ResumoIntrodução: A doença cárie e as alterações periodontais são problemas frequentemente encontrados em pacientes com deficiência. Objetivo: Avaliar as condições periodontais e de higiene bucal em escolares com deficiência neuropsicomotora matriculados em um centro de reabilitação. Método: A pesquisa envolveu entrevista com os responsáveis e exame clínico do escolar, para registro do índice de placa e do índice de sangramento. Após o exame, foi realizada profilaxia profissional, orientação de higiene bucal e esclarecimento de dúvidas. Os dados foram observados por meio de estatística descritiva. Resultado: A amostra foi constituída por 41 escolares com deficiência neuropsicomotora, com idades entre sete e 14 anos, sendo 20 do sexo feminino (48,8%) e 21 do sexo masculino (51,2%), e com idade média de 10,8 anos. A maioria dos pacientes tinha a figura da mãe como principal cuidador (85%). Observou-se que 71% dos escolares necessitavam de ajuda para realizar a higiene bucal e quase 40% dos cuidadores apresentavam dificuldade para realizá-la, principalmente pelo fato de o paciente "não abrir a boca". Quase a totalidade dos escolares não usava fio dental. Constatou-se, ainda, que 87,2% apresentaram sangramento gengival à sondagem e que, em 84,7%, verificou-se índice de placa maior ou igual a '2' . Conclusão: Concluiu-se que os escolares com deficiência neuropsicomotora incluídos neste estudo apresentaram condições periodontais e de higiene bucal insatisfatórias, demonstrando a necessidade de serem instituídas ações preventivas focadas na orientação aos pacientes e cuidadores, para a efetividade da higiene bucal.Descritores: Higiene bucal; pessoas com deficiência; gengivite. AbstractIntroduction: Caries and periodontal disease are problems frequently affecting patients with disabilities. Objective: In this study we aim to evaluate the periodontal conditions and the oral hygiene in students with neuropsychomotor disorders enrolled in a rehabilitation center. Method: Data collect was based in interview with the responsible and the scholar's clinical exam to register the plaque index and the bleeding index. After the clinical exam were performed professional prophylaxis, oral hygiene orientation and explanation of doubts. Result: The sample consisted of 41 students with neuropsychomotor disabilities, with ages among 7 and 14 years, 20 females (48.8%) and 21 males (51.2%) with a mean age of 10.8 years. Most patients had the figure of the mother as primary caregiver (85%). It was observed that 71% of students need help in performing oral hygiene and nearly 40% of caregivers had difficulty doing it, in most of the cases because the patient "did not open his mouth". Almost the totality of the students doesn't use dental floss. It was also found that 87.2% had gingival bleeding and 84.7% had the plaque index greater or equal to 2. Objective: To identify the relation between the use of continuous medication by children with neuropsychomotor disorders and the development of the dental caries disease. Material and Methods: The data were obtained from medical and dental records of children, who were assisted in an extension project of the Dentistry School of the Federal University of Pelotas. Socio-economic variables, the disability diagnosis, the type and frequency of the continuous medication, and the dental caries experience registered in the dental record were collected. The data were evaluated by double typing, analyzed by descriptive statistics, and the associations were tested by the Quisquared, Fisher exact, Mann-Whitney, and Kruskal-Wallis tests. The Stata Statistical Software, version 12.0, was utilized considering the level of significance of 5%. Results: 119 records were evaluated, and the results were distributed by age bracket due to the high age range. Undoubtedly, the cerebral palsy was the most prevalent (33.9%). From the total of the children, 68.8% were using continuous medication, and the majority (56%) uses medication there are more than 24 months. The anticonvulsants were the most used medications (33.8%), and 96.5% of the children used medication during the nocturnal period. Half of the utilized medications presented sucrose in its composition. Conclusion: A statistically significant association between the use of continuous medication, containing sucrose, and the dental caries experience were not observed, what suggests that other risk factors contribute to the disease installation.
Providing a simple and short-term educational intervention to students with disabilities had a positive impact on their oral health.
Dione Dias TORRIANI (in memorian)6 RESUMO: o objetivo deste estudo foi revisar na literatura estudos que abordem formas de realização de promoção de saúde e prevenção de doenças bucais para pacientes com deficiência visual. Foi realizada revisão sistemática da literatura nas bases de dados PubMed Central, Biblioteca Virtual de Saúde (BVS) e Scopus, sem limite de data de publicação. Foram incluídos ensaios clínicos nas línguas inglesa, portuguesa e espanhola, com algum tipo de intervenção de promoção de saúde bucal voltada para deficientes visuais totais ou parciais. Os artigos foram selecionados e os dados extraídos pelos autores. A busca resultou em 688 artigos dos sites PubMed, BVS e Scopus. Foram eliminados 303 artigos repetidos e, dentre os demais, selecionados nove que abordavam o tema desta revisão. As datas de publicação dos estudos variaram de 1991 a 2013. O total de deficientes visuais participantes destes estudos foi de 431 com idades entre seis e 49 anos. Todas as intervenções educativas foram efetivas e realizadas por odontólogos. A orientação verbal foi o recurso educativo mais utilizado Os resultados encontrados nos estudos deixam clara a necessidade e importância da implantação de estratégias de educação em saúde bucal para essa população. Programas de orientação de higiene bucal envolvendo recursos adaptados e comunicação verbal são efetivos no estabelecimento de uma rotina de higiene adequada, agregando conhecimento aos pacientes e seus familiares e resultando em melhora nos índices de saúde bucal. ABSTRACT:The aim of this study was to review the literature about oral health promotion and oral disease prevention for patients with visual impairment. A systematic literature review was conducted using PubMed Central, BVS (Virtual Health Library) and Scopus without restriction for publication date. Clinical trials in English, Portuguese and Spanish that presented any oral health promotion intervention targeting people with visual impairments were included. The articles were selected and data extracted by author. The search resulted in 688 articles from the PubMed, BVS and Scopus websites. Three hundred and three repeated items were eliminated and, among the others, 9 that addressed the topic of this review were selected. The publication dates of the studies ranged from 1991 to 2013. The total number of visually impaired participants of these studies was 431, aged 6 to 49 years. All educational interventions were effective and performed by dentists. The verbal guidance was the most used educational resource. The results demonstrated the need and importance of implementing strategies for oral health education for this population. Oral hygiene orientation programs involving tailored resources and verbal communication are effective in establishing a routine for proper hygiene, providing knowledge to patients and their families, with improvement in oral health indices.
