A Hipomineralização Molar Incisivo (HMI) é o defeito qualitativo do esmalte que altera a maturação e calcificação de molares e incisivos permanentes. Os defeitos ocorrem de forma muito variada, o esmalte apresenta-se opaco com sua parte mineral, microdureza, módulo de elasticidade reduzida e densidade de hidroxiapatita menor. Este estudo tem como objetivo conhecer as principais características clínicas, epidemiológicas, e o diagnóstico diferencial da HMI assim como identificar os diversos protocolos de tratamentos da HMI e suas particularidades, facilitando o entendimento e a conduta do cirurgião dentista. Este estudo se enquadra no delineamento de pesquisa, revisão bibliográfica descritiva com análise integrativa e qualitativa-exploratória com base nas principais plataformas de pesquisa, buscando os termos “Molar Incisor Hypomineralisation”, “Etiology”, ”Histology”,“Prevalence”;“Diagnosis”; "Differential diagnosis”, “Treatment”. Foram incluídos artigos no intervalo de tempo de 2011 a 2021 na língua inglesa, portuguesa e espanhola e a busca resultou em 39 artigos. Portanto, pode-se concluir que pacientes com HMI apresentem hipersensibilidade espontânea devido à exposição dos túbulos dentinários, o esmalte possui superfície rugosa favorecendo o acúmulo de placa, dentes afetados tornam se difícil anestesiar devido o estresse crônico que sofre a polpa, restaurações atípicas são comuns nesses pacientes assim como fraturas coronárias. A prevalência varia uma vez que exames clínicos não são padronizados e o diagnósticos podem ser errôneos. A causa da HMI é imprecisa, sendo considerados multifatoriais e relacionados com genética, ambiente, fatores sistêmicos, uso de medicamentos durante o período de odontogênese. O diagnóstico precoce e um correto plano de prevenção e tratamento evitam os agravos da doença.
A Hipersensibilidade dentinária (HD) pode ser definida como um processo patológico que tem etiologia multifatorial e depende de estímulos em túbulos dentinários expostos supragengivais, subgengivais e abaixo de defeitos estruturais e trincas de esmalte que podem desencadear uma dor de sintomatologia aguda e de curta duração. De acordo com a teoria da Hidrodinâmica, a HD é o resultado do fluxo de fluido nos túbulos da dentina. Comumente, está associada as Lesões cervicais não cariosas e a fatores como tensão, fricção e biocorrosão, que podem atuar isoladamente ou em conjunto. O presente trabalho revisou a literatura em relação aos tratamentos para a HD em Lesões cervicais não cariosas. Foram utilizadas as palavras-chave e seus correspondentes na língua inglesa, sendo possível encontrar na literatura científica variadas técnicas de recursos terapêuticos para reduzir os sintomas da HD, contudo, a aplicação destas deve ser iniciada a partir de um minucioso diagnóstico clínico e diferencial associado a um exame físico extra-oral, clínico intra-oral e exame radiográfico detalhado, diferenciando a HD de outras patologias. O objetivo dos tratamentos são identificar e controlar os fatores causais, o material ideal para tratamento de hipersensibilidade dentinária deve ser biocompatível, de fácil aplicação, ter efeito duradouro, permanente e de ação rápida, não ser um irritante pulpar e não alterar a cor da estrutura dental. Uma combinação de instrução de higiene oral, uso de dessensibilizantes caseiros e tratamentos realizados em consultório odontológico, incluindo dessensibilizantes de uso profissional e tratamentos restauradores, são capazes de diminuir e eliminar a hipersensibilidade dentinária.
O objetivo do presente trabalho foi avaliar a percepção dos cirurgiões-dentistas (CDs) da rede particular de Patos de Minas sobre os tratamentos realizados diante das lesões cervicais não cariosas (LCNCs). Em virtude do aumento significativo das LCNCs as distintas condutas terapêuticas, identificar os parâmetros mais utilizados no diagnóstico e no tratamento desta condição, tornou-se necessário. O trabalho é de natureza aplicada, abordagem quantitativa, com objetivo exploratório e modalidade de campo, com 62 CDs, que responderam ao questionário enviado obedecendo-se as normativas vigentes. Após a interpretação dos dados, os resultados foram tabulados e analisados de forma descritiva. Este estudo verificou que os CDs consideram a etiologia multifatorial das LCNCs, bem como a necessidade de se realizar uma avaliação oclusal. Além disso o trauma mecânico de escovação pode ter ação iniciadora e aceleradora, sendo fundamental que as decisões terapêuticas estejam embasadas por parâmetros como a profundidade da lesão e hipersensibilidade dentinária.
Apesar da evolução da odontologia restauradora, a adesão da resina na superfície dentinária ainda apresenta alguns obstáculos. Dentre esses obstáculos, encontram-se as enzimas metaloproteinases que hidrolisam os filamentos colágenos desprotegidos e comprometem as restaurações adesivas. A clorexidina (CHX) é um antimicrobiano amplamente discutido nos dias atuais para inibir ou retardar a ação destas enzimas. Diante disto, o objetivo deste trabalho foi avaliar, por meio da literatura, o mecanismo de ação da clorexidina sobre as enzimas metaloproteinases (MMPs) e sua repercussão clínica na adesividade das restaurações em resinas compostas. Essa pesquisa foi realizada na forma de revisão integrativa da literatura, tendo natureza básica. Para este fim, foram utilizadas as bases de dados Pubmed e Lilacs, com os descritores “chlorhexidine”, “metalloproteinase”, “hybrid layer”, “clorexidina”, “metaloproteinases” e “camada híbrida”, realizando o cruzamento aleatório e empregando o operador booleano AND. Perante o que foi avaliado, a clorexidina pode ser considerada benéfica para seu uso clínico pois consegue quelar íons fundamentais para a constância e função das MMPs, além de alterar a estrutura dimensional das mesmas. Entretanto, ainda existe a necessidade de mais estudos para avaliar os mecanismos de ação da CHX, além de estudos extensos e in vivo quanto ao seu uso.
Atualmente, um dos grandes desafios para a odontologia adesiva é manter conservada a integridade da camada híbrida em restaurações. É sabido que o fator dificultador está relacionado à ação das matrizes de metaloproteinase (MMP), que degradam os filamentos colágenos desprotegidos. É compreendido que a clorexidina (CHX) se comporta de forma inibitória sobre essa enzima, alterando sua estrutura e quelando os íons metálicos essenciais para sua ativação, o que quebraria essa sequência de degradação. Um dos objetivos deste trabalho foi descrever, com base na literatura, a influência do uso da CHX prévia a aplicação do sistema adesivo perante a conservação da camada hibrida e consequentemente melhora da adesão das resinas compostas. Outro propósito é verificar, entre os sistemas adesivos, se algum elucida melhores resultados após a aplicação da CHX e entender se, em longo prazo, essa inibição se mantém. Essa pesquisa foi realizada na forma de revisão integrativa de literatura, possuindo natureza básica pura. Para tal, foi realizada uma busca pela base de dados “PubMed-NCBI” com os termos de pesquisa: ((clorexidina) and (inibidores de matriz de metaloproteinase)) or (mmp) and (adesivos) considerando publicações dos últimos 10 anos. Perante aos dados coletados fica claro que este ainda é um assunto que demanda maiores e mais longas provas científicas devido a heterogenicidade dos resultados entre autores. Entretanto, nenhum estudo demonstrou que o uso do inibidor seria prejudicial a longevidade da restauração.
