As glândulas adrenais ou suprarrenais estão situadas nos polos superiores dos rins e pesam certa de 6 a 11 gramas. São divididas em duas partes: o córtex e a medula.Por possuírem seu próprio sistema de suporte sanguíneo e serem glândulas muito vascularizadas (formam o plexo subcapsular) podem ser acometidas mais facilmente por metástases, infecções ou doenças autoimunes. No caso de infecções como as causadas pela histoplasmose, essas massas fúngicas simulam massas neoplásicas.O quadro clínico causado por essas infecções geralmente se inicia com perda de peso (sem intenção), fadiga, desânimo, hiperpigmentação da pele, hipotensão ortostática, hipoglicemia, náuseas e vômitos. O diagnóstico se faz por meio de exames de imagem, nas quais é possível observar massas fúngicas nas adrenais, sendo confirmado através da tomografia computadorizada. O tratamento é realizado com o uso de antifúngicos, especialmente itraconazol.
Osteoporose é uma doença silenciosa definida como osteo-metabólica sistêmica progressiva, caracterizada pela diminuição da massa óssea e deterioração da microarquitetura do osso, gerando fragilidade e aumentando o risco de fraturas. As principais etiologias são as que atingem mulheres após a menopausa e idosos. O diagnóstico é clínico e por imagem, diante de uma redução da densidade mineral óssea ou quando há fraturas secundárias às fragilidades ósseas. É imprescindível, uma excelente anamnese, com a detecção dos fatores de risco e outras comorbidades que possam impactar na fisiopatologia da doença. O tratamento é baseado nas modificações do estilo de vida, as intervenções nutricionais e os tratamentos farmacológicos. Perante o exposto, o diagnóstico e a conduta tornam-se desafiadores devido suas possíveis repercussões clínicas, as quais guiam uma elaboração de protocolo de atendimento padronizado para a atenção primária à saúde, elaborado com base na literatura sobre conduta e manejo na investigação da osteoporose.
A Úlcera de Marjolin é uma condição de pele rara e usualmente agressiva, na qual uma área do corpo previamente danificada ou afetada por uma inflamação crônica sofre diferenciação, resultando em neoplasia. O tipo histológico mais comum é o carcinoma espinocelular (CEC), e pode resultar em prognósticos desfavoráveis caso não diagnosticado precocemente. O diagnóstico é realizado através de biópsia do local e o tratamento é cirúrgico. Portanto, é de fundamental importância que os profissionais da saúde reconheçam alterações em lesões crônicas que sugiram neoplasias, a fim de oferecer uma melhor qualidade de vida ao paciente. O presente trabalho trata-se de um relato de caso de uma paciente feminina de 73 anos, diagnosticada com Úlcera de Marjolin após ferimento cortante em membro inferior esquerdo.Palavras-chave: Carcinoma espinocelular; Neoplasia; Tumor.
