innovative aspects for the application of toxins in pharmaceutical industries. Finally, the search in the Virtual Health Library displayed scientific articles with innovative themes that contribute to biotechnological advances in the field of toxicology based on the evaluation of different snake toxins from the main genera of snakes found in Brazil. Conclusion: Brazilian snake venoms are a rich source of bioactive moleculescontaining peptides, proteins, and enzymes with important pharmacological activities. It is evident that these compounds have a great therapeutic potential, but still far from being adequately explored. When analyzing the results found in all databases surveyed, there is a higher prevalence of academic studies of basic pharmacology focused on the characterization and/or exploration of snake molecules and few Phase I, II or III clinicalstudies for the development of drugs or products potentially subject to registration with regulatory agencies. Although snake venoms have been the focus of investigation in several countries, little clinical research was found with snake toxins in Brazil. Despite this, there is a recent interest in toxicology and technological advances in this field. Introdução: De acordo com dados do Instituto Brasileiro de Geografia e Estatística (IBGE), considera-se população pediátrica indivíduos na faixa etária de até 17 anos. Estima-se que a população pediátrica brasileira seja de 61,5 milhões, o que corresponde a 29% da população total do país, a qual foi estimada no ano de 2020 em cerca de 210 milhões de pessoas. De acordo com a Demografia Médica no Brasil, publicada em 2020, 10,1% dos médicos especialistas no Brasil são pediatras. Ao traçar o perfil de pacientes pediátricos para pesquisa clínica,encontramos uma escassez de estudos que contemplem essa população, impedindo, assim, a utilização de medicamentos seguros e eficazes em pediatria ou com formulações específicas baseadas em estudos clínicos e farmacoepidemiológicos adequados para esse público-alvo. Assim sendo, em suas diversas faixas etárias, crianças e adolescentes são expostos a medicamentos cujo benefício clínico só é conhecido em adultos. Objetivo: Com base nos desafios para o desenvolvimento de estudos clínicos pediátricos e na grande importância e necessidade depesquisas clínicas para este público-alvo, este estudo buscou identificar o perfil das pesquisas clínicas pediátricas no Brasil nos últimos três anos (2018–2021) e discutir seus desafios e oportunidades. Método: A presente pesquisa consistiu em um estudo observacional, transversal e descritivo, e considerou-se pesquisas clínicas e acesso expandido exclusivamente em população pediátrica. Realizou-se um levantamento na base de dados no Clinical Trials.gov® das pesquisas clínicas pediátricas no Brasil, incluindo programas de acesso expandido, registradasde 06 de janeiro de 2018 a 01 de junho de 2021. Para esse levantamento, a base de dados Clinical Trials.gov® foi acessada em 30 de julho de 2021, às 19 horas e 22 minutos. Utilizou-se as seguintes palavras-chave como filtro de busca: “Brazil, Child (birth–17) and Study Start (MM/DD/YYYY): from 06/01/2018 to 01/06/2021”. Os dados extraídos da base de dados Clinical Trials.gov® foram registrados em uma planilha com uso do programa Microsoft Excel® 2010. Após a checagem da consistência dos dados, realizou-se a análise descritiva. Resultados: Triageminicial reuniu 264 estudos relacionados a ensaios clínicos em pediatria no Brasil, porém apenas 107 destes enquadravam-se nos critérios de delineamento propostos. De acordo com os resultados entre os anos de 2018 e 2021, houve queda de mais de 34% no número total de estudos para pacientes pediátricos. Neste mesmo período, é possível afirmar que em mais de 97% dessas pesquisas não há diferenciação de gêneros, e a concentração da localização do centro de estudo foi na região Sudeste do Brasil, sendo a cidade de São Paulo responsável por abrigar 28% deles. Ainda, de acordo com dados obtidos, 41% desses estudos estão em fase de recrutamento e apenas 24% foram concluídos. A área de maior interesse das pesquisas pediátricas foi em Odontologia, enquanto a área de Doenças Raras não alcançou nem 1% dos temas estudados. No entanto, durante o período entre 2018 e 2021, houve uma mudança do tipo predominante de patrocínio desse tipo de pesquisa, passandoa ser o patrocínio privado o mais frequente. Conclusão: Em decorrência da queda significativa de estudos clínicos em pediatria no Brasil nos últimos anos, conforme apresentado nos resultados, os prescritores e pacientes ficarão sem alternativas de tratamentos terapêuticos precisos e seguros para a população pediátrica brasileira. Considerando que 43% dos estudos clínicos pediátricos em fase de recrutamento no Brasilse concentram no ano 2019, a falta de novas pesquisas clínicas em pediatria nos últimos anos pode sugerir um aumento do uso off label de medicamentos em pediatria por causa da ausência de pesquisas com comprovação científica suficientes nessa área terapêutica. Quase a totalidade dos estudos clínicos em pediatria no Brasil se realizou com a participação de todos os gêneros, o que demonstra que doenças específicas de cada sexo não fazem parte do escopo desses estudos pediátricos, assim como doenças raras que não estavam nos principaistemas pesquisados. Com relação à distribuição geográfica dos estudos clínicos em pediatria no Brasil, observou-se que mais de 60,7% dessas pesquisas clínicas foram conduzidas na região Sudeste, principalmente em São Paulo, seguida pela região Sul, com 27% da concentração dessas pesquisas. Esses resultados