A Terapia Infusional vem sendo difundida e aprimorada nos países que buscam por um alto padrão de qualidade em saúde. Estudos mostram que as instituições buscam a excelência assistencial utilizando estratégias de gestão de qualidade que aprimoram as atividades já existentes, alcançando resultados a nível assistencial, econômico, burocrático e administrativo. O estudo teve por objetivo descrever estratégias multimodais sob a ótica da segurança do paciente no uso de acesso vasculares, demonstrando seus benefícios através da prática baseada em evidências para a gestão e fortalecimento da Comunicação Organizacional. Os resultados mostraram-se promissores sob a visão de diversos países e instituições quanto ao impacto positivo que ultrapassaram barreiras, promovendo uma mudança tanto na cultura de segurança do paciente como na política institucional. O desenvolvimento dos profissionais de saúde, comunicação efetiva e experiência do paciente ganharam destaque nesta pesquisa. O estudo propõe uma reflexão sobre a necessidade do engajamento de toda a organização para a implementação da Terapia Infusional de forma mais efetiva e incita novas pesquisas partindo de um olhar mais abrangente sobre o tema.
O cateter central de inserção periférica (PICC) é considerado um dispositivo importante e confiável para o tratamento intravenoso de longo prazo. A técnica seldinger modificada (TSM) para a inserção do PICC surge como um método efetivo na redução de eventos adversos relacionados à falha mecânica durante a inserção ou identificados mais tardiamente. O objetivo deste estudo foi descrever a importância da técnica Seldinger modificada na inserção do PICC na prevenção de eventos adversos. Uma revisão narrativa de literatura com abordagem qualitativa foi realizada analisando 10 artigos científicos dos últimos doze anos em nove países. Os resultados mostraram a eficácia da TSM na inserção do PICC acima de 90% e baixa ocorrência de complicações. Além disto, os estudos demonstraram também a efetividade da técnica utilizada com baixa taxa de eventos adversos durante sua utilização. Concluiu-se que a TSM na passagem do PICC é um método seguro, confiável, eficaz, econômico e efetivo, além de proporcionar maior segurança, conforto e satisfação do cliente que o utiliza.
Objetivo: Avaliar se a radioiodoterapia com 131I-metaiodobenzilguanidina (131I-mIBG) aumenta a sobrevida de pacientes com diagnóstico de neuroblastoma, mIBG-ávido, refratários ao tratamento ou de alto risco para recidiva, independentemente do estadiamento da doença. Métodos: Revisão sistemática da literatura. Consultadas as bases Medline (PUBMED), EMBASE e Cochrane Library, sendo incluídos estudos disponíveis em texto completo, publicados nos idiomas inglês, espanhol ou português. Os desfechos de interesse foram a sobrevida global e livre de progressão da doença. Principais Resultados: O tratamento pode ser viável e tolerável em pacientes com neuroblastoma de alto risco, entretanto, os benefícios deste regime para os desfechos de sobrevida são observados apenas nos pacientes que estavam em resposta completa ou muito boa resposta parcial após quimioterapia de indução, antes do início do tratamento com 131I-mIBG. Conclusões: Existem incertezas quanto a efetividade do tratamento com 131I-mIBG para o aumento da sobrevida de pacientes com diagnóstico de neuroblastoma.
Introduction: Despite penicillin being the drug of choice for the treatment of syphilis, many pregnant women who test positive for syphilis do not receive the drug as recommended by the Ministry of Health, contributing to the increase in costs associated with congenital syphilis. Objective: This study aims to estimate the incremental cost-effectiveness ratio of administering at least one dose of 2.4 million IU of benzathine penicillin in the first trimester of pregnancy as soon as the result of a positive rapid treponemal test performed during antenatal care in primary care units of the Brazilian National Health System. Methods: An analytical model was proposed based on a decision tree. The perspective of the analysis was the one used in The Brazilian National Health System. The clinical outcomes were abortion, prematurity, neonatal death, stillbirth, and congenital syphilis, estimated in terms of disability-adjusted life-years. Only direct costs were considered. Deterministic and probabilistic sensitivity analyses were performed. Results: The model predicted that the most efficient strategy is the one that includes the administration of penicillin in primary care for cases of gestational syphilis. This strategy is more effective, although more costly. The cost per disability-adjusted life-years averted with the use of this strategy was estimated at R$49.79 (US$ 10.67). Conclusion: The prenatal strategy in primary care units that includes the administration of penicillin to pregnant women with syphilis during the first trimester of pregnancy has the greatest potential to be cost-effective.
