KIM-1 (kidney injury molecule 1) is a transmembrane glycoprotein also known as HAVcr-1 and TIM-1 belongs to the T-cell immunoglobulin and mucin domain family (TIM) of proteins. TIM glycoproteins are presented on the immune cells and participate in the regulation of immune reactions. KIM-1 differs from other members of its family in that it is expressed not only by immunocompetent cells but epithelial cells as well. Cellular and humoral effects mediated by KIM-1 are involved in a variety of physiological and pathophysiological processes.Current understanding of the mechanisms determining the participation of KIM-1 in viral invasion, the immune response regulation, adaptive reactions of the kidney epithelium to acute ischemic or toxic injury, in progression of chronic renal diseases, and kidney cancer development have been presented in this review. Data of clinical researches demonstrating the association of KIM-1 with viral diseases and immune disorders have also been analyzed. Potential application of KIM-1 as urinary or serological marker in renal and cardiovascular diseases has been considered.

В данном обзоре проведен анализ результатов исследования традиционных серологических опухолеассоциированных маркеров и нового потенциального опухолеассоциированного маркера почечно-клеточного рака -молекулы повреждения почек 1 (kidney injury molecule-1, KIM-1). Описаны структура, источники и функции KIM-1 в норме и при повреждении почечных канальцев, ее возможная роль в канцерогенезе. Проанализирован опыт использования KIM-1 в уточняющей диагностике наиболее распространенных гистологических типов почечно-клеточного рака. Представлены данные, касающиеся экспрессии KIM-1 при злокачественных новообразованиях других локализаций и при заболеваниях почек неонкологического генеза. Показана роль KIM-1 в ранней диагностике нефротоксичного действия противоопухолевых препаратов. Накопленные данные открывают перспективы использования KIM-1 в клинической онкологии как уринологического и серологического маркера почечно-клеточного рака и нефротоксичности химиотерапии. Онкоурология 2019;15(3):132-42.The last decades are characterized by an active search for highly sensitive and specific urinological and serological tumor-associated markers of renal cell carcinoma. This review analyses the results of studies of traditional serological tumor-associated markers and a potential new tumor-associated marker of renal cell carcinoma: kidney injury molecule-1, or KIM-1. The structure, sources and functions of KIM-1 in normal conditions and in damaged renal tubules, its potential role in carcinogenesis are described. The experience of using KIM-1 for specifying diagnosis of the most common histological types of renal cell carcinoma is analyzed. Data on KIM-1 expression in malignant tumors in other locations and non-oncological kidney disorders are presented. The role of KIM-1 in early diagnosis of nephrotoxic effect of antitumor drugs is described. The accumulated data is promising in regards to using KIM-1 in clinical oncology as a urinological and serological marker of renal cell carcinoma and chemotherapy nephrotoxicity. 55Почечно-клеточный рак (ПКР) занимает одно из ве-дущих мест в мире по заболеваемости среди злокачествен-ных новообразований мочеполовой системы, в 2012 г. за-регистрировано более 337 000 первичных больных ПКР и 143 369 пациентов умерли от этого заболевания [1].В России в 2013 г. зарегистрировано 20 892 новых слу-чаев заболевания ПКР. По темпам прироста онкологиче-ской заболеваемости за последние 10 лет ПКР устойчиво занимает одно из ведущих мест (29,14%). Абсолютное число умерших от ПКР в России в 2013 г. составило 8459 человек, причем в последние 2 года впервые отмечено снижение смертности на 4,25%, которое обусловлено ран-ней диагностикой и лечением заболевания [2].Частота встречаемости билатерального ПКР, по данным разных авторов, составляет 2-6% от общей по-пуляции больных ПКР. Различают синхронный и мета-хронный двусторонний ПКР, который может быть на-следственным или спорадическим. У больных с наслед-ственной формой ПКР имеется семейный анамнез ПКР либо синдром Липпеля-Линдау. ПКР у этих больных чаще развивается в более молодом возрасте. Согласно базе данных SEER (Surveillance, Epidemiology, and End Results), почечно-клеточный рак (пкр) занимает одно из ведущих мест в мире по заболеваемости среди злокачественных ново-образований мочеполовой системы. частота встречаемости билатерального пкр, по данным разных авторов, составляет 2-6% общей популяции больных пкр. единственным эффективным методом лечения билатерального пкр является хи-рургическое лечение. Больные билатеральным пкр относятся к группе высокого риска развития местного рецидива или прогрессирования заболевания после органосохраняющих операций, в связи с чем перед хирургом стоит выбор между высоким риском развития почечной недостаточности или рецидива и/или прогрессирования заболевания в зависимости от объема хирургического вмешательства. У больных билатеральным пкр отмечено повышение частоты выявления па-пиллярного варианта пкр до 19% и наличие мультифокального поражения. Хирургическое лечение двустороннего пкр является единственным эффективным методом, позволяющим добиться удовлетворительных онкологических результатов при низкой частоте осложнений. Наиболее оправданным вариантом лечения билатерального пкр является выполнение двустороннего органосохраняющего лечения, позволяющего добиться оптимальных функциональных результатов. при-веден клинический случай длительного успешного хирургического лечения больного билатеральным пкр.
