a RESUMENLa transición epitelio mesénquima (EMT) es un proceso compuesto de diferentes fases, donde una célula epitelial adquiere un fenotipo mesenquimal. Dentro de los cambios involucrados se encuentran: pérdida de la polaridad celular, adquisición de una capacidad migratoria, capacidad invasora, resistencia a la apoptosis y aumento en la producción de componentes de la matriz extracelular. Todos estos cambios ocurren como una consecuencia de la activación y represión de genes involucrados con rutas de señalización específicas relacionadas con este evento. La EMT está relacionada con procesos fisiológicos y patológicos como el cáncer. Consta de tres fases: una de células no migratorias, células premigratorias y células migratorias; cada una de ellas producto de diferentes señales intra o extracelulares, factores de transcripción (TGF-B, Snail, TWIST, Sox, Slug, ZEB1, entre otras) y proteínas involucradas (E-cadherina, integrina, vimentina, ocludinas y claudinas).
RESUMENAntecedentes: el cáncer escamocelular de cabeza y cuello puede ser causado principalmente por un alto consumo de cigarrillo y alcohol. Sin embargo, cerca de un 30 % de los tumores puede ser ocasionado por una infección por virus del papiloma humano (VPH), principalmente en la orofaringe. En la última década, la frecuencia de este tipo de tumor se ha incrementado mundialmente. Propósito: analizar las características y diferencias que presentan los tumores de orofaringe asociados o no a la infección por VPH. Métodos: se revisó la literatura sobre la asociación entre el VPH, el cáncer de orofaringe escamocelular y su pronóstico. Hallazgos: el VPH es un virus oncogénico y se calcula que cerca de 40 tipos virales pueden infectar al ser humano, de los cuales 14 están relacionados directamente con procesos carcinogénicos. Varios estudios muestran que el genotipo VPH 16 se presenta con mayor frecuencia y agresividad que los otros genotipos de VPH asociados con este tipo de tumor. El cáncer de orofaringe presenta un mayor predominio en el género masculino, posiblemente relacionado con prácticas como el sexo oral. Recientemente, se ha establecido un mejor pronóstico para tumores VPH positivos en comparación con los negativos. Esto se debe a que los primeros presentan una mejor respuesta a la quimioterapia y a la radioterapia, lo que da como resultado una tasa de supervivencia global favorable (p = 0,046, log-rank), según estimaciones por curvas de Kaplan-Meier. However, about 30% of tumors can be caused by infection with human papillomavirus (HPV) mainly in the oropharynx. In the last decade, the frequency of this tumor has increased worldwide. Purpose: To analyze the characteristics and differences of oropharyngeal tumors associated or not with HPV infection. Methods: A review of the literature on the association between HPV, oropharyngeal squamous cell cancer, and its prognosis was performed. Findings: HPV is an oncogenic virus. It is estimated that about 40 HPV types can infect humans, of which 14 are directly related to carcinogenic processes. Several studies show that the HPV 16 genotype occurs more frequently and aggressively than other HPV genotypes associated with this type of tumor. Recently, a better prognosis for HPV-positive tumors has been observed when compared to negative tumors. El cáncer escamocelular es uno de los tumores más frecuentes de cabeza y cuello, ocupa el quinto lugar entre todos los tipos de cáncer en el mundo, entre los factores de riesgo se encuentran el consumo de tabaco y alcohol y las infecciones virales como el Virus del Papiloma Humano (VPH), éste último principalmente para el carcinoma orofaríngeo de células escamosas, donde se considera como el principal factor de riesgo, ya que según datos epidemiológicos existen frecuencias de prevalencias que varían desde un 20% hasta en un 80% dependiendo de la región geográfica. En Colombia solo existe un estudio de cabeza y cuello asociado a VPH, realizado por Quintero y colaboradores en el 2013, donde se reportó una prevalencia de 18.9%. Por esta razón el presente estudio busco identificar los tipos virales de alto riesgo más frecuentes en una muestra de pacientes con cáncer orofaríngeo así como determinar los niveles de expresión y estado físico del genoma viral (integrado/ episomal) para el tipo de virus más prevalente. El presente estudio fue de tipo multicéntrico descriptivo donde se incluyeron 46 adultos que asistieron a los servicios de cirugía de cabeza y cuello en las ciudades de Bogotá, Manizales y Bucaramanga, previo consentimiento informado. Todas las muestras diagnosticadas histopatológicamente con carcinoma escamocelular de orofaringe fueron incluidas en este estudio. A las muestras se les realizo extracción de ADN y ARN, con el ADN se realizó genotipificación de VPH y se estableció el estado físico del genoma viral. Con el ARN se determinó el número de copias por PCR en tiempo real. La prevalencia de VPH fue de 21.74% de los tumores, El tipo viral más frecuente fue el VPH 16 (9 casos) y el VPH58 (1 caso). La expresión viral fue baja para VPH 16 (1-11 copias) y el estado físico predominante de este virus fue el mixto (8 casos), prevaleciendo la disrupción en el sitio de unión de E1 y E2 (nt 2525 a 3720).
