Tooth injuries are one of the main emergencies in pediatric dentistry, being a stressful situation for the child and family. 1 In the interaction with the environment and while learning to crawl and walk, trauma can be more frequent due to the child's characteristics: curiosity, restlessness, motor immaturity and lack of protection. Falls from height and hitting objects while playing are the most common causes of traumatic injuries in children aged 1-to 3-years-old. 2 The maxillary central primary incisors are the most affected teeth. 1,3 Root fractures are rare traumatic injuries (1-4%), affecting dentin, cementum, periodontal ligament and the pulp. 2 They follow vertical, oblique or horizontal orientations. The diagnosis is performed through clinical and radiographic examinations. 4 Mobility and pain on palpation if there is displacement of the coronal fragment are frequent. 5,6 The treatment of root fractures in primary teeth will depend on the location and direction. 3,5 For horizontal fractures in the apical and middle thirds, when promptly attended, fragments should be repositioned by digital pressure. However, if the patient presents late, a blood clot is usually present, which makes this maneuver more difficult. 7,8 Apical third fractures usually recover without treatment, but middle third fractures or when there is excessive mobility of the involved tooth, rigid splinting is necessary for a period of 90-120 days. 3,5,8 Some late complications are related to pulp canal obliteration, whether or not accompanied by pulp necrosis, with or without apical resorption, and it may even affect the developing tooth germ's formation. 5

Além disto, estudos têmconcluído que a vitamina D não parece estar entre as variáveis potenciaispara reabsorção radicular. Por outro lado, pesquisas também têmsido conduzidas com o objetivo de avaliar sua influência no desenvolvimentodentário e do complexo craniofacial, uma vez que cálcioe fosfato são fundamentais para estes processos. Contudo, embora aprodução de pesquisas tenha aumentado consideravelmente, aindahá poucos trabalhos no campo da Ortodontia que esclareçam a realimplicação da deficiência ou excesso de vitamina D nas respostas teciduaisrelacionadas ao movimento dentário. ABSTRACTVitamin D is an hormone with receptors in a great variety of organs andtissues, acting to regulate calcium and phosphate levels in the body.Adequate levels of vitamin D are very important for the health of boneand teeth in both children and adults. However, nowadays, vitaminD deficiency has been verified with high prevalence. This deficiencyresults in abnormalities in bone mineralization, because of the reductionof osteoblasts function and disturbance in osteoclasts formationand recruitment. This influence in bone remodeling has stimulatedthe search for better understanding the influence and participation ofvitamin D in orthodontically induced dental movement. It has beensuggested that its local application promotes an increase in the rate ofrecruitment and activation of osteoclasts on the pressure side of theperiodontal ligament, which results in increased alveolar bone resorption,and an increase at the number of osteoblasts on the opposite surfaceof the alveolar bone, which indicates a greater bone formation.In addition, studies have shown that vitamin D does not appear to beamong the potential variables for root resorption. On the other hand,researches have also been conducted with the aim of evaluating itsinfluence on dental and facial development, since calcium and phosphateare essential for these processes. However, although the number of published researches has increased considerably, thereare still few studies involving orthodontics that clarify the realimplication of vitamin D deficiency or excess in tissue responsesrelated to tooth movement.
Introdução: A epidermólise bolhosa (EB) abrange um grupo de desordensbolhosas hereditárias de incidência rara na população mundial quese caracteriza pela extrema friabilidade do tecido cutâneo e mucoso. Indivíduoscom o subtipo distrófico recessivo, possuem manifestações maisgraves da patologia decorrentes do processo constante de ulceração ecura, com formação excessiva de cicatrizes, resultando em dor, microstomia,anquiloglossia, despapilação lingual, alto índice de cárie e gengivite.Além das limitações anatômicas, fatores comportamentais, sociais epsicológicos, geralmente, inviabilizam o tratamento odontológico ambulatorial.Portanto, a abordagem odontológica do paciente sob anestesiageral torna-se uma alternativa resolutiva. Objetivo: Relatar o caso de umapaciente infantil com diagnóstico de epidermólise bolhosa distrófica recessiva(EBDR), descrevendo a abordagem odontológica realizada sobanestesia geral no centro cirúrgico e o uso da fotobiomodulação a laserde baixa potência como coadjuvante na terapêutica das úlceras bucais.Descrição do caso: Paciente, sexo feminino, com 11 anos, diagnosticadacom EBDR. Ao exame clínico, observou-se microstomia, anquiloglossia,gengivite e cáries extensas em diversas unidades dentárias. Foi realizadaa adequação do meio bucal sob anestesia geral e no pós-operatório a laserterapia, de modo a proporcionar analgesia, melhoria na mastigação ena qualidade de vida. Conclusão: A abordagem odontológica em centrocirúrgico sob anestesia geral nos indivíduos com EB é uma modalidadeeficaz e segura. Além disso, a terapia com fotobiomodulação a laser promoveuanalgesia e aceleração da cicatrização das úlceras bucais. Individuals with recessivedystrophic subtype have the most severe manifestations of the diseasepresenting constant process of ulceration and healing, excessive scarring,resulting in pain, microstomy, ankyloglossia, lingual depapilation, high indexof caries and gingivitis. In addition to anatomical limitations, behavioral,social and psychological, generally, make outpatient dental treatmentunfeasible. Therefore, the patient’s dental approach under general anesthesiabecomes a resolutive alternative. Objective: Reporting a case of achild diagnosed with recessive dystrophic epidermolysis bullosa (RDEB),describing the dental approach performed under general anesthesia inthe operating room and the use of low-power laser photobiomodulationas an adjunct in the treatment of oral ulcers. Case description: Patient, female,11 years old, diagnosed with RDEB. On clinical examination, microstomy,ankyloglossia, gingivitis and extensive caries were observed in severaldental units. The adequacy of the oral environment was performedunder general anesthesia and in the postoperative period, low-level lasertherapy was performed. Providing analgesia, improvement in chewingand quality of life. Conclusion: The dental approach in a surgical centerunder general anesthesia in individuals with EB is an effective and safemodality. In addition, low-level laser therapy promoted analgesia and acceleratedhealing of oral ulcers.
Introdução: a epidermólise bolhosa compreende um grupo de doenças genéticas e raras da pele, onde há uma fragilidade estrutural da pele e das mucosas. A gravidade das manifestações clínicas varia de acordo com o subtipo envolvido, no qual a citada enfermidade distrófica recessiva é a mais severa. Há várias implicações sistêmicas, como a anemia, a desidratação e a desnutrição. O envolvimento bucal acompanha e torna mais severo o comprometimento sistêmico. As alterações em tecidos moles e duros dessa região variam consideravelmente. São relatados atrasos no crescimento e desenvolvimento dentoesquelético. Objetivo: considerando as possíveis alterações de desenvolvimento, este estudo tem o objetivo de mensurar a idade dentária em pacientes com epidermólise bolhosa distrófica recessiva, verificando a acurácia de dois métodos em relação à idade cronológica. Metodologia: foi realizado um estudo descritivo analítico transversal do tipo série de casos, que avaliou a idade dentária de 10 pacientes pelos métodos de Demirjian e Willems. Resultados: não houve divergência significativa entre as idades dentária e cronológica, com superestimação comum aos métodos utilizados. Conclusão: sugere-se que o tempo de maturação dentária pode ser um parâmetro estável frente a fatores adversos ao desenvolvimento, que são comuns a esse grupo. São necessários estudos que avaliem também outras variáveis para melhor compreender a relação da epidermólise bolhosa com o desenvolvimento dentoesquelético.