reafirmam dados do IBGE sobre a desigualdade regional que se reflete na concentração de investimentos, centros de pesquisas e investigadores nessas regiões do país. When profiling pediatric patients for clinical research, we found a shortage ofstudies that address this population, thus preventing the use of safe and effective drugs in pediatrics with specific formulations based on clinical and pharmacoepidemiological studies that are suitable for this target audience. Objective: Based on the challenges for the development of pediatric clinicalstudies and the great importance and need for clinical research for this target audience, this study sought to identify the profile of pediatric clinical research in Brazil in the past 3 years (2018–2021) and discuss its challenges and opportunities. Method: This is an observational, crosssectional, and descriptive study and considered clinical trials and expanded access exclusively to the pediatric population. A survey was carried out in the Clinical Trials.gov® database of pediatric clinical trials in Brazil, including expanded access programs, registered from January 6, 2018 to June 1, 2021. For this survey, the Clinical Trials database.gov® was accessed on July 30, 2021 at 7:22 p.m. The following keywords were used as a search filter: “Brazil, Child (birth–17) and Study Start (MM/DD/YYYY): from 01/06/2018 to 06/01/2021.” Data extracted from the Clinical Trials database.gov® were recorded in a spreadsheet using the Microsoft Excel® 2010 program. After checking the consistency of the data, adescriptive analysis was performed. Results: The research gathered 264 studies related to clinical trials in pediatrics in Brazil, but only 107 of the studies met the proposed design criteria. According to the results between the years 2018 and 2021, there was a decrease of more than 34% in the total number of studies on pediatric patients. In this same period, it is possible to affirm that more than 97% of the studies do not have gender differentiation, and the concentration of the study centers is located in the southeast region of Brazil, with the city of São Paulo having28% of them. In addition, according to the data obtained, 41% of the studies are still in the recruitment phase and only 24% have been completed. The area of greatest research interest is in dentistry, and research on rare diseases did not reach even 1% of the topics studied. However, during the period between 2018 and 2021, there was a shift in research sponsorship sectors, moving from public to private. Conclusion: As a result of the significant decrease in clinical studies in pediatrics in Brazil in recent years, as shown in the results, prescribers and patients will be left withno alternatives for accurate and safe therapeutic treatments for the Brazilian pediatric population. Considering that 43% of pediatric clinical studies in the “recruitment” phase in Brazil are concentrated in the year 2019, the lack of new clinical research in pediatrics in recent years may suggest an increase in the “off label” use of medicines in pediatrics due to the absence of research with sufficient scientific evidence in this therapeutic area. Almost all pediatric clinical studies in Brazil were carried out with the participation of all genders, which demonstratesthat specific diseases of each gender are not part of the scope of these pediatric studies, as well as rare diseases that were not included in the research topics. Regarding the geographical distribution of clinical studies in pediatrics in Brazil, it was observed that more than 60.7% of these clinical studies were conducted in the southeast region, mainly in São Paulo, followed by the south region, with 27% of the concentration of these studies. These results reaffirm IBGE data on regional inequality which is reflected in the concentration of investments, research centers,and researchers in these regions of the country. Introdução: Em dezembro de 2019, em Wuhan, na China, começaram a surgir casos de pneumonia derivados de um patógeno denominado como a doença de coronavírus 2019 (COVID-19). O principal alvo do SARS-CoV-2 é o trato respiratório superior e inferior. De acordo com a OMS, o continente Americano foi considerado o epicentro da doença, sendo que o Brasil chegou a ocupar o terceiro lugar na lista de países com maior número de casos. Portanto, identificar o perfil das pesquisas clínicas relacionadas ao COVID-19 no Brasil é de suma importância. Objetivo: Este estudo buscou identificar o perfil das pesquisas clínicas relacionadas ao COVID-19 no Brasil e discutir desafios envolvidos nesse processo. Método: Foi realizado um levantamento no banco de dados Plataforma Brasil sobre as pesquisas clínicas relacionadas a COVID-19 no Brasil e registradas nesta plataforma durante o período de 01 de janeiro de 2020 a 31 de março de 2021. Resultados e Discussão: No Brasil, as regiões que apresentam maior infraestrutura de pesquisa e assistência são as regiões Sul e Sudeste, que possuem a predominância das pesquisas clínicas no país, tanto relacionados com a COVID-19 quanto para outras áreas terapêuticas. São esses locais que apresentam o maior número de centros de pesquisa e também as melhores universidades. Além disso, o predomínio da participação de universidades públicas indica essas instituições como um polo de pesquisa no Brasil. Apesar da maior parte da pesquisa clínica no Brasil se concentrar nas regiões Sul e Sudeste, é interessante observar que a região Nordeste apareceu em segundo lugar em número de estudos envolvendo a COVID-19, com aproximadamente 100 estudos a mais do que a