Bei 10 Katzen wurden nach einem rechts parietalen Kälte-Hirntrauma der Ventrikeldruck (VP) ; der Epiduraldruck über der Läsion (rEP) und über dem linken Parietalbereich (1EP) fortlaufend gemessen. Gleichzeitig wurden arterieller (aP) und zentralvenöser (cvP) Druck aufgezeichnet. Das EEG wurde beidseits fronto-occipital bipolar abgeleitet und mit einem Rechner (FFT) kontinuierlich ausgewertet. Wir konnten so Hirndruckverlauf und EEG-Aktivität vor und nach Trauma und Therapie während der Gesamtdauer der Untersuchung anschaulich darstellen. In Abständen wurde die Serumosmolalität bestimmt.Sofort nach dem Trauma steigt der Hirndruck rasch an. Über der direkt geschädigten rechten Hemisphäre ist der epidurale Druckanstieg deutlicher. Auch der 1EP übersteigt den VP. Posttraumatisch verringert sich die EEG-Power-Intensität sofort, besonders über der rechten Hemisphäre. Eine spontane Erholung zeigt sich konstant nur über der linken Seite. Im a-, gelegentlich im HBand ist die Besserung deutlicher. Mit steigendem Hirndruck kommt es zunächst rechts zu einer Abnahme der EEG-Intensität.Eine Hyperventilationstherapie (art. PC0 2 20-25 mmHg) etwa 1 h nach dem Trauma (VP um 30 mmHg) senkt alle Hirndrucke rasch, der VP fällt um mehr als 15 mmHg. Nach diesem sprungartigen Abfall jedoch steigt der Druck wieder kontinuierlich an. Ist die Hyperventilation (HV) beendet, folgt ein steiler, sofortiger Druckanstieg. Eine HV bei fortgeschrittenem ödem (3-4 h nach Trauma, VP um 50 mmHg) senkt alle Hirndrucke einige Minuten um wenige mmHg mit anschließend unbeeinflußtem Verlauf. Die HV führt konstant zu einer Senkung des aP und zu einer Steigerung des cvP.Das EEG wird nur bei der frühzeitigen HV und nur über der linken Hemisphäre deutlich gebessert. Es verschlechtert sich jedoch noch wäh-rend der HV rasch und rechts betont. Eine HV bei fortgeschrittenem ödem und hohem intrakraniellen Druck (ICP) dagegen führt zu einer sofortigen, rechts betonten Verschlechterung der EEGPower-Intensität.Tham senkt auch bei erheblichem ödem den ICP rasch und deutlich. Durch besonders ausgeprägte Wirkung in der geschädigten rechten Hemisphäre werden die Druckdifferenzen zwischen rEP und lEP aufgehoben. Während der Therapie bleiben die Drucke niedrig, nach Infusionsende steigt der ICP jedoch rasch wieder an. Über der rechten Hemisphäre bleibt dabei der Druck relativ niedriger als links. Der arterielle Blutdruck steigt gering, Herzfrequenz und Serumosmolalität nehmen zu.Tham bessert die EEG-Power-Intensität in allen Frequenzbanden deutlich und über beiden Seiten. Diese EEG-Besserung überdauert das Infusionsende 1-2 h. Die Erholung im a-Frequenzband ist besonders ausgeprägt und lang anhaltend.Die Wirkung von HV und Tham wird im Vergleich zu anderen hirndrucksenkenden Maßnah-men diskutiert. Die HV führt nur bei lokalisiertem Hirnödem durch Vasokonstriktion neben einer Hirndrucksenkung zu einer Zirkulations-und EEG-Verbesserung im Ödemareal. A new miniaturized epidural pressure transducing system for monitoring intracranial pressure (ICP) has been developed. The system is of the as yet smallest known dimensions. The transducer is implanted automatically coplanar to the dura via a burr hole of 4 mm diameter in the conscious patient without special surgical equipment. Application is possible in adults, children and infants. We have achieved a maximal resolution of 10(-3) mm of Hg. At a full scale range of 100 mm of Hg the zero-point drift is less than 1 mm of Hg/24 h.
