Com o advento da Implantodontia moderna, tem sido cada vez maiora utilização de enxertos ósseos frente à perda fisiológica, traumáticaou patológica a que os ossos maxilares são submetidos. Objetivo: Opresente trabalho visa, por meio de um relato de caso clínico, abordara possibilidade de associação de enxertos ósseos na reconstrução dedefeito ósseo maxilar para instalação de implantes dentários. Descriçãodo Caso: Paciente M.A.G, gênero feminino, faioderma, compareceuao consultório particular encaminhada pela clínica de endodontia,para exodontia da unidade 22 devido à fratura radicular. Aoexame clínico, observou-se a presença de fístula ativa associada a estaunidade, e através de exame de imagem verificou-se área hipodensacompatível com extensa lesão periapical. O plano de tratamento incluiuexodontia da unidade fraturada, enucleação da lesão associada,reconstrução do defeito ósseo com enxerto autógeno e liofilizado bovinoe posterior reabilitação com implante dentário e prótese sobreimplante. O caso encontra-se com 3 anos de acompanhamento, comresultado estético e funcional satisfatório. Conclusão: Diferentes tiposde biomateriais agregados apresentam-se como uma técnica alternativaeficaz e de menor morbidade, possibilitando o reparo de defeitoscríticos em osso maxilar para posterior reabilitação.ABSTRACTWith the advent of modern Implantology, there has been increasingthe use of bone grafts by the physiological, traumatic or pathologicalloss to which the maxillary bones are submitted. Objective: This studyaims, through a case report, to describe the possibility of associationof bone grafts in the reconstruction of maxillary bone defect for dentalimplants. Case Description: The patient M.A.G, female, faioderm,attended the private practice sent by the endodontic clinic for unit22 extraction due to dental root fracture. At clinical examination wasobserved the presence of active fistula associated with the unit, andthrough image examination was verified hypodense area, compatiblewith extensive periapical lesion. The treatment plan included fractureunit extraction, enucleation of the associated lesion, reconstruction ofthe bone defect with autogenous graft and lyophilized bovine bone,and subsequent rehabilitation with dental implants and prostheses.The case has 3 years of follow-up with satisfactory aesthetic and functionalresults. Conclusion: Different types of aggregated biomaterialsare presented as an effective alternative technique, of lower morbidity,allowing the repair of critical defects in maxillary bone for later rehabilitation.
Introdução: A epidermólise bolhosa (EB) abrange um grupo de desordensbolhosas hereditárias de incidência rara na população mundial quese caracteriza pela extrema friabilidade do tecido cutâneo e mucoso. Indivíduoscom o subtipo distrófico recessivo, possuem manifestações maisgraves da patologia decorrentes do processo constante de ulceração ecura, com formação excessiva de cicatrizes, resultando em dor, microstomia,anquiloglossia, despapilação lingual, alto índice de cárie e gengivite.Além das limitações anatômicas, fatores comportamentais, sociais epsicológicos, geralmente, inviabilizam o tratamento odontológico ambulatorial.Portanto, a abordagem odontológica do paciente sob anestesiageral torna-se uma alternativa resolutiva. Objetivo: Relatar o caso de umapaciente infantil com diagnóstico de epidermólise bolhosa distrófica recessiva(EBDR), descrevendo a abordagem odontológica realizada sobanestesia geral no centro cirúrgico e o uso da fotobiomodulação a laserde baixa potência como coadjuvante na terapêutica das úlceras bucais.Descrição do caso: Paciente, sexo feminino, com 11 anos, diagnosticadacom EBDR. Ao exame clínico, observou-se microstomia, anquiloglossia,gengivite e cáries extensas em diversas unidades dentárias. Foi realizadaa adequação do meio bucal sob anestesia geral e no pós-operatório a laserterapia, de modo a proporcionar analgesia, melhoria na mastigação ena qualidade de vida. Conclusão: A abordagem odontológica em centrocirúrgico sob anestesia geral nos indivíduos com EB é uma modalidadeeficaz e segura. Além disso, a terapia com fotobiomodulação a laser promoveuanalgesia e aceleração da cicatrização das úlceras bucais. Individuals with recessivedystrophic subtype have the most severe manifestations of the diseasepresenting constant process of ulceration and healing, excessive scarring,resulting in pain, microstomy, ankyloglossia, lingual depapilation, high indexof caries and gingivitis. In addition to anatomical limitations, behavioral,social and psychological, generally, make outpatient dental treatmentunfeasible. Therefore, the patient’s dental approach under general anesthesiabecomes a resolutive alternative. Objective: Reporting a case of achild diagnosed with recessive dystrophic epidermolysis bullosa (RDEB),describing the dental approach performed under general anesthesia inthe operating room and the use of low-power laser photobiomodulationas an adjunct in the treatment of oral ulcers. Case description: Patient, female,11 years old, diagnosed with RDEB. On clinical examination, microstomy,ankyloglossia, gingivitis and extensive caries were observed in severaldental units. The adequacy of the oral environment was performedunder general anesthesia and in the postoperative period, low-level lasertherapy was performed. Providing analgesia, improvement in chewingand quality of life. Conclusion: The dental approach in a surgical centerunder general anesthesia in individuals with EB is an effective and safemodality. In addition, low-level laser therapy promoted analgesia and acceleratedhealing of oral ulcers.
