Fatal myocardial failure secondary to pulmonary hypertension is reported in three infants, from 24 days to 4 months old. The three cases showed persistent tachycardia and marked leukocytosis (> 40.000 leukocytes/mm 3), one infant reached hyperleukocytosis values (120.000 leukocytes/mm 3). All three died, 24 to 56 hours after admission to PICU. Treatment consisted in mechanical ventilation, inotropic support and antibiotics in all of them, high frequency oscillatory ventilation (HFOV) in two and inhaled nitric oxide in one case. Necropsy performed in two infants revealed signs of pulmonary hypertension. We recommend to keep in mind this not very well known and often fatal complication in infants with a Bordetella pertussis infection.
16. Resultados: Se presentan un total de 31 episodios de empleo de VAFO en 29 pacientes cuyo diagnóstico fue SDRA en 90%, con edad de 5 meses (1 día a 104 meses) y peso de 6 kilogramos (2,8 a 60 kg). La duración de la VMC previo a la VAFO fue de 33 horas (0-740). El IO al momento de iniciarse la VAFO fue de 27. La presión media vía aérea (PMVA) fue fijada en promedio 10 cmH2O sobre el valor en VMC. La duración promedio de la VAFO fue de 85 horas por paciente. Tanto en los pacientes vivos como fallecidos se observó un descenso del índice de oxigenación (IO) durante las primeras 48 horas, sin una diferencia estadística entre ambos grupos. El efecto adverso más frecuente fue la hipotensión transitoria (13 episodios). Diez pacientes fallecieron, 7 por causa pulmonar, dando una letalidad por este motivo de un 24% (7/29).
