Introduction: Viral hepatitis is a major public health concern in Brazil. There are few past studies on this issue, especially among riparian communities. This study aims at determining the seroprevalence of viral hepatitis B and C in the riparian community of Pacuí Island, within the Cametá municipality of Pará State, Brazil. Moreover, this study aims to investigate the principal risk factors that this community is exposed to. Methods: The current study has accessed blood samples from 181 volunteers who have answered an epidemiological questionnaire. Analyses on serological markers have been tested with commercial ELISA kits for detecting HBsAg, total anti-HBc, anti-HBs, and anti-HCV. Within seroreactive patients for HCV, RT-PCR and line probe assay have been performed to identify the viral genotype. Results: In the serological marker analysis for hepatitis B, no reactivity for HBsAg, rate of 1.1% for total anti-HBc, and rate of 19.3% for anti-HBs have been observed. On hepatitis C, 8.8% seroprevalence has been found, in which 62.5% have gotten viral RNA. Among the risk factors studied, the following have been highlighted: non-use of condoms, sharing of cutting instruments, use of illicit drugs, and reports of family disease with HBV or HCV. Conclusions: The vaccination coverage against HBV is low, and the high prevalence of HCV within this community has been observed.
Epidemiologically, the relevance of infection caused by hepatitis viruses is related mainly to their wide geographic distribution and the large number of infected individuals in all parts of the world. In this study, 668 residents from the islands around the Tucuruí Dam were selected. Blood samples were collected for investigation of serological markers (HBsAg, total anti-HBc, anti-HBS, and anti-HCV) by enzyme immunoassays. HCV-positive subjects were tested using RT-PCR and RFLP for the identification of viral genotypes. Among the 668 subjects studied, 1.9% were HBsAg positive, 28% were total anti-HBc positive, and 41.9% were anti-HBs positive. The anti-HBs marker alone (vaccine response) was detected in 25.7% of the volunteers. Anti-HCV antibody was detected in 2.2% of the subjects and genotype 1 was the predominant genotype (70%). The results indicate an intermediate level of HBV and HCV endemicity in the region studied, as well as low HBV vaccination coverage.
Alcoholic liver disease (ALD) is a highly prevalent spectrum of pathologies caused by alcohol overconsumption. Morbidity and mortality related to ALD are increasing worldwide, thereby demanding strategies for early diagnosis and detection of ALD predisposition. A potential candidate as a marker for ALD susceptibility is the transcription factor nuclear factor erythroid-related factor 2 (Nrf2), codified by the nuclear factor erythroid 2-related factor 2 gene (NFE2L2). Nrf2 regulates expression of proteins that protect against oxidative stress and inflammation caused by alcohol overconsumption. Here, we assessed genetic variants of NFE2L2 for association with ALD. Specimens from patients diagnosed with cirrhosis caused by ALD were genotyped for three NFE2L2 single nucleotide polymorphisms (SNP) (SNPs: rs35652124, rs4893819, and rs6721961). Hematoxylin & eosin and immunohistochemistry were performed to determine the inflammatory score and Nrf2 expression, respectively. SNPs rs4893819 and rs6721961 were not specifically associated with ALD, but analysis of SNP rs35652124 suggested that this polymorphism predisposes to ALD. Furthermore, SNP rs35652124 was associated with a lower level of Nrf2 expression. Moreover, liver samples from ALD patients with this polymorphism displayed more severe inflammatory activity. Together, these findings provide evidence that the SNP rs35652124 variation in the Nrf2-encoding gene NFE2L2 is a potential genetic marker for susceptibility to ALD.
The present results suggest a possible role of the -607 IL-18 gene promoter polymorphism in the pathogenesis of hepatitis C virus infection.
Introduction:The genomic heterogeneity of hepatitis C virus (HCV) influences liver disorders. This study aimed to determine the prevalence of HCV genotypes and to investigate the influence of these genotypes on disease progression. Methods: Blood samples and liver biopsies were collected from HCV-seropositive patients for serological analysis, biochemical marker measurements, HCV genotyping and histopathological evaluation. Results: Hepatitis C virus-ribonucleic acid (HCV-RNA) was detected in 107 patients (90.6% with genotype 1 and 9.4% with genotype 3). Patients infected with genotype 1 exhibited higher mean necroinflammatory activity and fibrosis. Conclusions: HCV genotype 1 was the most prevalent and was associated with greater liver dysfunction.Keywords: Hepatitis C virus. Genotype. Histopathology.Hepatitis C virus (HCV) infection is often asymptomatic, but it is chronic in a large (85%) proportion of cases. Approximately 20% of individuals with chronic HCV develop liver damage, cirrhosis or cancer 1 .In Brazil, the seroprevalence of HCV is moderate, although rates vary among different regions of the country 2 . Genotype 1 is predominant, followed by genotypes 3 and 2 3 . In the State of Pará, the overall prevalence ranges from 0.5 to 2% among blood donor candidates 4 , and the genotype distribution is similar to that reported elsewhere in Brazil 5 .Hepatitis C virus genotype 1 has been associated with severe liver damage and the response to treatment 6 . However, other studies were unable to confirm this association 7 . Brazilian studies correlating the different HCV genotypes with histological and clinical presentation are scarce and have not confirmed an association between liver disease severity and HCV genotype 8,9 .The objectives of this study were to determine the prevalence of HCV genotypes in patients with chronic hepatitis C in the State of Pará, Brazil and to investigate the influence of these genotypes on the biochemical and histopathological parameters of the disease.