Objetivo: relatar um caso clínico de uma criança diagnosticada com Hipomineralização Molar-Incisivo (HMI) severa, apresentando a proposta terapêutica e o acompanhamento. Relato de caso: uma paciente do sexo feminino, com 6 anos de idade, foi levada a um serviço odontológico com queixa principal de “dor no dente de baixo (lado direito)”. No exame clínico, foram constatadas opacidades demarcadas no esmalte dentário, de coloração variando de branco a tons de marrom, características de hipomineralização, nas superfícies vestibulares dos dentes 12, 11, 21, 32, 31 e 41 e nas oclusais do 16 e 26, sem perda de estrutura dentária. Nos dentes 36 e 46, observaram-se, além das opacidades demarcadas, fraturas de esmalte pós-eruptivas associadas com lesões ativas de cárie em dentina. Clinicamente e radiograficamente, não havia sinais de comprometimento da saúde pulpar. A criança foi então diagnosticada com HMI. O tratamento consistiu em restauração direta em resina composta dos elementos 36 e 46. Após 16 meses, observou-se que não houve perda de material restaurador em ambas as restaurações. A criança não relatou sensibilidade dentinária e nem sintomatologia dolorosa. Considerações finais: o tratamento proposto para o caso mostrou-se satisfatório durante o período avaliado, pois recuperou a funcionalidade dos dentes afetados pela HMI e não houve mais eventos de sensibilidade e dor.
IntroduçãoA cárie é uma doença multifatorial e, apesar de sua prevalência estar declinando nas diferentes faixas etárias, é considerada um relevante problema de saúde pública 1 . Conforme dados do Ministério da Saúde, no Brasil, aos 5 anos de idade, uma criança possui em média 2,43 dentes com experiência de cá-rie, sendo que o componente cariado é responsável por mais de 80% do índice e é a maior causa de dor dental, podendo afetar o crescimento e interferir na qualidade de vida das crianças 2,3 . Atualmente, uma das formas mais amplas da utilização do flúor é o emprego de dentifrícios fluoretados, sendo a substância mais importante presente no creme dental, aumentando o efeito de escovação mecânica sobre o controle da cárie dentária 4 . Os dentifrícios fluoretados contribuem aproximadamente com 57% da quantidade de flúor ingerida diariamente por crianças de 4 a 6 anos de idade. Isso acontece especialmente em pré-escolares, devido à inabilidade dessas crianças cuspirem todo o dentifrício 5 . Portanto, algumas medidas são necessá-rias para controlar a ingestão de fluoreto, como optar por aqueles que não seduzem a criança pelo sabor e diminuir a quantidade aplicada na escova dental 6 . Pendrys et al. 7 (2010) verificaram uma pequena relação entre o modo que algumas crianças realizavam a escovação e a incidência de fluorose. Constataram que as crianças que colocavam dentifrício na metade ou mais das cerdas da escova apresentavam frequências mais elevadas de fluorose leve. Martins
The objective of this paper was to present and to evaluate a motivational strategy for the removal of the sucking habit pacifier in preschool matriculate in schools of infantile education in the Pelotas/RS and evaluate prevalence and frequency of the sucking habit, characterizing these children according to sex, relative with whom it lives, brothers' presence and with who passes most of the time when is not in the school. The study it involved 150 children between 4 and 6 years old, of the three public schools and one private. The motivational strategy was developed in three stages: I) conversation with the parents and application of questionary, II) presentation of the problem to the child, III) development of playful activities with employment of slides, puppets and motivational appeal (pacifier tree) and IV) evaluation. The prevalence of sucking pacifier was of 24%, and the most of the children that was made use belonging to the feminine sex, it was living with the parents and was passing most of the time with them when they was not in the school, was presenting brothers and used pacifier only to sleep. Too, 63,6% of the families account told to have tried to removal of the habit. The success of the strategy was observed in (66.7%) of sample, because the abandonment the habit was established by parents or teachers relate. It was concluded that the motivational strategy proposal to was effective in the studied population and can constitute one implement in the development of education in buccal health in the school. However, it is suggested that experimental papers are conducted to confirm this hipothesis and that include more involvement of the parents and/or carers.
