Introduction: The standard treatment of patients with metastatic squamous cell carcinoma at loco-regionally advanced stage is the association of radiation-chemotherapy; however, a high percentage of these persist and require rescue surgery, which implies significant morbidity. Initial neck dissection could play an important role in reducing this rate of complications. Objective: To know oncological results of patients with neck metastasis. Materials and methods: Retrospective analysis of patients undergoing treatment for cervical lymph node metastasis of squamous cell carcinoma. The therapeutic sequence was decided based on volume, resectability, site and size of the primary and general condition of the patient. Overall survival was calculated based on HPV status. Results: We included 30 patients with a mean age of 57 years and a mean follow-up of 5 years: 22 men and 8 women. All with loco-regionally advanced stages, 10 N3 and 20 N2. In 18, determination of p16 was made in the lymph node metastases, in 13 of them it was positive and in 5, negative. Seven patients were treated with initial chemotherapy and 23 underwent neck dissection; 5-year survival was 25% and the prognosis was better in patient with positive p16. Conclusion: The treatment of cervical metastasis depends on its resectability; initial surgery should be promoted. Neo-adjuvant chemotherapy is an alternative in patients with bulky, unresectable or borderline metastases, HPV is a factor of good prognosis.
Artículo origiNAl resumen introducción: El tratamiento estándar de los pacientes con carcinoma epidermoide metastásico a cuello en etapa locorregionalmente avanzada es la asociación de quimioterapia y radioterapia; sin embargo un porcentaje elevado de estos persisten y requieren cirugía de rescate, lo que implica morbilidad importante. La disección de cielo inicial podría tener un papel importante para disminuir esta tasa de complicaciones. Material y métodos: Análisis retrospectivo de pacientes sometidos a tratamiento por metástasis de carcinoma epidermoide a cuello; la secuencia terapéutica fue decidida con base en el volumen metastásico, resecabilidad, sitio y tamaño del tumor primario y estado general del paciente. Se calculó la supervivencia global y con base del estado del virus del papiloma humano (VPH). resultados: Incluimos 30 pacientes con media de edad de 57 años y media de seguimiento de 5 años: 22 hombres y 8 mujeres. Todos con etapas locorregionalmente avanzadas, 10 N3 y 20 N2. En 18 se efectuó determinación de p16 en los ganglios metastásicos, en 13 de ellos fue positivo y en 5, negativo. Siete pacientes fueron tratados con quimioterapia inicial y 23 con disección de cuello; la supervivencia a 5 años fue del 25%, el pronóstico fue mejor en pacientes p16(+). conclusión: El tratamiento de las metástasis cervicales depende de su resecabilidad; se debe favorecer la cirugía inicial. La quimioterapia neoadyuvante es una alternativa en pacientes con metástasis voluminosas, irresecables o limítrofes, el VPH es un factor de buen pronóstico. Palabras clave: Metástasis cervicales. El tipo de incisión cervical es fundamental para lograr exposición adecuada a los niveles ganglionares y poder realizar una disección de cuello (DC) completa y que estadifique adecuadamente al paciente; la disección incompleta subestadifica e incrementa el riesgo de recaída. La incisión debe además ofrecer la menor morbilidad, sin secuelas estéticas ni funcionales, debe de combinar la seguridad oncológica con una adecuada estética, funcionalidad y calidad de vida. Material y métodos: Estudio retrospectivo y observacional en el que se evaluó la incisión cervical transversa al conocer el número de ganglios disecados y sus complicaciones. Resultados: Cuarenta pacientes, 35 con metástasis de carcinoma epidermoide y 5 con metástasis de melanoma. La media de ganglios disecados fue de 25. Una sola incisión permitió la disección de los cinco niveles ganglionares, no se requirió convertir la incisión ni hacer ampliaciones verticales; el resultado cosmético fue satisfactorio en todos los pacientes, no hubo complicaciones mayores. Conclusión: La incisión cervical transversa permite acceso a los cinco niveles cervicales. En la presente serie la media de ganglios disecados fue de 25, número suficiente para considerar al procedimiento completo. El resultado estético es satisfactorio y no se presentaron complicaciones mayores asociadas a ella.
Background: The type of cervical incision is fundamental to achieve an adequate exposure to the lymphatic levels and to be able to perform a complete neck dissection and a proper staging of the patient; incomplete dissection sub-stages and increases the risk of recurrence. The incision must also offer the lowest morbidity, without aesthetic or functional sequelae; it must combine oncological safety with adequate aesthetics, functionality, and quality of life. Material and methods: A retrospective and observational study in which we evaluate the transverse cervical incision by knowing the number of dissected lymph nodes and complications rate associated to the incision. Results: Forty patients, 35 with squamous cell carcinoma metastasis and 5 with melanoma metastasis. The average of dissected lymph nodes was 25. A single incision allowed the dissection of the five ganglion levels; it was not necessary to convert the incision or make vertical enlargements; the cosmetic result was satisfactory in all patients, there were no significant complications. Conclusion: The transverse cervical incision allows access to the five cervical levels, in the present series the average of dissected lymph nodes was enough to consider the complete procedure. The aesthetic result was satisfactory, and there were no significant complications associated.
