Summary. Background: Presence of bleeding symptoms, inheritance and reduced von Willebrand factor (VWF) contribute to the diagnosis of type 1 von Willebrand disease (VWD). However, quantitative analysis of the importance of VWF antigen (VWF:Ag) and ristocetin cofactor activity (VWF:RCo) levels in the diagnosis is lacking. Objectives: To evaluate the relative contribution of VWF measurement to the diagnosis of VWD. Patients and methods: From the MCMDM-1VWD study cohort, 204 subjects (considered as affected by VWD based on the enrolling Center diagnoses and the presence of linkage with the VWF locus) were compared with 1155 normal individuals. Sensitivity, specificity and diagnostic positive likelihood ratios (LR) of VWF:Ag and VWF:RCo were computed. Results: ABO blood group was the variable most influencing VWF levels, but adjustment of the lower reference limit for the ABO group did not improve sensitivity and specificity of VWF:Ag or VWF:RCo. The lower reference limit (2.5th percentile) was 47 IU dL ) of VWF only marginally indicated the probability of VWD. Conclusions: Although the conventional 2.5 lower percentile has good sensitivity and specificity, only VWF:Ag or VWF:RCo values below 40 IU dL )1 appear to significantly indicate the likelihood of type 1 VWD. The LR profile of VWF level could be used in a diagnostic algorithm.
This is a qualitative study that aims to understand the feelings of relatives of patients admitted to the Intensive Care Unit (ICU). The study was conducted in the ICU of a large general hospital in the western region of Santa Catarina. The data collection occurred in 2009 with a semi structured interview to eighteen families. For data treatment the collective subject discourse was used. Reports emerged of two items related to feelings: hospitalization in the ICU and while waiting to enter the unit. The analysis revealed feelings as pain, anguish, sadness, helplessness,fear, despair, anxiety and expectation infinite. It is hoped that these results may assist in the training of professionals, to host the family and its insertion in the ICU environment as an element to be integrated into nursing care, through actions welcoming, helping them to cope with hospitalization of a relative in a critical unit.
Este artigo teve como objetivo identificar os cuidados de enfermagem necessários para construir um protocolo de atendimento a pacientes adultos traumatizados, aerorremovidos, específico aos períodos durante e após o voo, baseado nos princípios do Prehospital Trauma Life Support. Trata-se de uma pesquisa qualitativa convergente-assistencial, realizada na Divisão de Operações Aéreas em São José, Santa Catarina, no período de abril a junho de 2010. Para a construção do protocolo foram utilizadas entrevista individual e encontros em grupo. Participaram oito enfermeiros que atuam no serviço. O protocolo contempla cuidados relativos à segurança em voo, avaliação constante do paciente durante a remoção e na entrega ao destino final. O protocolo propiciará uma prática de cuidado mais segura aos pacientes aerorremovidos. Sugere-se outros estudos voltados para a atuação do enfermeiro, como a criação de novos protocolos que orientem a assistência no ambiente aeroespacial.
RESUmoTrata-se de uma pesquisa qualitativa, realizada na Unidade de Terapia Intensiva (UTI) adulto de um hospital público no Sul do Brasil, que teve como objetivo avaliar as estratégias de acolhimento implementadas. Participaram 13 pacientes e 23 familiares. A coleta foi realizada de julho a outubro de 2008, com entrevistas semiestruturadas e gravadas. Para análise dos dados, utilizou--se o Discurso do Sujeito Coletivo. As informações deram origem a dois discursos: a família percebe o acolhimento e o paciente considera a equipe da UTI atenciosa. Ao incluir a família no cuidado como cliente da enfermagem, os familiares sentiram-se seguros e confiantes. Ao avaliar os resultados alcançados, destaca-se que, ao assumirem o compromisso e a responsabilidade de transformações da prática assistencial, os enfermeiros experienciaram um novo olhar para o cuidado em UTI, com enfoque no ser humano, aliando o acolhimento ao modelo assistencial que privilegia a objetividade do cuidado.

ABSTRACT Objective: to validate the content of a Standard Operational Procedure, regarding nursing interventions in emergency patients treated with vasoactive drugs. Method: methodological study, carried out from December 2015 to January 2016. Content validity was determined by 16 experts (13 nurses, 2 pharmacists and one biomedical scientist), who judged it through a four-point Likert scale. The items were evaluated in terms of: feasibility, objectivity, simplicity, clearness, pertinence and accuracy. The Content Validity Index was applied, accepting the value ≥ 0.90. Results: two rounds of evaluation were required to achieve the minimum index. The items were reviewed regarding writing, dilution, indication and replacement interval. The instrument was validated with 33 items and with total Content Validity Index of 0.99. Conclusion: the instrument was considered appropriate, fractionally and globally, for nursing care for the patient treated with vasoactive drugs in emergency, in a safe and reliable way.

A qualitative convergent care research was performed at the Division of Air Operations in the municipality of San Jose-SC, Brazil, from April to June 2010. The study was based on the principles of the Pre-hospital Trauma Life Support. Individual interviews and group meetings were used. Eight nurses participated in the service. The proposed protocol includes safety boarding, disembarking, assessment of the scene, primary assessment, secondary assessment and stabilization of the patient.
