Objetivo: Verificar se a estratégia de simulação clínica melhora o ganho de conhecimento e de autoconfiança da equipe de profissionais de saúde em situações de parada cardiorrespiratória (PCR) e/ou obstrução de vias aéreas por corpo estranho (OVACE) na atenção primária à saúde (APS). Metodologia: Estudo quantitativo, quase-experimental, desenvolvido em unidades básicas de saúde. Amostra composta por 68 profissionais de saúde da APS. Aplicou-se questionário semi-estruturado e escala de autoconfiança. Realizou-se análise descritiva e na inferencial considerou-se significativo resultados com p≤0,05. Resultados: Identificou-se ganho significativo de conhecimento (p <0,001) e de autoconfiança para assistência em situações de PCR e OVACE pela equipe de enfermagem (p=0,001) e agentes comunitários (p=0,005), após emprego da simulação como estratégia de ensino. Conclusão: A simulação mostrou efeito positivo para ganho de conhecimento e de autoconfiança para profissionais de saúde, inclusive daqueles que não participam diretamente do cuidado, como os agentes comunitários de saúde.Descritores: Conhecimento; Atenção Primária à Saúde; Treinamento por Simulação; Confiança.IMPLICATIONS OF SIMULATION ON SELF-CONFIDENCE AND KNOWLEDGE OF PROFESSIONALS IN PRIMARY CARE: qUASI EXPERIMENTALObjective: To verify if the clinical simulation strategy improves the knowledge and self-confidence gain of the healthcare team in situations of cardiopulmonary arrest (CPR) and/or foreign body airway obstruction (OVACE) in primary health care (PHC). Methodology: Quantitative, quasi-experimental study, developed in basic health units. The sample composed of 68 PHC health professionals. A semi-structured questionnaire and a self-confidence scale were applied. Descriptive analysis was performed and inferential analysis was considered significant with p≤0.05. Results: A significant gain of knowledge (p <0.001) and self-confidence in assisting in CRP and OVACE situations was identified by the nursing staff (p = 0.001) and community agents (p = 0.005), after the use of simulation as a strategy for teaching. Conclusion: The simulation showed a positive effect to gain knowledge and self-confidence for health professionals, including those who do not participate directly in care, such as community health agents. Resultados: El personal de enfermería (p=0.001) y los agentes comunitarios (p=0.005) identificaron una ganancia significativa de conocimiento (p<0.001) y confianza en sí mismos para ayudar en situaciones de CRP y OVACE, después del uso de la simulación como estrategia para enseñanza. Objetivo: Verificar se há influência da ventilação mecânica com pressão positiva, ao final da expiração, na função renal de pacientes internados em unidade de terapia intensiva. Método: Estudo prospectivo e quantitativo realizado, em unidade de terapia intensiva, especializada em trauma ortopédico e medular. A amostra foi não probabilística, composta por 31 pacientes em ventilação mecânica invasiva que desenvolveram lesão renal aguda na internação. Os pacientes foram alocados em grupos, conforme valor da pressão positiva, ao final da expiração, e os dados coletados por questionário estruturado. Para a análise das variáveis, realizaram-se testes não paramétricos. Resultados com p ≤ 0,05 foram considerados significativos. Resultados: A idade média foi 46,94±24,2 anos com predominância do sexo masculino (61,3%). A disfunção renal (70,97%), no estágio 1(risco) (35,5%), predominou. Pacientes com maior pressão positiva, ao final da expiração, evoluíram com disfunção renal (p=0,04). Além disso, para aqueles com pressão positiva, ao final da expiração≥ 10cmH2O, a idade (p=0,05) e a disfunção renal (p=0,04) mostraram-se associados significativamente. Conclusão: Pacientes críticos em ventilação mecânica invasiva com pressão positiva, ao final da expiração mais elevada, mostraram reunir maior predisposição para a lesão renal aguda.
Resumo Objetivo avaliar o efeito do monitoramento de enfermagem avançado em relação ao usual para identificação do risco cardiovascular e renal, adesão medicamentosa e prática de exercícios na atenção primária à saúde. Método estudo quantitativo, quase-experimental desenvolvido na Atenção primária. Em relação a intervenção, foram consultas de enfermagem trimestrais de monitoramento avançado e laboratorial trimestral. Para coleta de dados, aplicou-se questionário sociodemográfico, escala Morisky e Questionário Internacional de Atividade Física. Resultados sexo feminino predominou nos dois grupos intervenção e controle (62,79% vs. 76,74%). O percentual de pacientes sem risco cardiovascular do grupo intervenção superou o grupo controle da consulta 1 para consulta 3, (0,00% - 25,58% vs. 6,98 - 2,33). Adesão medicamentosa máxima, ao longo do tempo, foi superior no grupo intervenção comparado ao grupo controle (48,8% vs. 23,3%). O risco cardiovascular dos usuários interferiu na atividade física de forma significativa no grupo intervenção e controle (p=0,0261 vs. 0,0438). Conclusões e implicações para a prática a monitorização avançada possibilitou uma melhor identificação de pacientes de risco e orientações aos pacientes hipertensos e diabéticos com risco cardiovascular e renal, o que favoreceu o monitoramento avançado e contribuiu ao autogerenciamento da prática de exercícios e adesão medicamentosa, a partir de consultas de enfermagem.
