Seu diagnóstico representa um desafio clínico de grande importância, pois permite um tratamento precoce e direcionado para estas situações potencialmente graves. Da mesma forma, o acompanhamento evolutivo destas patologias depende de dados clínicos e/ou laboratoriais que permitam diferenciar atividade de remissão. No entanto, o quadro clínico inicial das vasculites é comumente inespecífico, exigindo a espera de uma confirmação histológica para o seu diagnóstico. Além disso, as recidivas da doença costumam apresentar-se de forma também incaracterística tanto do ponto de vista clínico quanto laboratorial. Assim sendo, a descoberta de anticorpos contra o citoplasma de neutrófilos (ANCA),(1,2) um marcador sorológico possivelmente específico para as vasculites sistêmicas, produziu grande impacto na literatura médica internacional. Desde então, vários consensos internacionais foram realizados, com o objetivo de padronizar técnicas e difundir conhecimentos atualizados sobre o papel desses anticorpos nas vasculites e outras doenças. A primeira descrição deste marcador surgiu no início da década de 1980, com o relato da presença de IgG contra componentes do citoplasma de polimorfonucleares neutrófilos no soro de 8 pacientes.(1) Estes pacientes apresentavam quadro clínico de glomerulonefrite necrotizante segmentar pauciimune associada a manifestações sistêmicas. Neste sentido, um estudo multicêntrico realizado na Holanda e Dinamarca (2) demonstrou o valor da detecção de ANCA por imunofluorescência, no diagnóstico e acompanhamento de portadores de granulomatose de Wegener (GW). Esta descoberta marcou o início do estudo dos ANCA nas vasculites e certamente representou um dos maiores avanços no diagnóstico e estudo destas patologias. Os autores detectaram ANCA em 25/27 portadores de GW ativa contra 4/32 com a doença em remissão. Não foi observado qualquer teste falso-positivo no grupo controle, que incluía diversos pacientes com vasculites variadas, glomerulonefrites, doenças do conectivo e doenças granulomatosas. O teste foi considerado altamente específico para GW e parecia ter relação com a atividade da doença.A descoberta do marcador sorológico denominado anticorpo anticitoplasma de neutrófilos revolucionou o diagnóstico e o seguimento das vasculites pulmonares, especialmente da granulomatose de Wegener. Seu padrão pode ser citoplasmático e perinuclear. Sua titulação auxilia no diagnóstico e no seguimento das vasculites pulmonares.
