Thus, the objective of this study is to undertake a systematic review of the literature on the hopelessness in the elderly people to analyze their prevalence and to demonstrate the problems that may be associated with this feeling in this population. Methods This systematic literature review was carried out through the analysis of scientific literature by five researchers about the hopelessness in elderly through the PubMed, LILACs, MEDLINE and SciELO databases during the first semester of 2017. The keywords used were "Elderly" AND "Beck scale" AND "Hopelessness". The inclusion criteria were original articles available in full, without financial costs, published between 2007 and 2017, which addressed the hopelessness in the elderly in Portuguese and English languages. Exclusion criteria were the background approach of hopelessness in elderly people, studies involving another age group beyond the elderly or that did not stratify their results according to the ages, qualitative studies and case reports.
We describe the original technique used for the treatment of a patient who presented with pain and bulging in the abdomen, who was diagnosed with Spigelian hernia (SH) using ultrasound. In this case, the hernia occurred in the anterolateral abdominal wall with herniation of the distal ileum and mesentery, in addition to a large right inguinal hernia. A mini-laparoscopic approach was proposed; due to Child-A hepatic cirrhosis, it was done by a hybrid technique, using a harmonic scalpel. The primary closure of the hernia defects was performed, followed by the placement of a polypropylene mesh in the preperitoneal space. The mesh was fixed. In this case, the inguinal hernia was homolateral to the SH. Following the surgery, the patient had no further complications, being discharged the day after the procedure.
Introduction:Given the aging population, it is necessary to develop strategies for the prevention and promotion of mental health in the elderly. Hope as the ability to trace and maintain paths can be used as a key construct for these strategies, promoting cognitive health if stimulated.Objective:Evaluate the behavior and cognitive development of elderly people through hope training as a clinical strategy for health promotion.Methods:A clinical experiment with a comparative and cross-sectional design. There were 2 data collection phases over a 6-month period. The sample comprised 51 participants, divided into groups having received either more or less than 6 months of stimulation. The instruments used were the Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS), and Expectations/Hope Scale About the Future and Life's Guidance. The Wilcoxon signed-ranks test (t test) was used to analyze the data.Results:Hope in addition to cognitive stimulation shows more significant results in elderly people at initial stimulation (0.18), mainly at the permanence and troubleshooting on the paths concerned; this corresponds with the results obtained for hope (0.18). Depressive symptomatology showed significant differences among the 2 groups (0.39 and 0.02). In the between-groups comparisons, only GDS showed significance (0.46).Hope training has an impact on the remission of depressive symptoms and, despite increased hope levels, training was effective for the reduction of depression symptoms for up to 6 months, thereafter only playing a maintenance role. Broadly, hope has been shown to promote elderly people's mental health.

Instituição: FACULDADE DE CIÊNCIAS MÉDICAS -UNIVERSIDADE DE PERNAMBUCOApresentação do caso: Paciente do sexo feminino, 20 anos, com queixa de aumento de volume abdominal à esquerda, há 3 anos, sem sintomas associados. Ao exame físico foi palpada massa indolor à esquerda. Ultrassonografia evidenciou esplenomegalia por presença de cisto esplênico hipoecogênico, de conteúdo espesso, sem vascularização. Formação cística simples esplênica foi caracterizada pela TC de abdome em 21 x 14 x 18 centímetros, sendo levantada a hipótese de cisto esplênico gigante benigno. Optou-se, então, pela realização de esplenectomia parcial videolaparoscópica. A cirurgia foi realizada com paciente em decúbito lateral direito. A confecção do pneumoperitônio deu-se pela técnica aberta, introduzindo trocater de 10 mm em portal umbilical. Em seguida, foi inserido trocater de 5 mm em hipocôndrio direito e outro, de 3 mm, na região paramediana direita. Observou-se cisto em íntima comunicação com o baço e pequena aderência a lobo hepático esquerdo. O cisto foi perfurado em seu ápice, sendo utilizado trocater de 5 mm para aspiração completa de seu conteúdo, totalizando 1800 ml. A ressecção e destelhamento de grande parte da parede do cisto foram realizados e pequeno fragmento de tecido esplênico foi também removido. A cirurgia transcorreu sem complicações, com tempo total de 130 minutos. A paciente teve alta após três dias em boas condições clínicas. O resultado do histopatológico demonstrou tratar-se de pseudocisto esplênico de conteúdo hemorrágico. Discussão: Lesões císticas do baço são pouco frequentes e em geral, assintomáticas, sendo por vezes diagnóstico incidental de exames de imagem. Os cistos esplênicos classificam-se em verdadeiros (capsulados, parasitários ou não parasitários) e pseudocistos, sem cápsula. O tratamento de cistos esplênicos com diâmetro maior que 05 centímetros é cirúrgico (esplenectomia parcial ou total, cistectomia total, marsupialização ou decapsulação) devido ao melhor controle em longo prazo, comparado a abordagens conservadoras. Visto que o baço desempenha papel importante na função imunológica, a esplenectomia parcial, que preserva mais que 25% do órgão, tem maior aplicação, em especial pela técnica laparoscópica como método minimamente invasivo. Comentários Finais: A laparoscopia mostrou-se segura e eficaz para a realização deste procedimento, diminuindo risco de recorrência do cisto, com vantagens de melhor visualização, menor trauma, menor tempo cirúrgico e preservação de tecido esplênico.
