Objective: To estimate the prevalence of mechanical restraint in the hospital environment and the factors associated with its performance. Method: A cross-sectional, observational study with patients from a public hospital from the medical clinic, surgical clinic and intensive care unit evaluated by descriptive, univariate and multivariate analyses. Results: One hundred eleven (111) patients participated in the study. The prevalence of mechanical restraint was 51.4%; bilateral rails on the bed were used in 100% of the restraints, and bilateral wrist restraints were also observed in 29.8%. The most common justifications were the risk of falls (100.0%) and the risk of non-scheduled removal of invasive devices (57.9%). The restrained patients differ significantly from those not restrained by the following associated factors: male gender; age; stroke diagnosis; the hospitalization unit; ambulation capacity; the use of sedative medication; and the use of invasive devices. Conclusion: This study estimated a high mechanical restraint prevalence in the hospital environment and determined factors associated with the risk of a patient being restrained. A medical restraint evaluation team is recommended for an in-depth analysis of indication and therapy.
Objective To estimate the prevalence of mechanical restraint and factors associated with its practice in elderly in Home Care. Methods This was a cross-sectional study with 162 elderly randomly assigned to a home care program in Rio de Janeiro, from March 2018 to July 2018. Used as a technique for data collection and direct observation and structured interview of elderly clinical data. Data were analyzed descriptively and inferentially. Results There was a 13% prevalence of mechanical restraint in elderly in home care. The most frequent restraints were the use of bandage, tissues and sheets in the arms/legs and chests of the elderly, and the justification for their use were control of aggressive behavior (28.6%), prevention of falls (19%) and protection (19%). Of the total elderly participants, 42.9% remained contained for more than 24 hours, and in 85.7% of the cases, the individuals were confined to a room. Conclusion It is necessary to expand the training of formal and informal caregivers, recommending the rehabilitation of care practices that preserve the elderly's autonomy, giving them dignity, respecting gerontological and home care principles.
Objective: to identify the use of mechanical restraint in long-stay institutions for the elderly. Method: this is a bibliographical study, type integrative review, in articles published from 2005 to 2015, in the Databases MEDLINE, LILACS, CINAHL and SCOPUS. The results are presented in figures. The results were discussed comparing the studies and conclusions with the literature. Results: 15 articles were published and the continent with the highest production was Europe, followed by North America and Asia. It was inferred that the most used forms were the containment with grids in the bed and the containment by the trunk in wheelchair, in older people, with high dependence and immobility, and the reason for the restraint, was mostly to prevent the risk of falls and institutional routine. Conclusion: it has been demonstrated that there is a high prevalence of mechanical restraint in long-stay institutions for the elderly, with high variability, and efforts have been made to avoid adverse events and improve the quality of care. The importance of studying the theme in the country and in the Latin American reality is reinforced.
Objective: To verify the association of the practice of mechanical restraint with pressure injury (PI) in hospitalized patients. Method: A cross-sectional study of 111 patients from medical, surgical and intensive care units at a public hospital in the state of Rio de Janeiro, Brazil. To analyze the association between variables, the [odds ratio (OR)] was adopted. Results: It were found 57 patients in mechanical restraint (51.4%). In the group of patients contained, the occurrence of PI was estimated in 43.9% and in the group of patients not contained in 5.6%. The chance of the contained patient to present PI was 13 times higher than in patients not contained. The location of the injury was more frequently in the sacral region, classified as stage 2 (21.1%) and stage 3 (12.3%), followed by trochanter (15.8%) and calcaneus (10.5%). Conclusion: It is recommended the adoption of nursing practices of suppression or reduction of the time in the use of mechanical restraint, adopting educational measures and prevention of PI.