Introdução: A hanseníase é uma doença infecciosa causada pelo Mycobacterium leprae, enfermidade crônica, de acometimento dermatoneurológico, evolução lenta, na ausência de tratamento pode ser incapacitante e deformante. No início das epidemias, e sem tratamento comprovado, foram criados leprosários como medida profilática, que no Estado do Pará, Lazarópolis do Prata era referência. Objetivos: O objetivo da pesquisa foi comparar a terapia sequencial aplicada no antigo Lazarópolis do Prata, desde a utilização de derivados chaulmúrgicos à terapêutica atual, em relação ao progresso do tratamento nacional. Métodos: O estudo analítico retrospectivo utilizou dados quantitativos do leprosário de 1923 a 2017, correspondente ao período de admissão de pacientes. Resultados e discussão: Nos períodos avaliados, o gênero masculino teve maior prevalência de admissão (65,7%). Entre 1935 a 2015, houve crescimento médio de 5 casos a cada 5 anos, cujas formas mais prevalentes apresentaram picos significantes de 1971 a 1975, representado pelo tipo virchowiana e 2011 a 2015 pelo aumento dos casos do tipo tuberculoide. Conclusões: A hanseníase ainda persiste como doença negligenciada e problema de saúde pública, porém, houve uma considerável mudança no perfil epidemiológico através das estratégias de medidas profiláticas e modulações no esquema terapêutico, como observado na Colônia do Prata.
Introdução: O uso de plantas com finalidades terapêuticas já faz parte da medicina popular há muitos anos, devido à esse conhecimento e o avanço da ciência tornou-se possível utiliza-las com eficácia e segurança em tratamentos de diversas enfermidades1. Em 2020, o mundo mergulhou em uma crise sanitária nunca vista. A COVID-19 impôs à humanidade uma maior atenção com a saúde mental e física. Segundo Pereira2, a pandemia da COVID-19 desencadeou um aumento na incidência de alterações comportamentais associadas ao medo, angustia, estresse e ansiedade, tais danos que podem se estender até mesmo após o controle da pandemia. Nesse contexto, a procura por terapias alternativas, como ansiolíticos de origem natural com a finalidade de amenizar ou tratar os transtornos causados pelo impacto da pandemia, ganhou notoriedade da população, uma vez que tem se mostrado um método eficaz e de baixo custo3. Objetivo: Realizar um levantamento no perfil de comercialização dos fitoterápicos de ação ansiolítica mais vendidos em duas redes de farmácias de manipulação de Belém do Pará, durante o período da pandemia da COVID-19. Metodologia: Estudo do tipo descritivo com abordagem quantitativa e qualitativa, sobre os fitoterápicos de ação ansiolítica mais consumidos nessas farmácias de manipulação, nos anos de 2020 e 2021, durante o período da pandemia pela COVID-19 e compará-los ao período que antecedeu a pandemia, nos anos de 2018 e 2019. Resultados: Pode-se observar que os anos referentes ao período da pandemia (2020-2021), obtiveram um aumento significativo de vendas (p < 0.0001), totalizado 4.869 fitoterápicos vendidos (56%), comparado ao período que antecedeu a pandemia (2018/2019), representando 3.831 fitoterápicos vendidos (44%), além disso, observou-se que dentre os três fitoterápicos de ação ansiolítica mais vendidos nas redes de farmácias magistrais utilizadas como fontes da pesquisa, a Passiflora incarnata foi significativamente o mais consumido (p = 0.0002), com 4.769 formulações (54,8%). Conclusão: A partir do estudo proposto, verificou-se como pandemia da COVID- 19 refletiu diretamente na saúde mental da população e nas possíveis elevações de casos de ansiedade. Evidenciado que o consumo de fitoterápicos de ação ansiolítica foi utilizado como alternativa viável para o tratamento desses transtornos.
Objetivo: Analisar como o descarte domiciliar de medicamentos é realizado na cidade de Belém e o grau de conhecimento da população quanto aos riscos. Métodos: Foi realizado um estudo do tipo transversal análitico, qualitativo e quantitativo, através da aplicação de uma abordagem investigativa quanto ao conhecimento da população acerca dos riscos de um descarte inadequado de medicamentos, seus conhecimentos sobre leis vigentes, bem como a influência da automedicação no acúmulo de resíduos farmacêuticos. O estudo contou com a aplicação de entrevistas em caráter socioepidemiológica, em usuários de medicamentos, de forma aleatória, em locais públicos de grande fluxo nas regiões metropolitana e periféricas da cidade de Belém-Pará. O presente estudo foi apreciado pelo comitê de ética em pesquisa. Resultados: Os resultados mostraram que 98% dos entrevistados possuem fármacos em suas residências, 83,45% desconhecem os reais riscos no descarte incorreto e 91,18%, destes realizam o descarte de forma inadequada (p <0.0001), desprezando principalmente no lixo comum (89,9%). Conclusão: Observamos uma falta de conhecimento por parte da população de Belém e região metropolitana em relação ao descarte de medicamentos vencidos e/ou não utilizados, sendo necessário conscientização e sensibilização dos indivíduos através de uma educação ambiental.
