Investigations indicate a potential link between exposure to extremely low-frequency electromagnetic field (ELF-EMF) and some cancers. Carcinogenesis of ELF-EMF may be mediated by effect on the immune system. During an immune response, naïve T cells differentiate to effector type 1 helper T cells (T(H)1), T(H)2, or T(H)17 subsets according to existence of different cytokines and T(H)1 is important in defense against tumors. Therefore, it will be reasonable to test whether ELF-EMF can change cytokines like interferon gamma (IFN-γ), interleukin-4 (IL-4), IL-6, and IL-12 that regulate T(H)1/T(H)2/T(H)17 balance. Forty adult male rats were randomly separated into ELF-EMF-exposed and sham-exposed control groups. The ELF-EMF group was exposed to a flux density of 100 μT, frequency 50 Hz, 2 h/day for 3 months. The controls were placed in identical chamber without ELF-EMF. The results showed there were no significant differences between the mean mass of rats, thymuses, and spleens in ELF-EMF exposed group compared with controls. Serum IL-12 level was decreased from 418 ± 47 pg/ml in controls to 300 ± 23 pg/ml (p < 0.05) in ELF-EMF-exposed group. Phytohemagglutinin activated of in vitro production of IL-6 by the whole spleen culture (1356 ± 92 pg/ml) and total blood culture (418 ± 40 pg/ml) of ELF-EMF-exposed rats were higher (p < 0.001) comparing with controls (905 ± 74 pg/ml), (182 ± 26 pg/ml), respectively. However, the levels of IFN-γ, IL-4, and IL-6 of serum and IFN-γ, IL-4, and IL-12 in spleen culture and total blood culture of two groups were not significantly different. It seems that ELF-EMF may change T(H)1/T(H)2/T(H)17 balance toward down regulation of T(H)1 and upregulation T(H)17 type responses.
Introduction
Noise pollution in the operating rooms is one of the remaining challenges. Both patients and physicians are exposed to different sound levels during the operative cases, many of which can last for hours. This study aims to evaluate the noise pollution in the operating rooms during different surgical procedures.
Materials and methods
In this cross-sectional study, sound level in the operating rooms of Hamadan University-affiliated hospitals (totally 10) in Iran during different surgical procedures was measured using B&K sound meter. The gathered data were compared with national and international standards. Statistical analysis was performed using descriptive statistics and one-way ANOVA, t-test, and Pearson’s correlation test.
Results
Noise pollution level at majority of surgical procedures is higher than national and international documented standards. The highest level of noise pollution is related to orthopedic procedures, and the lowest one related to laparoscopic and heart surgery procedures. The highest and lowest registered sound level during the operation was 93 and 55 dB, respectively. Sound level generated by equipments (69 ± 4.1 dB), trolley movement (66 ± 2.3 dB), and personnel conversations (64 ± 3.9 dB) are the main sources of noise.
Conclusion
The noise pollution of operating rooms are higher than available standards. The procedure needs to be corrected for achieving the proper conditions.
The general population, everywhere in the world is exposed to a small dose of ionising radiation from natural sources. Stochastic effects such as cancer and genetic disorders are caused when living creatures are exposed to low doses. In Iran, it is measured in some cities, especially in high-background areas such as Ramsar, but so far there is no measurement in the Hamadan province. Hamadan is located in the west of Iran. Measurements were performed using a RDS-110 survey meter, CaSO(4):Dy thermoluminecense dosimetries (TLDs) and a Harshaw 4000 TLD reader. To estimate the dose rate outdoors, four stations along the main directions (north, south, west and east) and one in the town centre were selected. Mean annual X and gamma equivalent dose in Hamadan province are 1.12±0.22 and 1.66±0.07 mSv, which related to RDS-110 survey meter and TLDs measurements, respectively. The TLDs and RDS-110 results are representative of the external photon radiation doses for the selected monitoring locations and for those locations for the hours during which the measurements were taken, respectively. Maximum and minimum of external photon radiation doses are related to Hamadan and Kaboudar-Ahang towns, respectively. According to the results of the study, it seems that the annual X and gamma equivalent dose in Hamadan province exceeded the global mean external exposure amounts by the UNSCEAR, and further studies are needed to measure internal exposures to determine the total environmental radiation level in Hamadan province.