região Sul durante o período pesquisado. Essa diferença pode ser explicada pelo fato de que a região Nordeste foi mais afetada pelo novo coronavírus do que a região Sul, o que estimularia os cientistas da região a realizar mais pesquisas para auxiliar a população local. Os idosos fazem parte do grupo de maior risco de contrair a COVID-19 por serem mais frágeis devido à idade e por normalmente apresentarem várias comorbidades. No entanto, o baixo número de pesquisa clínicas relacionadas à essa população provavelmente se deve aos riscos apresentados por esse grupo, a quantidade de medicações concomitantes e por possíveis interações medicamentosas. Por outro lado, o oposto foi encontrado quando analisamos as pesquisas envolvendo a população pediátrica, onde, apesar desta população ser considerada igualmente vulnerável para pesquisas clínicas e poucos serem os casos de crianças e adolescentes que contraíam o novo coronavírus, o número de pesquisas nesta população superou o encontrado em idosos. Conclusão: De acordo com os dados encontrados, o perfil das pesquisas clínicas relacionadas ao COVID-19 no Brasil é caracterizado em sua maioria por estudos observacionais, unicêntricos, envolvendo população adulta e conduzidos em instituições públicas da região Sudeste do país. Apesar do sistema CEP/CONEP e da ANVISA terem adaptado muito bem seus processos para reduzir com excelência os prazos e priorizar os estudos do novo coronavírus, o país, a princípio, não foi escolhido pelas multinacionais para conduzir estudos intervencionistas internacionais, mesmo pertencendo a região considerada como o epicentro da doença. Durante esse período, foram constatadas ações positivas e grandes avanços no empenho das instâncias regulatórias para a celeridade das aprovações das pesquisas clínicas envolvendo COVID-19 e também pode-se destacar o potencial científico do Brasil, que, durante uma pandemia de um vírus desconhecido, desenvolveu 2.706 pesquisas relacionadas a COVID-19, além da continuidade da realização de outras pesquisas clínicas nas diferentes áreas terapêuticas. The main target of SARS-CoV-2 is the upper and lower respiratory tract. According to World Health Organization (WHO), the American continent was considered the epicenter of the disease, with Brazil occupying the third place in the list of countries with the highest number of cases. Therefore, identifying the profile of clinical research related to COVID-19 in Brazil has become important. Objective: This study sought to identify the profile of clinical research related to COVID-19 in Brazil and discuss the challenges involved in this process. Methods: Active research was performed in the database Plataforma Brasil for clinical research related to COVID-19 in Brazil and registered in this platform during the period January 01st 2020 until March 31st, 2021. Results and discussion: In Brazil, the regions with the greatest research infrastructure and assistance are the South and Southeast regions, that have the predominance of clinical research in the country, both related to COVID-19 and to other therapeutic areas. These are the places that have the largest number of clinical research sites and also the best universities. Moreover, the prevalence of participation by public universities indicates these institutions as a research hub in Brazil. Although most clinical research in Brazil concentrates in the South and Southeast regions, it is interesting to observe that the Northeast region appeared in second place in the number of studies involving COVID-19, with approximately 100 more studies than in the South region during the researched period. This difference can be explained by the fact that the Northeast region was more affected by the new coronavirus than the South region, which would encourage scientists in this region to perform more research to help the local population. The elderly is part of the group at highest risk to contract COVID-19 because they are more fragile due to the age and because they usually have several comorbidities. However, the low number of clinical research related to this population probably is due to the risks presented by them, the quantity of concomitant medications and by possible drug interactions. On the other hand, the opposite was found when we analyze the research involving the pediatric population, where, although this population is considered equally vulnerable for clinical research and there were few cases of children and teenagers who contracted the new coronavirus, the number of research in this population overcame the one found with the elderly. Conclusion: According to the data found, the profile of clinical research related to COVID-19 in Brazil is compound mostly by observational studies, single-center, involving adult population and conducted in public institution in the Southeast region of the country. Despite CEP/CONEP system and ANVISA had adapt their process very well to reduce with excellence the deadlines and prioritize the new coronavirus studies, the country, at first, was not chosen by the multinational to conduct international interventional studies, even belonging to the region considered as the epicenter of the disease. During this period, there were positive actions and great advances in the efforts of regulatory institutions to speed up the approval of clinical research involving COVID-19 and it can also stand out Brazil’s scientific potential which, during a pandemic with an unknown virus, developed 2706 research related to the COVID-19 in addition to continuing perform other clinical research in different therapeutic areas.