Objetivo: evaluar el impacto del cambio en la expresión de los biomarcadores, utilizados para la clasificación del cáncer de mama en subtipos intrínsecos, asociado al tratamiento neoadyuvante en el pronóstico de pacientes con la enfermedad. Metodología: Se llevó a cabo un estudio retrospectivo en 337 mujeres diagnosticadas con cáncer de mama. La población se agrupó en casos sin neoadyuvancia (NAT) (n=158) y con NAT (n=179), y de acuerdo con el esquema de tratamiento en NAT hormonal (n=10), citotóxica (n=122), citotóxica + trastuzumab (n=32) y combinada (hormonal + citotóxico, n= 15). Se evaluaron las diferencias en el estatus y el porcentaje de expresión por inmunohistoquímica (IHQ) del receptor de estrógeno (RE), receptor de progesterona (RP), HER2 y Ki67 entre muestras pareadas de biopsia y producto de la cirugía de mama. Adicionalmente, se analizaron las diferencias en la supervivencia global (SG) y libre de recurrencia (SLR) de acuerdo con la ganancia y pérdida de los biomarcadores, y finalmente se calculó el cociente de riesgo (HR) de acuerdo con las variaciones en el estatus de los biomarcadores, en los grupos con NAT y sin NAT. Resultados: Se reportaron disminuciones en el porcentaje de expresión del RP (p=0.0024) y del Ki67 (p-0.001) después de la NAT, específicamente asociados al esquema hormonal (RP: p= 0.01732, Ki67: p= 0.02154) y citotóxico (RP: p= 0.02604, Ki67: p= 0.00736). El esquema de NAT citotóxico + trastuzumab se asoció a la disminución en la positividad del HER2. La pérdida de expresión del Ki67 después de la NAT se encontró asociado con una mejor SG (p= 0.038) y SLR (p= 0.0025), así como con una reducción en el riesgo de recurrencia (HR: 0.12, 95% CI, 0.01–0.98, p=0.0483), después de ajustar por variables clínico-patológicas. El análisis en el grupo sin NAT mostró cambios estadísticamente significativos en el porcentaje de expresión del RE (p= 0.022), asociados a la variabilidad inter- observador. Conclusiones: los resultados de este estudio sugieren que la NAT se encuentra asociada al cambio en la expresión de los biomarcadores de mama, especialmente del Ki67, y que este evento confiere menor riesgo de recurrencia. Otros factores como la variabilidad inter- observador también pueden estar asociados a la presentación de cambios en los biomarcadores de mama.
En esta investigación se revisó el cumplimiento de normas éticas de investigación en 41 tesis de Maestrías en Biología elaboradas en la Universidad Autónoma de Chiriquí (Panamá) de 2002 a 2018. El instrumento de medición fue una lista de verificación de ocho criterios, basados en principios éticos de la investigación establecidos en normas éticas nacionales e internacionales vigentes. Las tesis estudiadas fueron éticamente deficientes en forma global con 24,3 % de cumplimiento de normas éticas de investigación. Ninguno de los ocho criterios evaluados alcanzó el nivel de éticamente “excelente”. Considerando los sujetos de estudio, 32,7 % de las tesis hechas en sujetos humanos, presentó buen cumplimiento de normas éticas; mientras que fueron deficiente, las tesis hechas en sujetos no humanos (23,3 %). Los criterios mejor evaluados en tesis hechas utilizando sujetos humanos fueron: “validez científica” (79,1 %), “valor social”: (56,2 %), “selección justa de sujetos participantes” (53,1 %) y “revisión del consentimiento informado (48,7 %); los peor evaluados fueron: “compromisos del autor” (28,9 %), “respeto a los participantes” (16,2 %), “balance favorable riesgo-beneficio” (16,2 % y “revisión independiente” (10,4 %). Los criterios mejor evaluados en tesis hechas utilizando sujetos no humanos, fueron: “validez científica” (75,4 %), “valor social”: (45,4 %), “selección justa de sujetos participantes” (40,5 %); mientras que los peor evaluados, fueron: “compromisos del autor” (27,2 %), “balance favorable riesgo-beneficio” (22,2 %), “revisión independiente” (11,9 %), “respeto a los participantes” (2,7 %) y “revisión del consentimiento informado” (1,3 %). La falta de comité de ética de investigación institucional y la no obligatoriedad de someter a normas éticas las tesis de Maestrías en Biología; afecta el nivel de cumplimiento de normas éticas en las tesis estudiadas.
