As glândulas adrenais ou suprarrenais estão situadas nos polos superiores dos rins e pesam certa de 6 a 11 gramas. São divididas em duas partes: o córtex e a medula.Por possuírem seu próprio sistema de suporte sanguíneo e serem glândulas muito vascularizadas (formam o plexo subcapsular) podem ser acometidas mais facilmente por metástases, infecções ou doenças autoimunes. No caso de infecções como as causadas pela histoplasmose, essas massas fúngicas simulam massas neoplásicas.O quadro clínico causado por essas infecções geralmente se inicia com perda de peso (sem intenção), fadiga, desânimo, hiperpigmentação da pele, hipotensão ortostática, hipoglicemia, náuseas e vômitos. O diagnóstico se faz por meio de exames de imagem, nas quais é possível observar massas fúngicas nas adrenais, sendo confirmado através da tomografia computadorizada. O tratamento é realizado com o uso de antifúngicos, especialmente itraconazol.
