O uso de Briefing/Debriefing interprofissional está associado a melhoria no processo de atendimento ao paciente em urgência e emergência em atendimento préhospitalar aéreo. Sua utilização favorece a comunicação, trabalho em equipe, gerenciamento de riscos e está associado a melhores resultados na assistência ao paciente.Justificativa: o uso do Briefing/Debriefing interprofissional é uma importante ferramenta para reduzir erros e favorecer a segurança operacional. Objetivo: descrever a experiência de profissionais que atuam em serviço pré-hospitalar aéreo no uso do Briefing/Debriefing operacional. Métodos: relato de experiência de profissionais que atuam em serviço de resgate aeromédico no uso da ferramenta Briefing/Debriefing interprofissional durante as ocorrências. Resultados e Conclusões: o briefing e debriefing são ferramentas indispensáveis para o planejamento, desenvolvimento e análise do voo. Atuam como instrumentos importantes para se pensar a assistência prestada ao paciente até antes que ela ocorra, otimizando todo o processo de trabalho a fim de reduzir os riscos. Constituem parte da normatização do serviço aéreo e representam momentos cruciais para discutir fragilidades e melhorias tanto para a segurança do paciente como para a operacionalidade do voo.
HFACS (Human Factors Analysis and Classification System) é uma ferramenta desenvolvida por Scott Shappel e Doug Weigmann. Utilizada no processo de identificação de problemas, objetiva analisar e classificar os fatores humanos, direcionar ações de prevenção, para que eventos danosos sejam mitigados (SMALL, 2020). Um sistema de segurança que proporciona o elo entre a teoria do erro humano e a prática de análise do erro humano (SOUZA, M. R. C; RUSSOMANO, 2017). Justificativa: após realização de RELPREV (Relatório de Prevenção) em uma Unidade de Aviação Pública, foi publicado uma DivOp (Divulgação Operacional). Desta leitura incitou a aplicação da estrutura do HFACS. Objetivo: conhecer a situação de perigo, analisar, classificar e atuar a fim de eliminar ou mitigar novos eventos. Métodos: segundo o objetivo da pesquisa, metodologias exploratória, descritiva, analítica e segundo as fontes de dados, laboratorial, descritiva, controlada e bibliográfica. Resultados e Conclusões: pontos importantes a serem trabalhados nos quatro níveis dessa ferramenta: precondições à atos inseguros, atos inseguros, supervisão insegura e influências organizacionais. Há 11 categorias causais: ambiente físico, estado mental adverso, gerenciamento de recurso de cabine e prontidão pessoal, erros de percepção, violação de rotina e violação excepcional, supervisão inadequada e falhas em corrigir problema conhecido, clima e processo organizacional. O incidente cabível de ser evitado se uma das falhas, latentes ou ativas, em um dos quatro níveis, tivesse sido interrompida em momento adequado. Novos eventos de RELPREV devem ser incitados para novas aplicações da estrutura do HFACS e mitigar riscos. Palavras-chave: Gestão; Prevenção; Fatores humanos. RESUMO:O atendimento às vítimas de causas externas gera um grande impacto para a economia abrangendo a capacidade laborativa, seguridade social e custo dos serviços de saúde, podendo esse impacto também ser percebido nos serviços préhospitalares aéreos. Dessa forma, esse estudo objetiva analisar os atendimentos às vítimas por causas externas por um serviço pré-hospitalar aéreo público do Distrito Federal e verificar fatores associados. Realizou-se um estudo transversal com 595 registros de atendimentos do serviço pré-hospitalar aéreo público do Distrito Federal de 2016 a 2017. Realizou-se modelo simples e múltiplo de regressão logística. Metade dos atendimentos ocorreu às vítimas de causas externas (53,6%). Os atendimentos aos casos clínicos (31,6%) e transporte (14,5%) foram menos frequentes. A maioria dos atendimentos ocorreu no período diurno (73,6%). Os atendimentos por causas externas apresentaram maior chance de ocorrer em vítima do sexo masculino (Odds Ratio -OR): 1,99; Intervalo de Confiança (IC) 95% 1,24-3,20), com idade entre 15 e 29 anos (OR: 12,28; IC 95% 5,14-29,35), no período noturno (OR: 2,48; IC 95% 1,33-4,65) e nos finais de semana (OR: 1,61; IC 95% 1,02-2,55). O atendimento por causas externas representou importante demanda para o serviço pré-hospitalar aéreo, com maior chance de ocorrerem com homens jovens e acrescentando maior risco ao atendimento por terem maior chance de ocorrerem no período noturno. Isso reforça o forte impacto das causas externas nas perdas humanas, nas incapacidades e nos custos dos serviços de saúde. do CARMO 4 . RESUMO: Justificativa Analisar o uso do ácido tranexâmico (TXA) no serviço préhospitalar. Objetivo: analisar o perfil clínico, epidemiológico e desfecho dos usuários atendidos em um serviço pré-hospitalar aéreo público do Distrito Federal. Método: estudo descritivo com delineamento transversal e abordagem quantitativa.Resultados: Houve maior prevalência de pacientes do sexo masculino, idade média de 38 anos com lesão em tórax (19,05%), cabeça (23,4%) e abdômen (9,52%).Houve redução no grau de choque e parâmetros hemodinâmicos do momento préhospitalar para o intra-hospitalar após uso do TXA e demais medidas para controle da hemorragia. Conclusão: Observou-se no presente estudo que o uso do TXA associado a outras medidas de controle de hemorragia mostrou-se eficaz para melhora da condição hemodinâmica dos pacientes. A reposição volêmica dos pacientes ainda no período pré-hospitalar e demais medidas para contenção de hemorragia mostraram-se efetiva para reanimação em graus severos de choque.
Este relato descreve o uso do ácido tranexâmico em situação de choque hemorrágico na abordagem inicial ao paciente vítima de trauma. O caso descreve a atuação da equipe de resgate aéreo em apoio à unidade de atendimento préhospitalar fixa no atendimento a homem jovem com múltiplas perfurações em tórax por arma branca com evolução rápida à condição de choque hipovolêmico grave e posterior transporte à unidade especializada com centro de trauma. Justificativa: o serviço aéreo de resgate é uma importante ferramenta quando o assunto é agilidade para atendimentos aos casos de vítimas em grave estado de saúde, como nas situações de choque hipovolêmico causados por hemorragia. Para essa condição clínica é fundamental que se inicie a administração endovenosa precoce de ácido tranexâmico. Objetivos: descrever o uso do ácido tranexâmico em paciente vítima de trauma com hemorragia severa e a importância do serviço de suporte avançado de vida no rápido transporte de paciente para unidade de saúde especializada. Método: trata-se de um relato de experiência baseado em atendimento inicial a paciente vítima de trauma por múltiplas perfurações por arma branca em tórax realizado por serviço aéreo de resgate em conjunto com Unidade de Pronto Atendimento -UPA com posterior transporte para tratamento definitivo em centro de trauma de referência. Resultados: a vítima foi transportada após ser estabilizada hemodinamicamente pela equipe do serviço de resgate aéreo. Conclusão: A administração endovenosa precoce de ácido tranexâmico repercute positivamente no prognóstico de vítimas de trauma com hemorragia severa.Palavras-chave: resgate aéreo, choque hemorrágico, ácido tranexâmico.
