BackgroundCommunity-based primary mental health care is recommended in low and middle-income countries. The Brazilian Health System has been restructuring primary care by expanding its Family Health Strategy. Due to mental health problems, psychosocial vulnerability and accessibility, Matrix Support teams are being set up to broaden the professional scope of primary care. This paper aims to analyse the perceptions of health professionals and managers about the integration of primary care and mental health.MethodIn this mixed-method study 18 health managers and 24 professionals were interviewed from different primary and mental health care services in Rio de Janeiro. A semi-structured survey was conducted with 185 closed questions ranging from 1 to 5 and one open-ended question, to evaluate: access, gateway, trust, family focus, primary mental health interventions, mental health records, mental health problems, team collaboration, integration with community resources and primary mental health education. Two comparisons were made: health managers and professionals’ (Mann-Whitney non-parametric test) and health managers’ perceptions (Kruskall-Wallis non parametric-test) in 4 service designs (General Traditional Outpatients, Mental Health Specialised Outpatients, Psychosocial Community Centre and Family Health Strategy)(SPSS version 17.0). Qualitative data were subjected to Framework Analysis.ResultsFirstly, health managers and professionals’ perceptions converged in all components, except the health record system. Secondly, managers’ perceptions in traditional services contrasted with managers’ perceptions in community-based services in components such as mental health interventions and team collaboration, and converged in gateway, trust, record system and primary mental health education. Qualitative data revealed an acceptance of mental health and primary care integration, but a lack of communication between institutions. The Mixed Method demonstrated that interviewees consider mental health and primary care integration as a requirement of the system, while their perceptions and the model of work produced by the institutional culture are inextricably linked.ConclusionThere is a gap between health managers’ and professionals’ understanding of community-based primary mental health care. A lacuna de cuidados em Saúde Mental (SM) é um problema mundial. A Atenção Primária à Saúde (APS) é a base para estruturação da rede assistencial integral que o solucione. Sua integração com a Psiquiatria é parte deste desafio. Partindo da pergunta norteadora "Como vem sendo desenvolvida a inserção da APS no cuidado em Saúde Mental através da integração com a Psiquiatria pelo Matriciamento?", este artigo analisa a participação da Psiquiatria na organização da assistência em saúde mental centrada na APS, dentro de um modelo integral de cuidado em saúde. Foi utilizada metodologia qualitativa baseada na problematização da integração do Psiquiatra com as equipes da Estratégia de Saúde da Família no processo de matriciamento, sistematizam-se as experiências e conhecimento desenvolvidos pelos autores nos projetos do Laboratório Interdisciplinar de Pesquisa em APS da Universidade do Estado do Rio de Janeiro. A partir dos referenciais conceituais dos Cuidados Colaborativos, Apoio Matricial, Matriciamento e Cuidado Integral, e da caracterização das demandas clínicas de SM mais frequentes na APS, discutem-se as repercussões destes na estruturação do cuidado em SM, na formação médica e na transformação da Psiquiatria. Conclui-se que a APS é um novo campo de atuação da Psiquiatria trazendo desafios importantes para a formação e a assistência em SM.
Objetivo: Analisar artigos publicados entre 2000 e 2010 que versam sobre a articulação entre saúde mental e Atenção Primária no Brasil, na perspectiva da prática do matriciamento. Método: Revisão em banco de dados virtual, no período referido. Os descritores foram: “saúde mental”, “atenção primária”, “atenção básica”, “apoio matricial” e “matriciamento” e as categorias de análise foram construídas a partir da leitura dos artigos. Resultados: Foram selecionados 86 artigos, divididos em 3 categorias: a articulação analisada a partir da saúde mental, a partir da Atenção Primária e experiências de matriciamento. A análise evidenciou a existência de diferentes formas de descrever o trabalho em colaboração. Conclusões: A experiência brasileira está em consonância com as recomendações mundiais rumo à diminuição da lacuna terapêutica entre a oferta e as necessidades de cuidado. A construção da Rede de Saúde Mental integrada à Estratégia Saúde da Família é um novo desafio para a reforma psiquiátrica brasileira.
Autores da Saúde Mental Global defendem, de um lado, o aumento do acesso aos cuidados de saúde mental, inclusive com o uso de intervenções padronizadas. De outro lado, na Atenção Primária à Saúde no Brasil, a apresentação sintomatológica do sofrimento emocional pelos pacientes dificulta sua identificação pelos profissionais, comprometendo o cuidado e exigindo uma abordagem culturalmente sensível. Objetivando conhecer a percepção de pacientes sobre o sofrimento emocional e seu cuidado na Atenção Primária, adotou-se método qualitativo de abordagem narrativa, com coleta de dados em grupos em salas de espera e roteiro baseado no instrumento McGill Ilness Narrative Interview. As narrativas foram tratadas por Análise de Conteúdo e revelaram que limitar a comunicação do sofrimento é negar o acesso ao cuidado. Contrapondo tal limitação, apontaram-se estratégias para nortear a estruturação de um cuidado que seja integral e culturalmente sensível.
Introduction:In Brazil, the public health system is universal and regionalized. Currently, it is being restructured starting from the primary care system.In Rio de Janeiro there is a lack of children and adolescents mental health professionals, and there is a deficient system to attend the demand, without integration between health and educational systems and insufficient and inadequate psychology, pedagogy, speech therapy, occupational therapy and medical care.The city of Rio de Janeiro is divided in 10 Programmatic Areas (PA). The Child and Adolescent psychiatry of Piquet Carneiro Policlinic (PPC)/ UERJ covers, amongst three more units, the PA 2.2 of 371.120 habitants, amid those, 74.257 are under 15 years old (IBGE, 2010).Objectives:Analyze the Child and Adolescent Psychiatric demand in PPC/ UERJ in 2013, arising from the primary care system.Methodology:A sample of 50 patients that were evaluated in this policlinic referred from the primary care system were attended and directed through consultations. Those evaluations were made by psychiatrists and reports and counter-referrals were fulfilled. The patients in need of psychiatric care are being accompanied.Results:Out of the 50 patients, we analyzed gender, age, diagnosis, familial background, social-economical background, educational achievement and prescribed drugs.Conclusions:Among the analyzed patients, we found that the majority of the cases regard learning disabilities and family issues that could have better prognosis with an integrated and multidisciplinary care.The existing units covering Child and Adolescent Mental Health in the referred PA are not enough to provide the necessary care.
