www.medigraphic.org.mx RESUMEN. Introducción: La enfermedad pulmonar obstructiva crónica (EPOC) es una de las principales causas de muerte en México y en el mundo. Pese a que en los últimos años se ha alcanzado un gran progreso en su manejo y tratamiento, esto también ha conllevado una mayor complejidad. En México, es fundamental el desarrollo de guías de práctica clínica (GPC) que estén basadas en estándares internacionales para establecer recomendaciones a todos los niveles de atención. Objetivos: El objetivo fue desarrollar una GPC con recomendaciones clínicas desarrolladas de forma sistematizada para asistir a la toma de decisiones, tanto de médicos de todos los niveles de atención, como de pacientes, cuidadores de pacientes y todos los elaboradores de políticas públicas involucrados en el manejo de pacientes con enfermedad pulmonar obstructiva crónica. Material y métodos: Este documento fue desarrollado por parte de la Sociedad Mexicana de Neumología y Cirugía de Tórax en cumplimiento con los estándares internacionales como los descritos por el Instituto de Medicina de los Estados Unidos (IOM, por sus siglas en inglés). Se integró un Grupo de Desarrollo de la Guía (GDG) de manera interdisciplinaria, considerando la participación de neumólogos y metodólogos con experiencia en revisiones sistemáticas de la literatura y el desarrollo de GPC. De forma sistematizada, se consensuaron los alcances, las preguntas clínicas y se evaluaron otras Guías Internacionales sobre el tema. Se evaluó críticamente la calidad metodológica de esas GPC. A través del abordaje ADAPTE, se transculturizó la GPC a nuestro país y, finalmente, utilizando metodología de consenso formal de expertos, se desarrollaron las recomendaciones clínicas. Resultados: Se consensuaron 40 preguntas estructuradas clínicamente relevantes para el diagnóstico y tratamiento, así como las recomendaciones clínicas pertinentes y su texto de soporte. Las recomendaciones abarcan preguntas sobre diagnóstico y tratamiento. Conclusión: Esta GPC pretende proveer recomendaciones clínicas para el diagnóstico y manejo terapéutico de los distintos estadios de la EPOC basadas en evidencia científica, que asisten en el proceso de toma de decisiones compartidas y que esperamos contribuyan a mejorar la calidad de la atención clínica en las pacientes con EPOC. Introducción: El síndrome hepatopulmonar (SHP) es una de las manifestaciones extrahepáticas menos frecuentes de la insuficiencia hepática. Tiene una incidencia de 13 a 47%, con una sobrevida de 40% a 2.5 años. Objetivo: Describir el diagnóstico y la evolución clínica del SHP en pacientes mexicanos. Material y métodos: Estudio descriptivo, observacional, longitudinal y ambilectivo. Se estudiaron 94 pacientes de la Clínica de Hígado, estableciendo como criterios diagnósticos para SHP: datos clínicos sugestivos como tos no asociada a procesos infecciosos, dolor torácico, disnea, ortodeoxia y/o platipnea, gradiente alvéolo-arterial de oxígeno mayor de 15 mmHg, dilataciones vasculares intrapulmonares evaluado mediante ecocardiografía de contraste y pruebas de función respiratoria por el Departamento de Neumología del Hospital de Especialidades 5 de Mayo, ISSSTEP, Puebla, México, en el período de marzo a agosto de 2014. Resultados: Se determinó la presencia de SHP en 4 pacientes (4.2%), la etiología asociada más prevalente fue por enfermedad hepática no alcohólica, 35.1%; asociada a alcohol, 24.4%; viral, 15.9%; y otras, 24.5%. Los síntomas más frecuentes asociados a SHP fueron: disnea, dolor torácico y tos crónica. Cien por ciento de los pacientes (4 pacientes) presentaban hipoxemia en diferentes grados. La severidad de la enfermedad catalogada por Child Pugh no determina la aparición de los síntomas. Conclusión: El SHP se produjo en 4.2% de los pacientes, presentaron hipoxemia y elevación del gradiente alvéolo-arterial. La severidad de la enfermedad hepática no determina el desarrollo de la enfermedad. Su detección oportuna disminuye la mortalidad de los pacientes.

Introducción: El binomio asma/EPOC denominado SOAE (sobreposición asma/EPOC) es un subconjunto de pacientes con persistencia y limitación del flujo aéreo que presenta características clínicas tanto del asma como de la EPOC. Los estudios iniciales han demostrado que omalizumab puede ser útil en pacientes con SOAE. Ha demostrado mejorar los síntomas, reducir las exacerbaciones y la hospitalización, así como mejorar parámetros de función pulmonar y requerimiento de esteroides reducido en estos pacientes. Objetivo: Describir el efecto de omalizumab en cinco pacientes con diagnóstico de síndrome de sobreposición asma/EPOC (SOAE) y administración de omalizumab. Describimos la experiencia de nuestro centro y los beneficios que el tratamiento ha dado a nuestros pacientes. Material y métodos: Se presentan 5 casos de pacientes con diagnóstico de síndrome de sobreposición asma/EPOC que tras la aplicación de omalizumab (300 mg SC cada mes) mejoraron significativamente sus síntomas y sus pruebas funcionales respiratorias, principalmente en la espirometría. Los 5 pacientes dieron su consentimiento de presentación de casos. Resultados: Los 5 pacientes con síndrome de sobreposición asma/ EPOC que recibieron omalizumab experimentaron mejoría de los síntomas con mejoría del Cuestionario del Control del Asma de 10 a 20 y 25 puntos, así como en la espirometría. Omalizumab fue bien tolerado sin presentar complicaciones. El 100% de los pacientes presentó mejoría de los síntomas en el Cuestionario del Control del Asma, el mayor porcentaje fueron mujeres igual a como lo describe la literatura, los pacientes toleraron adecuadamente el tratamiento sin presentar efectos adversos mayores. Conclusión: Los casos descritos ofrecen una evidencia inicial de nuestra práctica médica sobre los beneficios del omalizumab en los pacientes con síndrome de sobreposición asma/EPOC mejorando clínicamente. Omalizumab demuestra buena tolerancia sin presentar efectos adversos. Estos beneficios permiten al paciente una mejor calidad de vida y disminuir de manera radical su morbilidad.