Introduction: Breast cancer is the most prevalent cancer in women worldwide. It is a heterogeneous disease that has been classified into intrinsic subtypes based on gene expression analyses. Previous studies have reported a higher proportion of HER2-positive (HER2+) tumors in Hispanics/Latinas, compared to Non-Hispanic White women, characterized by their aggressive phenotype. A gene expression analysis of breast tumor tissues from Colombian women reported that patients with a higher Indigenous American (IA) ancestry expressed higher mRNA levels of ERBB2, and of other genes in the HER2-amplicon region (17q12) such as GRB7. Although the expression of HER2 and GRB7 is highly correlated, it has been reported that these proteins might also be expressed independently. It is still unclear whether IA ancestry is associated with the co-expression of HER2/GRB7, or only with one of these two proteins. Aim of the study: To explore differences in HER2 and GRB7 protein expression according to genetic ancestry and the presentation of clinical-pathological features in Colombian women with breast cancer. Methods: A total of 517 Colombian breast cancer patients from different health institutions were included. For genetic ancestry estimation, 106 ancestry informative markers were genotyped from non-tumoral DNA. The protein expression of HER2 and GRB7 was evaluated in paraffin blocks with tumoral tissue by immunohistochemistry by a single pathologist. We tested the association between genetic ancestry and protein expression of HER2, GRB7, and the co-expression of both using logistic and multinomial logistic regression models. Results: Higher IA ancestry fractions were observed in patients with HER2+ compared to HER2- tumors (46% vs 41%, respectively, p = 0.013). We found higher odds of having HER2+ (OR=1.65, 95% CI, 1.05-2.63, p=0.0293) and GRB7+ tumors (OR=1.92, 95% CI, 1.03-3.56, p=0.0376) for every 25% increase in the IA ancestry proportion. The association between IA ancestry and HER2/GRB7 co-expression was influenced by estrogen receptor (ER) status. Patients with a higher IA ancestry (>42%) and with ER+ subtypes presented higher odds of having HER2+/GRB7- breast tumors (OR=3.59, 95% CI, 1.11-11.65, p=0.0327), whereas ER- patients with high IA ancestry tended to have HER2+/GRB7+ tumors (OR=2.27, 95% CI, 0.69 - 7.45, p=0.1730). The co-expression of these proteins was found associated with advanced clinical stages (p=0.018), less differentiated tumors (p=0.005), and a higher proliferation index (p<0.001), compared to HER2+/GRB7- and HER2-/GRB7- tumors. Conclusions: Our results confirm the previously reported association between IA ancestry and HER2 expression in breast tumors and suggest that HER2/GRB7 co-expression in patients with high AI ancestry may change according to the ER status. This event also correlates with unfavorable clinical-pathological features. Introduction: HER2-low expression tumors, either with weak or incomplete membrane expression (immunohistochemistry (IHC) 1+ or 2+ with negative in situ hybridization (ISH) result), are considered and treated as HER2-negative tumors. However, recent clinical trials have shown promising results in the application of antibody-drug conjugates (ADCs) for patients with HER2-low breast tumors, highlighting the need to characterize the clinical phenotype of this group. This is the first study in Colombia aiming to explore the clinical-pathological features and survival of women affected with HER2-low breast cancer, compared with HER2-negative (0+) and HER2-positive groups (2+ with ISH+ or 3+). Methods: We included 516 breast cancer patients from different health institutions in Colombia, diagnosed at clinical stages that ranged between I-III. Protein expression of HER2, Ki67, and hormone receptors (estrogen (ER) and progesterone (PR) receptor) was evaluated by IHC in a paraffin block with tumoral tissue by a single pathologist. We defined HER2 expression as positive (3+ or 2+ with positive ISH test), low (1+/2+ with negative ISH result), and negative (0+). We applied a Chi-square or Fisher’s exact test to evaluate clinical-pathological features for HER2-low cases, compared with HER2-negative (low vs. negative) and HER2-positive tumors (low vs. positive). Differences in overall survival (OS) and risk of mortality were assessed using the Kaplan-Meier and log-rang test, and a Cox proportional hazard model. Results: 325 cases (63%) were classified as HER2-negative, 97 (18.8%) as HER2-low, and 94 (18.2%) as HER2-positive. We encountered more clinical-pathological significant differences between HER2-low and HER2-positive tumors than with HER2-negative cases. A higher proportion of patients diagnosed after the age of 50 (79.4% vs. 62.4%, p=0.018), with better-differentiated tumors (Bloom Richardson II: 58.8 vs. 41.2%, p=0.025), and a lower proliferation index (Ki67 <20%: 50.5 % vs. 21.2%, p<0.001) was observed for HER2-low tumors, compared to the HER2-positive group; these characteristics were not significantly different between HER2-low and HER2-negative tumors. The proportion of ER-positivity was significantly higher in the HER2-low group, compared to both HER2-positive (89.7% vs. 62.4%, p<0.001) and negative groups (89.7% vs. 79.7%, p=0.039). Moreover, although longer OS rates were found in HER2-low patients, compared to the negative group (5.5 vs. 5.0 years, p=0.0032), the multivariate Cox model adjusted by ER status showed no statistically significant differences between these groups (HR=0.62, 95% CI, 0.34 – 1.13, p=0.122). Conclusions: HER2-low tumors represent an important proportion of all breast cancer cases diagnosed in Colombian women. Our results are consistent with other reports that show differences in ER expression between HER2-low tumors and both HER2-positive and negative subtypes, although it seems that the prognosis of HER2-low patients is not different than of the other groups.
