BackgroundThe venous access for the insertion of permanent leads of cardiac implantable electronic devices is often achieved by venous cutdown of the cephalic vein, or by “blind” puncture of the subclavian vein using anatomical landmarks, or by fluoroscopy‐assisted methods.MethodsWe have retrospectively analyzed our clinical experience to verify the feasibility, the safety, and efficacy of the adoption of ultrasound‐guided puncture/cannulation of the axillary vein for this purpose.ResultsNine hundred eighty‐seven leads were placed during 548 consecutive procedures, accessing the axillary vein in the infraclavicular area using real‐time ultrasound guidance. Venipuncture was successful in 99.8% of cases. The access time was 11 seconds (range 4‐580). We recorded three cases of pneumothorax (0.5%), but no hemothorax and no hemo‐mediastinum. The incidence of local hematoma was 2.1% (12 cases). No injury to the brachial plexus or to the phrenic nerve was recorded. In a follow‐up of 33 months (range 16‐39), we observed no cases of “subclavian crush syndrome” (damage of the leads at the level of the thoracic inlet), and the rate of pocket infection/infective endocarditis was 0.7%.ConclusionIn our experience, ultrasound‐guided puncture/cannulation of the axillary vein for implantation of permanent leads is feasible, effective, and safe. It might be considered as a first option for this procedure.
Resumo Este artigo ressalta a diferença de comportamento da China no Conselho de Segurança nos períodos entre 1971 e 2006, e entre 2007 e 2012. O primeiro período, marcado pela presença mais discreta do país entre os membros permanentes, e o segundo caracterizado por um comportamento mais ativo, resultado direto de seu novo papel na ordem e no sistema internacional, e com potencial para reconfigurar a política entre potências e a geopolítica mundial.
Introduction: In the covid-19 pandemic, health services began to reorganize and offer telehealth assistance modalities. The Cognitive Rehabilitation Service has been restructured to receive new patients. There was a need to develop an evaluative resource for remote patient screening. Objectives: To adapt the cognitive screening instrument Mini-Mental State Examination (MMSE) to an informal oral interview script useful in call centers. Methods: Adaptation of the 11 domains of the MMSE that were converted into informal questions, eliminating the need to apply it according to routine methodological rigor. We opted for colloquial oral language, necessary for the referred telemarketing, making it easier to screen patients using technological resources incorporated from mobile phones and videoconferences. Results: An interview script was developed with 38 questions equivalent to the MMSE. Conclusion: Facing social isolation, due to the restriction of social participation to the homes of professionals and patients, provided an opportunity to innovate actions and care structures in Cognitive Rehabilitation. An adapted instrument was proposed, capable of allowing the continuity of actions, appropriate to the establishment of Telehealth, during the current pandemic.
O raciocínio clínico é importante para a caracterização do domínio da prática em Terapia Ocupacional (TO), mas o seu aprendizado é difícil, em função de sua estrutura complexa que, ao incluir as dimensões científica, narrativa, pragmática e ética, implica na execução de variadas operações mentais pelo aluno no seu aprendizado e pelo profissional na sua atuação cotidiana. Este artigo objetiva apresentar a metacognição como recurso auxiliar para o ensino do raciocínio clínico em Terapia Ocupacional. Realizou-se estudo metodológico fundamentado em revisão narrativa que busca estabelecer interface entre o ensino do raciocínio clínico em TO e as bases teóricas e conceituais do domínio metacognitivo e da aprendizagem significativa. Percebe-se a necessidade de reflexão e pensamento crítico para fazer diagnóstico clínico em TO. A metacognição caracteriza-se pelo seu foco dirigido a esses processos e por sua capacidade de explicitá-los e de permitir o seu gerenciamento. No entanto na Terapia Ocupacional, o uso da metacognição ainda não faz parte das propostas de ensino-aprendizagem do raciocínio clínico. Dado o seu desempenho positivo em domínios similares, conclui-se ser factível o uso da metacognição para facilitar o ensino e o aprendizado do raciocínio clínico em TO.
