Successful root canal treatment depends on thorough cleaning & shaping and 3- dimensional fluid impervious obturation of tooth within the confines of canals. To achieve this objective the apical constriction must be detected accurately during canal preparation and precise control over working length during the procedure must be maintained. There are many methods of working length determination including radigraphs and electronic method(apex locator). Introduction of apex locators have definitely served as an effective adjuvant to radiographs.
Paget's disease is a chronic progressive disturbance of bone metabolism, characterized by simultaneous resorption and apposition neither of which are related to functional requirement, resulting in larger amount of less dense bones that are very vascular and fragile. When the jaws are involved with Paget's disease, even routine and systemic procedures may become complicated. Osteomyelitis when occur with Paget's disease, it complicates the disease. A significant number of patients develop osteomyelitis secondary to locally infected extraction sites. We present a case report of 59 years old male patient with osteomyelitis of mandible as a complicating factor in Paget's disease.
